[Experience with the Ender hooked plate in the management of 42 scaphoid pseudarthroses].

To treat the pseudarthrosis of scaphoid, there exist many operative procedures. We describe the method of H. G. Ender: A hook plate fixes the main bone fragments and exerts a pressure on the in-between lying bone transplant. We report on 42 patients operated on using the Ender-plate. In 39 cases a follow-up was possible. 95% of the patients had a consolidation of the bone, in two cases a further operation was required. 36 patients complained preoperatively of pain during exercise and only seven patients had the same complaints after the operation. In some cases, a minor reduction of the motion

Long-term results of scaphoid nonunion surgery: 50 patients reviewed after 8 to 18 years.

OBJECTIVES: Untreated scaphoid nonunions are a hazard to the wrist, resulting in deteriorating function and radiologic degenerative changes with increasing time. Long-term results after surgery of scaphoid nonunion reporting clinical, radiologic, and subjective outcomes are scarce. DESIGN: Retrospective follow-up study. PATIENTS: From 1990 to 1998, 53 patients were operated on for persistent scaphoid nonunion; three patients were excluded from the follow-up (one expelled foreigner, another died of an unrelated cause, one never appeared after surgery), leaving 50 patients eligible for follow-

Patient-reported outcomes and comprehensive assessment of wrist function after scaphoid reconstruction: A single-center retrospective study on 162 patients.

A vast body of literature emphasizes the use of radiological data for assessing outcomes in scaphoid reconstruction. The goal of this single-center study was to examine patient-reported outcomes and wrist function after scaphoid reconstruction. A total of 162 of 370 patients who underwent scaphoid reconstruction between January 2007 and December 2020 were included in the study. Analyses were conducted using R version 4.0.2 with a significance level at p ≤ 0.05. The clinical follow-up averaged 7.44 years (range 1.03-14.65), with 87% of patients achieving union. The average Disability of the Arm

Radial nerve mobilization decreases pain sensitivity and improves motor performance in patients with thumb carpometacarpal osteoarthritis: a randomized controlled trial.

OBJECTIVE: To examine the effects of radial nerve mobilization on pain sensitivity and motor performance in subjects with secondary thumb ca … tis. We measured pressure pain threshold (PPT) at the trapeziometacarpal joint, the tubercle of the scaphoid bone, and the unciform apophysis of the hamate bone by algometry. Tip pinch strength and tripod pinch strength were measured by a mechanical pinch gauge. RESULTS: Treatment increased PPT by 3.33±.24 kg/cm(2) (P<.001) in the trapeziometacarpal joint and …

[Bilateral congenital fusion of the scaphoid and the trapezium. A case report].

Congenital synostosis has been described between nearly all of the carpal bones. It can occur in an isolated form or most commonly in relation between the lunatum and the triquetrum. It can also occur in a multiple form and be associated with other malformations. We report a rare case of scaphoid-trapezium fusion observed bilaterally in a nearly asymptomatic 63-year-old patient.

Three-Dimensional In Vivo Kinematic Analysis of Kienböck Disease Treated with Arthroscopic Lunate Excision.

PURPOSE: Treatment options for advanced stage Kienböck disease primarily involve lunate excision, either in the context of proximal row carpectomy or in association with partial wrist fusion. This study aimed to investigate the in vivo three-dimensional carpal kinematics following arthroscopic lunate excision. METHODS: We analyzed carpal translation and rotation of the affected wrist in five patients who underwent arthroscopic lunate excision for stage IIIC Kienböck disease. Our dynamic computed tomography image-based analysis focused on determining the centroid position of the scaphoid, triq

Arthroscopy-Assisted Scapholunate Reconstruction With Internal Brace Augmentation.

Various surgical techniques exist to repair or reconstruct complete scapholunate (SL) interosseous ligament tears, including capsulodesis, static or dynamic tenodesis, ligament reconstruction with tendon graft, bone-retinaculum-bone reconstruction, and the reduction and association of the scaphoid and lunate (RASL) procedure. The choice of surgical technique depends on arthroscopic assessment using the Geissler classification and European Wrist Arthroscopy Society staging of SL injury. This article describes arthroscopy-assisted extracapsular SL reconstruction using free tendon graft and inter

A Rare Complete Metacarpal Pseudoepiphysis in a Teenager With a Scaphoid Fracture.

A 13-yr-old adolescent boy presented with wrist pain after falling off a scooter onto his outstretched hand. Radiographs revealed a nondisplaced hairline fracture of the scaphoid bone and an irregular radiolucent line in the proximal metaphysis of the second metacarpal bone, consistent with an anomalous growth plate, or complete pseudoepiphysis. Complete pseudoepiphysis is a rare finding, with only a few cases reported. Learning about the common locations of growth plates and the radiographic differences between normal, injured, and anomalous growth plates can help decrease physician error and…

Comparison of conventional and transtrapezial palmar approaches for screw fixation of scaphoid waist fractures: a clinical study.

We investigated two palmar approaches for screw fixation of acute scaphoid waist fractures: the conventional percutaneous or transtrapezial approach. Thirty cases who underwent operation from 2013 to 2021 were reviewed (conventional group, 15; transtrapezial approach group, 15). Cross-sections were constructed along the long axis of the scaphoid on postoperative computed tomography to evaluate the screw position, relative to the centre point in the distal-third, midwaist and proximal-third of the bone. The screw could be inserted centrally in the proximal and distal regions using the transtrap

Scaphoid Osteoid Osteoma in a Pediatric Patient: A Case Report and Review of Diagnostic Pitfalls and Management Strategies.

Osteoid osteoma (OO) of the scaphoid is an uncommon entity, particularly in pediatric patients, where diagnosis is often delayed due to its rarity and overlapping presentation with more common wrist pathologies. We report the case of a pediatric athlete with persistent wrist pain initially diagnosed as a scaphoid fracture. Imaging progression from radiographs to MRI suggested fracture, but confirmatory CT revealed a 5-mm radiolucent lesion with central mineralization, consistent with OO. Surgical excision via volar approach was performed due to intraosseous lesion location and proximity to art

Retrograde Herbert screw fixation for treatment of proximal pole scaphoid nonunions.

Most nonunions of the carpal scaphoid bone can be treated with a high rate of success by use of conventional bone grafting techniques. However, fractures with a small proximal pole fragment may be difficult to treat by use of these techniques. Nine patients with nonunion and three patients with unstable proximal pole fractures were treated with retrograde dorsal Herbert screw fixation and adjunctive bone grafting. Follow-up averaged 25 months. Of the 12 patients, the fracture healed in 11 and one fracture remained ununited. This technique has been successful in our practice and should be consi…

Pediatric scaphoid fractures: predictors of surgery and fracture complications.

OBJECTIVES: To identify clinical and radiographic findings of pediatric scaphoid fractures that predict the need for surgical treatment. METHODS: A retrospective review of pediatric patients (≤ 18 years) with scaphoid fractures, who underwent radiographic examination and treatment at our tertiary care pediatric hospital, between 2018 and 2024, identified all surgically treated patients. From the remaining conservatively treated patients, age matched comparisons were randomly selected. After randomization and blinded to outcome, skeletal age, fracture characteristics (location, displacement, c

Additively manufactured polyethylene terephthalate scaffolds for scapholunate interosseous ligament reconstruction.

The regeneration of the ruptured scapholunate interosseous ligament (SLIL) represents a clinical challenge. Here, we propose the use of a Bone-Ligament-Bone (BLB) 3D-printed polyethylene terephthalate (PET) scaffold for achieving mechanical stabilisation of the scaphoid and lunate following SLIL rupture. The BLB scaffold featured two bone compartments bridged by aligned fibres (ligament compartment) mimicking the architecture of the native tissue. The scaffold presented tensile stiffness in the range of 260 ± 38 N/mm and ultimate load of 113 ± 13 N, which would support physiological loading. A
