Methodological problems in the determination of homolateral stapedius reflex threshold.

Generally, a bilateral symmetry of the acoustic stapedius reflex is assumed. But a homolaterally higher sensitivity has been described. We examined 71 patients with complete unilateral deafness. With a TDH 39 headphone, stimuli of intensities up to 133 dB were conveyed to the deaf ear. A fairly accurate estimate of the normal homolateral threshold is obtained by correcting for the cross-hearing loss and by mathematical consideration of the relative probabilities of the positive and negative cases. Methodology-specific influences are discussed and a correction made for them. The homolateral ref…

[Clinicophysiological studies of orbicularis oculi reflex in brain stem diseases].

Orbicularis oculi reflex is consisted of two components; one is early reflex response (R1) which is 14.1 +/- 2.2 msec in latency, and the other is late reflex response (R2) that is 35.0 +/- 8.6 msec in latency (mean +/- 2SD). R1 is observed on the ipsilateral side of stimulation and R2 is visible bilaterally on ipsiand contralateral sides of stimulation. Clinically orbicularis oculi reflex can be recorded easily, however, its clinical and physiological significance has not been clearly solved. Orbicularis oculi reflex was tested on patients with the brain stem lesions of various kinds, and the…

The blink reflex in coma and after recovery from coma.

Comparative studies of the blink reflex in a series of head injuries through the stages of coma and recovery from coma in a series of miscellaneous hemispheral lesions, and in a normal series, reveals that, although the principal centre for the R1 component of the blink reflex seems to be localised in the upper pons, the seat of the R2 late component is in the reticular system. The reappearance of this late component of the blink reflex in our cases of coma although seeming to depend on the integrity of the mesencephalic formation, correlates with the recovery of the patient's alertness. On th…

Vagal role and pacemaker indication in hypersensitive carotid sinus reflex.

The effect of carotid sinus pressure (CSP) on sinus rate was studied in 24 patients with sinoatrial pauses below 3 sec (control) and 23 patients with sinoatrial pauses above 3 sec (hypersensitive carotid sinus reflex equal to (HCSR). For registration of P waves, intraatrial electrograms were used. CSP was applied several times before and after atropine. Patients with HCSR showed inconstant response to CSP. The maximal result was considered diagnostic. No difference in the lengths of sinoatrial pauses was found between patients without dizziness or syncopes in the presence of HCSR and patients …

A Reflexivity Lab as a methodological tool in end-of-life research.

BACKGROUND: Reflexivity is essential in end-of-life research due to its ethical sensitivity, emotional intensity, and relational complexity, yet it is often left to individual initiative and remains weakly institutionalized in palliative care research settings. OBJECTIVE: To describe the design, implementation, and initial outputs of a Reflexivity Lab developed as a structured, collective reflexive method embedded within a palliative care research service. METHODS: A Reflexivity Lab was implemented in 2023 within the palliative and supportive care service of the Lausanne University Hospital,…

Relationship Between Spinal Reflexes and Spastic Movement Disorders.

The central nervous system (CNS) generates various motor functions, in part by modulating the excitability of spinal reflex pathways. When a reflex becomes not modulable or its modulation becomes impaired due to spinal cord lesions or other CNS diseases, it could become a part of spastic movement disorders. For the CNS with or without chronic lesions, thus, reflexes can be used as tools to probe its state. Utilizing such features of spinal reflexes, it is possible to operantly condition a reflex to improve CNS activity and resulting motor function in spastic individuals with spinal cord lesion…
