[Treatment approach to apnestic breathing in a patient with vocal cord paralysis due to Arnold Chiari malformation and tracheostomy].

Arnold Chiari malformation (ACM) is herniation of brain stem structures, especially cerebellar tonsils, from the foramen magnum into the cervical spinal canal. Sleep-related respiratory disorders are reported at a higher rate in this patient group than the general population, at about 60-75%. A 43-year-old male patient was diagnosed with ACM type 1.5 at the center where he applied with complaints of weakness in the extremities, speech disorder and syncope triggered by coughing, and a decompression of posterior fossa (PFD) was applied to the patient. Since 2018, the apnea attacks, which were ab

Stay-at-home circumstances do not produce sleep disorders: An international survey during the COVID-19 pandemic.

OBJECTIVE: The anxiety-related insomnia and other sleep disorders were mentioned as possible side effects of quarantine and stay-at-home conditions. The questions to be explored were: Are there discernable differences in hours of sleep and sleep habits between the normal operational environment and the stay-at-home condition? and How seriously anxiety-induced insomnia or other sleep disorders may affect individuals during the stay-at-home? METHODS: This international prospective study analyzed results from the sleep-wake patterns questionnaire, daily logs, and interviews. During COVID-19 pand

[Hypersomnia and thalamic and brain stem stroke: a study of seven patients].

BACKGROUND AND AIM: Thalamic and brainstem strokes are a cause of organic hypersomnia. In thalamic lesions it has been attributed to disruption of ascending activating impulses from the brainstem reticular formation and to insufficient spindling and slow-wave production, which depends upon activities of reticular thalamic nucleus and thalamocortical neurons, respectively. Reported sleep disorders in brainstem lesions have occasionally been contradictory and that is because of the presence of nearby structures in the brainstem with different functions in sleep-waking cycle. The aim of the study

The benefit and neural mechanisms of computerized inhibitory control training for insomnia with short sleep duration phenotype: a rs-fMRI study.

BACKGROUND: Inhibitory control (IC) impairment is characteristic of insomnia disorder with short sleep duration (ISSD), but not with normal sleep duration (INSD). IC is critical for sleep-wake regulation. This study evaluates whether computerized IC training can improve sleep in ISSD and explores related neural mechanisms using resting-state fMRI (rs-fMRI). METHODS: Twenty ISSD patients participated in a three-week computerized IC training program (15 sessions), alongside a control group of 17 participants. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) and the Ins

Dynamic changes in sleep architecture in a mouse model of acute kidney injury transitioning to chronic kidney disease.

INTRODUCTION: Sleep disorders are common in individuals with kidney failure. Whether kidney impairment is the direct cause of sleep abnormalities is unclear, however, partly due to confounding factors including comorbidities, dialysis, and drugs. METHODS: Here, we used a mouse model of acute kidney injury (AKI) transitioning to chronic kidney disease (CKD) induced by aristolochic acid to examine the effects of kidney impairment on sleep architecture. Each group, comprising 8~10 male mice, underwent cortical electroencephalogram (EEG) and electroencephalogram (EMG) recordings to measure sleep

Treatment of insomnia in Parkinson's disease: a controlled trial of eszopiclone and placebo.

Parkinson's disease (PD) is a common neurodegenerative disease affecting up to 1 million individuals in the United States. Sleep disturbances, typically in sleep maintenance, are found in up to 88% of these individuals and are associated with a variety of poor outcomes. Despite being common and important, there are few data to guide clinical care. We conducted a 6-week, randomized, controlled trial of eszopiclone and placebo in 30 patients with PD and insomnia. Patients with other primary sleep disorders (PSG defined) were excluded. The primary outcome was total sleep time (TST), and secondary

Exploding head syndrome's clinical features and an evaluation of polysomnographic findings.

BACKGROUND: Exploding head syndrome (EHS) is defined by the sudden onset of a short-term loud sounds or explosion-like sensation in the head during sleep, or sleep-wake transitions. OBJECTIVE: To evaluate the clinical characteristics and polysomnographic (PSG) findings of patients diagnosed with EHS. METHODS: The files of patients with an EHS diagnosis were retrospectively examined. Patients' clinical characteristics; PGS data; and results from the Epworth Sleepiness Scale (ESS), Pittsburgh Sleep Quality Index (PSQI) and Beck Depression and Anxiety Inventory were evaluated. Also, PSG finding

The clinical diagnosis of the narcoleptic syndrome.

Sleep-wake habits and control of postural muscle tone were investigated by self-report questionnaire in 183 subjects considered to have the narcoleptic syndrome, 62 subjects with hypersomnia and 10 with obstructive sleep apnoea. Results were compared with those in a group of 188 control subjects with normal sleep wake habits. Excessive daytime sleepiness, determined by the Epworth Sleepiness Scale (ESS), was five times greater in the narcoleptic syndrome than in control subjects (score range 0-24, mean scores +/-SD 19.6+/-3.0; and 4.5+/-3.3 respectively; P<0.001). The propensity to cataplexy,

A systematic review to explore the feasibility of a behavioural sleep intervention for insomnia in children with neurodevelopmental disorders: A transdiagnostic approach.

Children with neurodevelopmental disorders (NDD) are at high risk for sleep problems, especially insomnia. It is currently not known whether behavioural sleep interventions developed for typically developing (TD) children are effective for children with NDD, and if interventions need to be modified for each diagnostic group. The aim of this systematic review was to identify and evaluate commonalities, trends in outcomes, and the methodological quality of parent-delivered behavioural sleep interventions for children with NDD, specifically Attention-Deficit/Hyperactivity Disorder (ADHD), Autism

Sleep latency and duration estimates among sleep disorder patients: variability as a function of sleep disorder diagnosis, sleep history, and psychological characteristics.

STUDY OBJECTIVES: Insomnia patients often report greater sleep disturbance than found via polysomnography; yet the specific patient factors related to such sleep time misperceptions are poorly understood. We sought to characterize the extent to which a diverse group of patients complaining of insomnia (n=104) misperceive overnight total sleep time and sleep latency, and to identify patient factors associated with these variations. DESIGN: Cross-sectional. SETTING: University based sleep disorders center. PATIENTS: Sleep disorder groups consisted of patients with psychophysiological insomnia

Waking Up to the Issue! Research Inattention and Sex-Related Differences Warrant More Sleep Studies in Female Athletes.

Understanding sleep patterns and behaviors of athletes is essential for developing targeted sleep-based interventions for implementation in practice. However, more than double the number of sleep studies have examined male athletes compared with female athletes, making the current understanding of sleep patterns, behaviors, and interventions among athletes disproportionately indicative of men. Consequently, this review demonstrates the need for more female-specific sleep data among athlete populations due to research inattention and sex-related differences. Specifically, this review identifies

The effects of ketogenic dietary therapies on sleep: A scoping review.

Sleep problems are common in neurological conditions for which ketogenic dietary therapies (KDTs) are recognised as an effective intervention (drug-resistant epilepsy, autism spectrum disorder, and migraine). Given the composite framework of action of ketogenic dietary therapies, the prevalence of sleep disturbance, and the importance of sleep regulation, the present scoping review aimed at identifying and mapping available evidence of the effects of ketogenic dietary therapies on sleep. A comprehensive web-based literature search was performed retrieving publications published to June 2023 us

A challenging case of sexsomnia in an adolescent female presenting with depressive-like symptoms.

UNLABELLED: Sleep-related sexualized behaviors occur in the parasomnia known as sexsomnia, recognized as a variant of confusional arousals in the International Classification of Sleep Disorders, third edition. These instinctive behaviors of a sexual nature emerge from deep non-rapid eye movement sleep, and patients often present with distinguishing features within this sleep disorder category. There are often adverse psychosocial consequences and not uncommonly medicolegal implications. While associations to psychiatric consequences from the sexsomnia have been demonstrated and efforts to furt

[Sleep medicine in psychiatry and psychotherapy].

Sleep medicine is a cross-disciplinary subject with high relevance for psychiatry and psychotherapy. The three most common sleep disorders (insomnia, sleep apnea syndrome and restless legs syndrome) are presented here with practical relevance. Cognitive behavioral therapy is the treatment of choice for insomnia, whereas symptomatic drug treatment (especially with GABA receptor agonists) should be used with caution. Sleep-related breathing disorders have a high prevalence of around 24% among psychiatric inpatients in Germany. Typical symptoms of obstructive sleep apnea syndrome should lead to a

Daily Associations Between Adolescent Sleep and Socioemotional Experiences During an Ongoing Stressor.

PURPOSE: Ongoing stress can result in sleep disturbances and daytime socioemotional difficulties. Data on how sleep and daytime socioemotional functioning may be bidirectionally related to one another in the midst of an ongoing stressor are limited, particularly during adolescence, a developmental period when risk for the onset of mental health difficulties and sleep disturbances is high. METHODS: Participants (N = 459, ages of 13-18 years) were recruited from across the United States and completed an intake survey and one week of daily reports beginning 2 weeks after COVID-19 was declared a

Beyond Stress: Altered Sleep-Wake Patterns are a Key Behavioral Risk Factor for Acute Insomnia During Times of Crisis.

BACKGROUND: Stress is a common precipitant of acute insomnia; however, reducing stress during times of crisis is challenging. This study aimed to determine which modifiable factors, beyond stress, were associated with acute insomnia during a major crisis, the COVID-19 pandemic. PARTICIPANTS/METHODS: A global online survey assessed sleep/circadian, stress, mental health, and lifestyle factors between April-May 2020. Logistic regression models analyzed data from 1319 participants (578 acute insomnia, 731 good sleepers), adjusted for demographic differences. RESULTS: Perceived stress was a sign

An alternative to the multiple sleep latency test for determining sleepiness in narcolepsy and hypersomnia: polygraphic score of sleepiness.

This article describes a quantitative assessment of pathological diurnal sleepiness in three groups of patients with excessive daytime sleepiness: narcoleptic patients, idiopathic hypersomniac patients, hypersomniac patients with sleep apnea syndrome. We analyzed polygraphic diurnal recordings of 45 min duration obtained under standardized conditions. We called the percentage of total sleep time during the 45-min recording the polygraphic index of sleepiness. The polygraphic score of sleepiness is determined by the latencies and total durations of the individual sleep stages. Because deeper sl
