Anteromedial Petrous (Gardner's) Triangle: Surgical Anatomy and Relevance for Endoscopic Endonasal Approach to the Petrous Apex and Petroclival Region.

BACKGROUND AND OBJECTIVES: Triangular corridors have been used as reliable surgical entry points for open transcranial approaches to the pet … role to safely define the triangle were the pterygosphenoidal fissure, the petrosal process of the sphenoid bone, and the petroclival synchondrosis. CONCLUSION: The anteromedial (Gardner's) triangle is a well-defined bone corridor which provides access to the entire petrous bone and petroclival junction through endoscopic endonasal route. Regardless of the an …

Impact of removal of the lateral orbital rim on intraorbital pressure during endoscopic trans-orbital approach (ETOA): a cadaveric study.

BACKGROUND: The endoscopic transorbital approach (ETOA) is being established as a new corridor for the lateral portion of the anterior and middle skull base. One of the main concerns is the risk of ophthalmological complications due to orbital retraction. Removal of the lateral orbital rim (LOR) is a simple measure that widens the corridor and may diminish injuries secondary to retraction. This study analyzes the impact of LOR removal on intraorbital pressure (IORP) during the stages of ETOA. METHODS: In this prospective cadaveric study, standard ETOA to the anterior and middle fossae were pe

Surgical limits in transnasal approach to opticocarotid region and planum sphenoidale: an anatomic cadaveric study.

BACKGROUND: The significance of medial and lateral opticocarotid recesses and the planum sphenoidale region in skull base pathologies for the transsphenoidal-transplanum approach were evaluated. METHODS: The sphenoid bone block samples were extracted from adult cadavers. Dissections and measurements in the opticocarotid and planum sphenoidale regions were performed in 29 samples using a surgical microscope. For histologic evaluation, oblique sections through the bilateral opticocarotid regions were obtained and examined in eight samples. RESULTS: Optic, carotid prominences, and medial and la…

One-piece frontoorbital advancement with distraction but without a supraorbital bar for coronal craniosynostosis.

Traditional frontoorbital advancement with a supraorbital bar is the standard technique for correcting coronal craniosynostosis. However, se … ation of the dural attachment. The 1.5 cm burr hole allowed visualization of the greater and lesser sphenoid bone wings, which is necessary for a safe osteotomy. The average length of distractions was 17 mm. This approach was less invasive than the traditional approach and resulted in satisfactory correction. Transfusions were not required for six patients, whi …

Morphological relationship between the cranial base and dentofacial complex obtained by reconstructive computer tomographic images.

The aims of this study were to investigate the relationships between the cranial base, including the glenoid fossa and maxillofacial morphology, obtained by three-dimensional (3D) computed tomography (CT). The specimens were 45 Skeletal I and Skeletal II dry skulls of modern Japanese males without marked crowding, anterior crossbite, or maxillofacial asymmetry, which had been preserved in the Tokyo University Museum. To examine the differences in the cranial base and maxilla between two groups classified by the median value of N line-A [N defined as a perpendicular line to Frankfort horizontal

Subgaleal versus subperiosteal dissection in fronto-orbital advancement procedures.

Extensive cranioorbital surgery for craniosynostosis in young children can produce excessive blood loss. Thus, to overcome the disadvantage of unappropriated blood loss and translocation of the osteosynthesis material, we performed a subgaleal dissection in infants and compared the subperiosteal with the subgaleal surgical approach in 29 children who underwent surgery for craniosynostosis. The treatment goal of correction of the deformity was accomplished in each case. Fronto-orbital deformities were significantly improved in all patients. Blood loss was less in the group with subgaleal elevat

Defining genetic and physiopathological bases of orbital hypertelorism: about a systematic literature review.

Orbital hypertelorism is observed in a wide spectrum of congenital disorders, such as in case of frontonasoethmoidal tumors or encephalocele … ing. Perinatal disturbances of cranial suture fusion or skull base development, particularly of the sphenoid bone and cranial base synchondroses, contribute to hypertelorism in syndromic craniosynostoses and some RASopathies. Advances in molecular genetics have identified numerous genes implicated in these processes. However, establishing genotype-phenotype cor …

Acute Vision Loss in Patients With Allergic Fungal Rhinosinusitis: A Case Series.

Allergic fungal rhinosinusitis (AFRS) is a subtype of chronic noninvasive sinusitis accounting for 7.8% (0.2%-26.7%) of all chronic rhinosinusitis cases. A definitive diagnosis is usually made after sinus surgery. Successful treatment requires a combination of surgical and medical management. Although orbital involvement is relatively common, reports on optic neuropathy and acute vision loss are limited. Herein, we present a series of 3 patients with AFRS who presented with acute visual loss as the chief complaint. All 3 patients were otherwise healthy adults in their early 20s with extensive

Uncommon presentation of an intradiploic orbital epidermoid tumor: case report.

PURPOSE: Epidermoid tumors are uncommon entities that usually occur intradurally in the cerebellopontine angle and parasellar cisterns. The few cases that have been reported to occur in the intraorbital location involve patients in the first four decades of life. To our knowledge, an elderly person presenting with an orbital epidermoid tumor has never been reported. METHODS: A 67-year-old man presented with left-sided headache and eye pain. He was found to have a hemorrhagic lesion eroding bone of the greater and lesser wings of the sphenoid that was compressing the frontal and temporal lobes

Intracranial Myeloid Sarcoma as the First Manifestation of Acute Monoblastic Leukemia.

Background Myeloid sarcoma, also called granulocytic sarcoma, extramedullary myeloid tumor, or chloroma, is a manifestation of a myeloid neoplasm. These tumors can develop in various anatomical sites, including the central nervous system, presenting as an extra-axial tumor affecting the meninges or the ependyma. Diagnosis is challenging due to its resemblance to other intracranial tumors such as meningiomas. Case Description We present the case of a 17-year-old male with intermittent left frontotemporal headache, diplopia during horizontal gaze, and proptosis of the left eye. During the imagin

Preoperative Embolization for Sinonasal Angiosarcoma Optimized Intraoperative and Postoperative Outcomes: A Case Report.

BACKGROUND: Angiosarcoma is a rare and aggressive malignancy, representing less than 1% of head and neck cancers. Primary sinonasal angiosarcoma is exceedingly uncommon, with fewer than 40 cases reported. It carries a poor prognosis due to early recurrence and metastatic potential, and no standard treatment exists. CASE REPORT: A 33-year-old male presented with a 2-year history of progressive nasal obstruction, later accompanied by epistaxis, facial pain, and headache. Imaging revealed a large vascular sinonasal mass extending into the nasopharynx, sphenoid, and ethmoid sinuses with bone dest

Ectopic craniopharyngiomas.

Craniopharyngiomas (CPs) are rare embryonic malformational tumours of the sellar/parasellar region, classified by the World Health Organization (WHO) 5th edition as grade 1 tumours. They may arise anywhere on the path of embryonic cell migration between the midline sphenoid bone and the floor of the sella turcica. On rare occasions, they can be found remotely in areas like the nasopharynx, the orbit, the posterior fossa, the brain parenchyma and the spine. Such ectopic CPs may present as primary in an atypical location, or as secondary, which represent distant recurrence after initial surgical…

Giant ethmoid osteoma originated from the lamina papyracea.

INTRODUCTION: Osteomas are slow- growing, benign tumors. They are the most common neoplasms of the paranasal sinuses. They are usually originates from the frontal and ethmoid sinus and much less frequently seen in the maxillary and sphenoid sinuses. Although the lamina papyracea is a part of ethmoid bone, a giant osteoma originated from the lamina papyracea is very uncommon. An osteoma of the paranasal sinus is usually asymptomatic. Headache, proptosis, epiphora, diplopia, dizziness, facial deformity, face pain and cerebral complications are possible symptoms. The treatment of the paranasal os
