BACKGROUND: Brucellar spondylitis (BS) is a focal osteoarticular manifestation of brucellosis with nonspecific clinical and imaging features. Diagnosis may be difficult in areas where brucellosis is not traditionally considered endemic, particularly when epidemiologic exposure is not identified and serologic tests yield discordant results. We report a case in which metagenomic next-generation sequencing (mNGS) of vertebral lesion tissue provided additional microbiologic evidence for the diagnosis.

Histoplasmosis is an endemic disease in many regions of the United States. Physicians must be aware of the clinical syndromes and take advantage of epidemiologic clues when diagnosing histoplasmosis pericarditis. Clinicians must also be familiar with the uses and limitations of a battery of serologic and mycologic tests. Finally, further clinical trials are needed to study the role of antifungal therapy in the treatment of histoplasmosis pericarditis.

CT and MRI of the brain were performed in an acutely ill child with serology-proven eastern equine encephalitis. The studies showed lesions involving the basal ganglia and thalami according with findings described in the literature. In the correct clinical setting, these findings should prompt the obtaining of appropriate serologic confirmatory tests and lead to institution of control measures to prevent spread of the disease.

We present a case report of leptospirosis with acute renal failure and hepatic dysfunction. Aspecific clinical presentation and serologic tests do not help therapeutic choices. The patient received 18 sessions of hemodialysis and early treatment with intravenous ceftazidime 1 g tid (under the impression of Gram- sepsis). Third generation cephalosporins, ceftriaxone and cefotaxime, are highly active against Leptospira [10] while clinical experience with ceftazidime is lacking.

Encephalitic illnesses with Epstein-Barr virus (EBV) only represent a small percentage of the causes of viral encephalitis. Clinical symptoms are not specific. Biological standards tests carry few elements of direction, only serologic test of infectious mononucleosis confirms the diagnostic of recent EBV infection. Previous observation shows that the initial study can present similarities with herpes simplex encephalitis, which forces the immediate start of treatment by Acyclovir.

Recurrent rectal bleeding over a period of seven and 12 months, respectively, was the main and unusual manifestation of amebic colitis in two patients. The difficulty in establishing the diagnosis led to one patient's receiving potentially dangerous steroid therapy. Only by taking multiple colonoscopic biopsies was the amebic colitis recognized. This and serologic tests, should be routine in all cases of inflammatory bowel disease before initiating therapy.

A Q fever epidemic focus with 46 cases was detected in a rural locality among the workmen of livestock farms. In 33 cases, the subclinical and inapparent form of the disease was detected by serologic tests, performed in the focus. Complement fixation antibody titers persisting in very high titers, a retrospective diagnosis of infection may be established even 3-4 months after the onset of the disease.

The frequency of appendicular bone abnormalities was prospectively evaluated in 93 newborns suspected of having congenital syphilis. The evaluation of each newborn included prenatal history, physical examination, serologic tests, and long bone radiography, which was performed during the first week after birth.
