Factors influencing prioritization for carpal tunnel syndrome consultation.

BACKGROUND: In a socialized medicine model, prioritization of referrals for specialist consultation is highly important in the distribution of heath care. For high-burden diseases, such as carpal tunnel syndrome (CTS), the factors that influence prioritization are not well understood. OBJECTIVE: To determine the factors that influence the prioritization of referrals for CTS consultation by plastic surgeons in Canada. METHODS: All members of the Canadian Society of Plastic Surgery with e-mail addresses were invited to participate in an online survey regarding the method by which they prioriti

Practice profiles in breast reduction: a survey among Canadian plastic surgeons.

BACKGROUND: Breast reduction is an increasingly common procedure performed by Canadian plastic surgeons. Recent studies in the United States show that use of the inferior/central pedicle inverted T scar method is predominant. However, it is unknown what the practice preferences are among Canadian plastic surgeons. OBJECTIVE: The goal of the present study was to assess trends in breast reduction surgery among Canadian surgeons, including patient selection criteria, surgical techniques and outcomes. METHOD: Surveys were distributed to plastic surgeons at the Canadian Society for Plastic Surger

Complex Wound Closure in Adult Spinal Deformity Surgery Reduces Complications in High-Risk Patients.

BACKGROUND: Adult spinal deformity (ASD) surgery is becoming increasingly prevalent. Soft tissue defects arising from revision closure and impaired healing can predispose to wound complications including dehiscence and infection. Soft tissue coverage with local muscle flaps has been shown to minimize wound complications in high-risk patients. In this study we evaluate the role of complex wound closure in preventing wound complications in high-risk spinal deformity patients. METHODS: The authors retrospectively reviewed charts of patients who underwent ASD surgery. Patients were stratified int

Single-specialty management and reconstruction of necrotizing fasciitis of the upper extremities: clinical and economic benefits from a case series.

PURPOSE: Patients with necrotizing fasciitis are managed with multiple prompt, radical surgical debridements and critical care support. Debridement and reconstruction are often provided by different surgical teams. Anecdotally, single-specialty management seemed to be a more efficient management strategy. This study aimed to investigate and compare the outcomes of management by plastic surgery versus multiple disciplines through a retrospective economic and clinical analysis of patients with necrotizing fasciitis treated over 8 years. We also present 3 index cases for which our service functio

The Drivers of Academic Success in Cleft and Craniofacial Centers: A 10-Year Analysis of over 2000 Publications.

BACKGROUND: Cleft and craniofacial centers require significant investment by medical institutions, yet variables contributing to their academic productivity remain unknown. This study characterizes the elements associated with high academic productivity in these centers. METHODS: The authors analyzed cleft and craniofacial centers accredited by the American Cleft Palate-Craniofacial Association. Variables such as university affiliation; resident training; number of plastic surgery, oral-maxillofacial, and dental faculty; and investment in a craniofacial surgery, craniofacial orthodontics fell

Survey of Current Practice Patterns in the Management of Frontal Sinus Fractures.

The management of frontal sinus fractures has evolved in the endoscopic era. The development of functional endoscopic sinus surgery (FESS) has been incorporated into management algorithms proposed by otolaryngologists, but the extent of its influence on plastic surgeons and oral and maxillofacial surgeons is heretofore unknown. A cross-sectional survey was performed to assess the practice pattern variations in frontal sinus fracture management across multiple surgical disciplines. A total of 298 surveys were reviewed. 33.5% were facial plastic surgeons with otolaryngology training, 25.8% gener

Glucagon-Like Peptide-1 Agonists: A Practical Overview for Plastic and Reconstructive Surgeons.

BACKGROUND: Glucagon-like peptide-1 (GLP-1) agonists, such as exenatide, liraglutide, dulaglutide, semaglutide, and tirzepatide, effectively manage type 2 diabetes by promoting insulin release, suppressing glucagon secretion, and enhancing glucose metabolism. They also aid weight reduction and cardiovascular health, potentially broadening their therapeutic scope. In plastic surgery, they hold promise for perioperative weight management and glycemic control, potentially impacting surgical outcomes. METHODS: A comprehensive review was conducted to assess GLP-1 agonists' utilization in plastic s

[Expert consensus on the clinical diagnosis and treatment of Congenital syndactyly (2025 Edition)].

Syndactyly is one of the most common congenital malformations of the hand/foot, characterized by varying degrees of soft tissue and/or skeletal fusion between adjacent fingers or toes. It may also occur alongside polydactyly, camptodactyly, brachydactyly, or congenital joint fusion. The heterogeneity in its clinical phenotypes and incomplete penetrance makes it challenging to establish clear diagnostic and treatment protocols/procedures. Conventionally, the management of syndactyly has mainly relied on clinical experience, with limited reference to cutting-edge genetic research and systematic

Arthur Rainsford Mowlem (1902-1986), plastic surgeon.

Arthur Rainsford Mowlem, the junior of the 'big four' plastic surgeons, with Harold Delf Gillies, Thomas Pomfret Kilner and Archibald Hector McIndoe, came to prominence managing casualties of the Second World War. Rainsford Mowlem's ancestor was John Mowlem, the creator of the construction firm. Rainsford worked his passage to the United Kingdom from New Zealand in 1927 and did not return to New Zealand but retired at the age of 60 to enjoy 23 more years in Spain. He was the driving force between 1934 and 1939 at the Plastic Surgery Unit at St James's Hospital, Balham, and instigated the North

Psychological and Psychiatric Traits in Post-bariatric Patients Asking for Body-Contouring Surgery.

BACKGROUNDS: Obese patients, mainly females, feel uncomfortable and unsatisfied with their physical appearance; they have a wrong perception of their image and consequently diminish their self-esteem, sometimes showing difficulties in functional areas such as work, relationship, social activity. Beside health concerns, improving their appearance and body image are often common motives for weight loss in obese individuals and after weight loss about 30% of bariatric surgery patients undergo plastic surgical correction of excessive skin. The authors investigated psychological and psychiatric tra

[Repair of nerve defects with autotransplants].

Over 15 years (1985-1999) 60 plastic reconstructions of nerve defects with autograft were performed after peripheral nerves injuries of the upper limbs (42 patients), lower limbs (14) and face (4). It is demonstrated that efficacy of surgery depends on period from injury to surgery, age of patient, length of autograft, nature and location of injury, kind of injured nerve. Better results were achieved after operations on peripheral nerves of the upper limbs. After operations on lower limbs more favorable results were achieved in repair of tibial nerve defects. Plastic surgery of facial nerve de

Craniosynostosis surgery practice patterns in the United States: what are we doing and how are we doing it?

OBJECTIVE: The surgical management of craniosynostosis varies without consensus on technique or standard outcomes reporting. The authors of this study aimed to investigate current surgical management of craniosynostosis in the United States. METHODS: Two hundred seventy-five surgeons actively treating craniosynostosis in the United States were surveyed. The results from a 28-item instrument were analyzed according to surgeon specialty, surgeon tenure, and geographic location of practice. RESULTS: The overall response rate was 47.6% (131/275), and final analyses included 58 plastic and recons

Risk factors for recurrent pressure ulcers after reconstructive surgery: A retrospective study in a single medical centre.

Recurrence of pressure ulcers following reconstructive surgery occurs frequently, causing a significant burden on the patient and the public health care system. We assessed risk factors for the recurrence of pressure ulcers based on the experience of a single surgeon at our medical centre. We retrospectively analysed patients admitted to our medical centre with stage III and IV pressure ulcers who underwent reconstructive surgery. The hospital database was searched for patients diagnosed with pressure ulcers who underwent reconstructive surgery. Patient characteristics analysed included age, s

What Should Be the Surgeon's Role in Defining "Normal" Genital Appearance?

The recent rise in women seeking cosmetic surgery of their genitalia (labiaplasty) coincides with the increasing number of surgeons posting videos of these operations on social media accounts and websites. Sociocultural influences significantly contribute to our ideas of what constitutes healthy and pathologic, and surgeons have historically played a role in defining "normal" and "abnormal" anatomy. In the nineteenth century, Saartjie Baartman-a woman with a large posterior and unusually long labia minora-was used by physicians to "educate" the public about these differences. We examine the pa
