Lethal short-rib with median cleft and without polydactyly: a fourth case.

A new case of Beemer short-rib lethal dwarfism is presented in a 27 weeks fetus with discussion of the radiological and clinical pecularities. Differential diagnosis with other short-rib-polydactyly syndromes is briefly developped.

A surgical case of giant mediastinal tumor in a child associated with rib deformity.

A five-year-old boy was admitted, complaining of a persistent cough. Chest film showed a smooth round mass in right mediastinal region and deformity of 6th rib. CT studies revealed the presence of a well-defined mass with low density. Pulmonary arteriography demonstrated hypovascularity in right pulmonary field. Preoperative diagnosis was a benign neurogenic tumor originated from intercostal nerve located at the posterior mediastinum. At operation, a large round smooth mass was found adhered to chest wall and, in part, parietal pleura, and it was removed completely. The sixth rib with deformit

[A case of aneurysmal bone cyst arising from the left first rib].

A 30 year-old female was admitted to our department because of an abnormal chest radiograph. The chest CT scan revealed a tumor expanding from the left first rib. The tumor was resected through a left posterolateral thoracotomy. Histological findings showed aneurysmal bone cyst. Aneurysmal bone cyst is an uncommon lesion, especially in the rib. The patient's postoperative course was uneventful and there has been no evidence of recurrence during the 15 months since operation.

[Extractability of collagen from the rib cartilage and skin in funnel chest in children].

Content of collagen and relative content of unextractable collagen were increased by 35% and 50%, respectively, while the content of immobilized water was decreased in costal cartilage of children with the isolated form of funnel chest or with the deformation accompanied by Ehlers-Danlos syndrome. These anomalies may be considered as indicators of the cartilage premature ageing. In skin of the children with isolated form of funnel chest the ratio of collagen extracted with acetic acid was increased 2-fold.

The place of first rib resection in the management of axillary-subclavian vein thrombosis.

Eight patients with axillary-subclavian vein thrombosis were studied. Three presented acutely and five with chronic symptoms of pain and swelling exacerbated by and limiting normal use. Initial venography showed complete occlusion of the axillary vein in 7 cases and marked narrowing in the remaining patient. Venographic evidence of narrowing or occlusion was also seen in 6/8 clinically normal contralateral arms. One of these arms became symptomatic 2 years later, and required surgery. The patients presenting acutely were anticoagulated. Symptoms resolved slowly in 2 cases, but persisted in one

Ribbing's disease: radiographic-scintigraphic correlation and comparative analysis with Engelmann's disease.

Engelmann's disease is an uncommon condition characterized radiographically by symmetric diaphyseal sclerosis involving predominantly the tubular bones. Ribbing described an entity with similar but less extensive or severe changes which he thought differed sufficiently to constitute a distinct process. We present two cases of Ribbing's disease and one of Engelmann's disease, emphasizing the differences between the two. Radiographic-scintigraphic correlation is provided to demonstrate the value of skeletal scintigraphy in cases of Ribbing's disease and progressive diaphyseal dysplasia.

Rib regeneration area as an indicator of fusion area in adolescent idiopathic scoliosis.

This retrospective study of 24 patients with adolescent idiopathic scoliosis who underwent L-rod instrumentation, posterior thoracoplasty, and rib-bone grafting analyzed the correlation between the quality of rib regeneration and the host response to the rib-bone grafts. A planimetric technique was used to quantitate the coronal plane area of the resected ribs and of an L1-L2 concave fusion area immediately after surgery and at the end of the first postsurgical year. Using computerized regression analysis, these and other variables such as age, Cobb angle, and grafting technique were studied. …

The eleventh rib transcostal incision: an extrapleural, transperitoneal approach to the upper abdomen.

Subperiosteal resection of the eleventh rib with transcostal, extrapleural entrance into the retroperitoneum for exposure of the kidneys or adrenal glands has been well described in the urologic literature. We have used the incision with transperitoneal extension for various procedures in the upper abdomen. The following is a description of our operative technique with suggestions for procedures in which the incision may be used. The incision is ideal for splenectomy, distal splenorenal shunts, resections of the pancreatic body and tail, portacaval shunts, right hepatic lobectomy, and transper

Cranio-caudal rib cage distortion with increasing inspiratory airflow in man.

Changes in anteroposterior and transverse diameters of the chest wall, in cranial and caudal esophageal pressure (delta Pes), and diaphragm (Adi) and cranial intercostal muscle activity (Aic) were measured in supine and seated subjects during relaxation and full inspirations from FRC at constant slow or high flow rates. No distortion of the rib cage occurred during relaxation of slow inspirations. During fast inspirations cranial part of rib cage expanded relatively more than caudal one because of greater Aic for any level of Adi. Rib cage distortion during fast inspirations, as well as the oc

Winging of the scapula as a complication of first rib resection: a report of six cases.

Long thoracic nerve injury occurred in 6 patients as a complication of first rib resection by the transaxillary route for relief of thoracic outlet syndrome. In a review of 38 patients following 48 first rib resections, this is the second most frequent complication. This complication has rarely been reported in the literature. In 4 patients the long thoracic nerve palsy was transient, and complete recovery of serratus anterior function occurred within 6 months. In one patient, complete recovery followed repair of the long thoracic nerve.

Scalenectomy versus first rib resection for treatment of the thoracic outlet syndrome.

Five years ago a follow-up study of first rib resections disclosed a recurrence rate of over 15%. Many patients were reexplored supraclavicularly, and in every case the anterior scalene muslce was found to be reattached to the bed of the first rib. Scalenectomy invariably was successful, which led to this study of scalenctomy as the first operation for all cases of persistent thoracic outlet syndrome (TOS). The study revealed that most patients with TOS gave a history of neck trauma and had symptoms not only of paraesthesias of the hands and weakness of the arms, but also of neck pains and hea
