Arthroscopic suture bridge technique for intratendinous tear of rotator cuff in chronically painful calcific tendinitis of the shoulder.

Arthroscopic removal, now the main treatment option, has almost replaced open surgery for treatment of resistant calcific tendinitis. In some cases of chronic calcific tendinitis of the shoulder, the calcific materials are hard and adherent to the tendon. Removal of these materials can cause significant intratendinous tears between the superficial and deep layers of the degenerated rotator cuff. Thus far, there are no established surgical techniques for removing the calcific materials while ensuring cuff integrity. Good clinical results for rotator cuff repair were achieved by using an arthros

[Recurrent defects of the rotary cuff : Causes and therapeutic strategies].

Recurrent defects of the rotator cuff still represent the most common complication after rotator cuff repair. The etiology is multifactorial; however, predominantly biological determinants seem to influence the structural outcome. Basically, recurrent defects due to non-healing of the rotator cuff tendons have to be distinguished from true re-ruptures, which occur after primary successful osteofibroblastic integration. Different modes of failure are responsible for recurrent defects of the rotator cuff. The management of recurrent defects depends on the clinical symptoms of the patient, the ob

Hemiarthroplasty for the rotator cuff-deficient shoulder. Surgical technique.

BACKGROUND: Hemiarthroplasty is a common treatment for cuff tear arthropathy and glenohumeral arthritis associated with a massive tear of the rotator cuff; however, to our knowledge, long-term outcomes and preoperative factors affecting results have not been reported. METHODS: Thirty-four shoulders in thirty-one patients with cuff tear arthropathy or a massive rotator cuff tear with glenohumeral arthritis underwent hemiarthroplasty at an average age of seventy-two years. Outcomes in all patients were evaluated by direct physical examination and according to the limited goals criteria of Neer

Glenohumeral stability in simulated rotator cuff tears.

Rotator cuff tears disrupt the force balance in the shoulder and the glenohumeral joint in particular, resulting in compromised arm elevation torques. The trade-off between glenohumeral torque and glenohumeral stability is not yet understood. We hypothesize that compensation of lost abduction torque will lead to a superior redirection of the reaction force vector onto the glenoid surface, which will require additional muscle forces to maintain glenohumeral stability. Muscle forces in a single arm position for five combinations of simulated cuff tears were estimated by inverse dynamic simulatio

Supraspinatus tears: propagation and strain alteration.

It was hypothesized that there would be an alteration in strain when macroscopically normal supraspinatus tendons were subjected to three patterns of surgically created tear. The propagation of joint-side partial-thickness tears was also examined. Cadaveric shoulders were tested on a purpose-built rig with static loading from 20 to 200 N and during glenohumeral abduction from 0 degrees to 120 degrees with a 100-N tensile load. Differential variable reluctance transducers were used to calculate strain. Six-millimeter-wide midsubstance full-thickness tears (n = 2) caused an increase in bursal-si

Biomechanical comparison of effects of supraspinatus tendon detachments, tendon defects, and muscle retractions.

BACKGROUND: Rotator cuff ruptures are frequently associated with loss of strength of the shoulder. However, the characteristics of the rotator cuff tear that are responsible for the loss of force generation and transmission have not yet been identified. The purpose of this study was to compare the effects of supraspinatus tendon detachments, tendon defects, and muscle retractions on in vitro force transmission by the rotator cuff to the humerus. METHODS: The rotator cuff tendons from ten cadaver shoulders were loaded proportionally to the respective cross-sectional areas of their muscles. A f

The association between irregular greater tuberosities and rotator cuff tears: a sonographic study.

OBJECTIVE: Although an association between irregular greater tuberosities and rotator cuff tears has been suggested, no formal studies have been done. This study was designed to investigate the relationship between greater tuberosity irregularities, rotator cuff tears, and age. SUBJECTS AND METHODS: Sonographic examinations of both shoulders of 77 asymptomatic individuals (154 shoulders) between 30 and 80 years old were performed. The rotator cuffs and greater tuberosities were evaluated. Full- and partial-thickness rotator cuff tears were given equal significance. A two-tailed Fisher's exact

The value of ultrasonography in the diagnosis of labral lesions in patients with anterior shoulder dislocation.

OBJECTIVE: To evaluate the ability of ultrasonography to detect labral lesions in patients with anterior shoulder dislocation. METHODS: We examined 29 patients with anterior shoulder dislocation with ultrasonography prior to arthroscopy. RESULTS: Twenty-six patients had labral lesions diagnosed by arthroscopy. These lesions were detected by ultrasonography with a sensitivity of 88% and a specificity of 67%. All lesions affected the anterior labrum, while six extended to the posterior labrum. The latter were not visualized by ultrasonography. CONCLUSION: Ultrasonography is valuable in the de

The rotator cuff. Large and massive tears. Technique of open repair.

Symptomatic large and massive rotator cuff tears can be successfully managed with operative repair. Five factors that facilitate favorable results include: (1) performing an adequate subacromial decompression; (2) maintaining the integrity of the deltoid origin; (3) mobilizing the torn tendons, performing an interval slide when indicated; (4) repairing the tendons to bone; and (5) carefully staging and supervising the rehabilitation program.

Common Pickleball Injuries and Prevention.

Pickleball's rapid growth in the United States is accompanied by a significant rise in injuries, particularly among older adults. This review summarizes current evidence on the epidemiology, mechanisms, and risk factors associated with pickleball-related injuries. Falls represent the most common cause, with wrist fractures, rotator cuff tears, ankle sprains, and heat-related illnesses occurring frequently. Age, sex, higher body mass index, and inadequate physical conditioning increase the likelihood of injury. Practical prevention strategies are presented, including pre-participation health as

Shoulder injuries in the throwing athlete.

Shoulder injuries in the throwing athlete are becoming more frequent. Sports specialization at a younger age, playing multiple seasons, increased awareness of injury and injury prevention, advances in diagnosis, and surgical treatment all play a part in the increase in diagnosis of these injuries. Understanding the biomechanics of throwing and pathologies that are encountered in the throwing athlete can aid the clinician in successful diagnosis and nonoperative/operative treatment of the throwing athlete. This article discusses the relevant anatomy, biomechanics, and pathoanatomy of the throwi

Remodeling the Senescent Microenvironment for Promoting Osteoporotic Tendon-to-Bone Healing via Synergizing Senolytic Quercetin and Aligned Nanowire-Structured Hydrogels.

Osteoporotic tendon-to-bone healing remains a major challenge, as cellular senescence disrupts tissue regeneration and impairs repair outcomes. Although the role of cellular senescence in rotator cuff repair is increasingly recognized, current strategies often overlook the complex pathological context, particularly the dual impacts of senescence on both bone marrow-derived mesenchymal stem cells (BMSCs) and tendon-derived stem cells (TDSCs). This gap hampers effective tendon-to-bone healing and integration, especially under osteoporotic conditions. Herein, a composite hydrogel system, querceti

Development of a musculoskeletal shoulder model considering anatomic joint structures and soft-tissue deformation for dynamic simulation.

The shoulder joint has a high degree of freedom and an extremely complex and unstable kinematic mechanism. Coordinated contraction of the rotator cuff muscles that stop around the humeral head and the deltoid muscles and the extensibility of soft tissues, such as the joint capsule, labrum, and ligaments, contribute to shoulder-joint stability. Understanding the mechanics of shoulder-joint movement, including soft-tissue characteristics, is important for disease prevention and the development of a device for disease treatment. This study aimed to create a musculoskeletal shoulder model to repre

Internal impingement of the shoulder: a clinical and arthroscopic analysis.

Internal impingement was first described by Walch in 1992 and defined as contact between the supraspinatus tendon and posterior-superior glenoid rim with the shoulder in the cocked, throwing position of 90 degrees of abduction and maximum external rotation. The hypothesis of the study was that this contact may be seen in patients who are not throwing athletes nor in those who have instability. One hundred five consecutive patients who underwent shoulder arthroscopy were prospectively studied with preoperative history and physical examination. All patients underwent general anesthesia and arthr
