Visual association encoding activates the medial temporal lobe: a functional magnetic resonance imaging study.

The involvement of structures in the medial temporal lobe during the encoding of visual associations was studied with functional magnetic resonance imaging. In 11 out of 12 normal healthy volunteers this task resulted in activation in posterior portions of the parahippocampal region, close to the collateral sulcus. In seven subjects activation was encountered in the hippocampal formation. The visual association task as adapted for this study may provide a sensitive measure to study anterograde amnesia prevalent in Alzheimer's disease. Therefore, the present paradigm enables the study of indivi…

[Pure word deafness after cerebral hemorrhage in the left temporal lobe: a case report].

We report a patient with pure word deafness after subcortical hemorrhage in the left temporal lobe. Repetition and auditory comprehension were severely impaired, while reading and visual comprehension of the same material were almost normal. He did not show hearing loss, but speech discrimination and melody recognition was poor. On the speech discrimination test, his score was low especially in the right ear. The threshold on the directional hearing test was mildly elevated. There was no temporal summation by click sounds. CT and MRI disclosed a subcortical hematoma in the left superior tempor…

Pure amnestic seizures in temporal lobe epilepsy. Definition, clinical symptomatology and functional anatomical considerations.

Pure amnestic seizures (PAS) sometimes occur in patients with temporal lobe epilepsy. They never represent the only type of seizures in these patients. Pure amnestic seizures are defined as seizures during which the only clinical manifestation is the patients' inability to retain in memory what occurs during the seizure coupled with the preservation of other cognitive functions and the ability to interact normally with their physical and social environment. It is postulated that PAS result from selective ictal inactivation of mesial temporal (MT) structures without isocortical involvement. Thi…

Focal retrograde amnesia following bilateral temporal lobe pathology. A neuropsychological and magnetic resonance study.

A patient who suffered a severe closed head injury was left with a dense retrograde amnesia for events which she had experienced prior to he … of focused magnetic resonance scan images revealed major pathology in the anterior portion of both temporal lobes. No significant abnormality was found in the hippocampus, thalamus or other medial structures in the limbic-diencephalic system. Our findings indicate the independence of anterograde and retrograde memory mechanisms, and point to structures and pat …

Using spectral and temporal filters with EEG signal to predict the temporal lobe epilepsy outcome after antiseizure medication via machine learning.

Epilepsy is a neurological disorder in which the brain is transiently altered. Predicting outcomes in epilepsy is essential for providing feedback that can foster improved outcomes in the future. This study aimed to investigate whether applying spectral and temporal filters to resting-state electroencephalography (EEG) signals could improve the prediction of outcomes for patients taking antiseizure medication to treat temporal lobe epilepsy (TLE). We collected EEG data from a total of 46 patients (divided into a seizure-free group (SF, n = 22) and a non-seizure-free group (NSF, n = 24)) with T…

Amplitude synchronization of spontaneous activity of medial and lateral temporal gyri reveals altered thalamic connectivity in patients with temporal lobe epilepsy.

In this study, we examined whether amplitude synchronization of medial (MTL) and lateral (LTL) temporal lobes can detect unique alterations in patients with MTL epilepsy (mTLE) with mesial temporal sclerosis (MTS). This was a retrospective study of preoperative resting-state fMRI (rsfMRI) data from 31 patients with mTLE with MTS (age 23-69) and 16 controls (age 21-35). fMRI data were preprocessed based on a multistep preprocessing pipeline and registered to a standard space. Using each subject's T1-weighted scan, the MTL and LTL were automatically segmented, manually revised and then fit to a …

Astrocytic Igfbp2 Promotes Spontaneous Seizures in a Mouse Model of Mesial Temporal Lobe Epilepsy.

Mesial temporal lobe epilepsy (MTLE) is a common, frequently drug-resistant epilepsy characterized by seizures arising from the hippocampus. Its hallmark pathology is hippocampal sclerosis with neuronal loss and reactive astrogliosis. Although astrocytes have emerged as potential targets for antiepileptic therapies, their role in epilepsy development remains poorly defined. Here, we combined adeno-associated virus (AAV)-mediated labeling with translating ribosomal affinity purification (TRAP) to generate astrocyte-enriched transcriptome profiles from sclerotic hippocampal regions in a mouse mo…

An rs-fMRI based neural marker for MRI-negative temporal lobe epilepsy with depression.

OBJECTIVE: Depression is the most common comorbidity in epilepsy. Currently, the diagnosis of comorbid depression in epilepsy primarily relies on medical history and scales. However, this approach is highly subjective and heavily dependent on the physician's experience, and prone to missed or misdiagnosis. The primary objective of this study was to evaluate the effectiveness of network homogeneity (NH) measurements analyzed via support vector machine (SVM) in diagnosing MRI-negative temporal lobe epilepsy with depression (MRI-negative TLED). METHODS: The study included a total of 217 particip…

Medial temporal lobe atrophy is associated with age and pathologies, especially small vessel disease.

Visual assessment of medial temporal lobe atrophy (MTA) in the clinical workup of cognitive impairment is traditionally corrected for age since MTA increases with age. In addition, common pathologies in the elderly such as amyloid, tau, alpha-synuclein and TDP-43 accumulation as well as white matter hyperintensities representing small vessel disease may affect the association between MTA and age. We investigated this in 949 cognitively unimpaired (CU) and 854 cognitively impaired (CI) individuals focusing on amyloid, tau and alpha-synuclein that at present can be measured in vivo in plasma, CS…

Temporal lobe epilepsy is associated with neuroinflammation, extracellular matrix remodeling, and synaptic protein alterations.

INTRODUCTION: Temporal lobe epilepsy is the most prevalent form of drug-resistant focal epilepsy and is frequently associated with neuronal cell loss and astrogliosis in the hippocampus, i.e. hippocampal sclerosis (HS). METHODS: In this study, we performed mass spectrometry-based proteomic profiling of microdissected hippocampal, neocortical, and white matter tissue obtained from TLE patients and respective control samples. RESULTS: In hippocampal TLE tissue, we observed significant upregulation of proteins involved in complement system activation, extracellular matrix (ECM) organization, an…

Contralateral language network integration predicts and protects against naming decline after temporal lobe resection.

OBJECTIVE: Anterior temporal lobe resection (ATLR) is an effective treatment for drug-resistant temporal lobe epilepsy (TLE) but carries a substantial risk of language impairment, particularly in naming. Understanding and predicting the impact of ATLR on language functions remains a major clinical challenge. METHODS: In this study, we used functional magnetic resonance imaging (fMRI) to investigate the short-term effects of ATLR on the organization of the functional language connectome with a focus on the role of the nondominant hemisphere. We studied 44 patients with TLE due to unilateral hi…

Tweak regulates glial cell activation in temporal lobe epilepsy through a positive feedback circuit.

Gliosis is a hallmark of temporal lobe epilepsy (TLE) and contributes to disease progression and cognitive deficits, yet its regulatory mechanisms remain poorly understood. Tweak (tumor necrosis factor-related weak inducer of apoptosis) has been implicated in glial activation and inflammation, but its role in TLE remains unclear. In this study, a TLE mouse model was established by intraperitoneal injection of pilocarpine. Knockdown of either Tweak or long non-coding RNA Snhg3 (small nucleolar RNA host gene 3), a lncRNA co-expressed with Tweak, alleviated glial activation, neuroinflammatory, an…

Thalamocortical connectivity and nucleus-specific thalamic dysfunction in temporal lobe epilepsy: an integrative review.

Temporal lobe epilepsy (TLE) is increasingly recognized as a distributed network disorder extending beyond mesial temporal structures. Among the brain regions implicated in epileptic network organization, the thalamus has emerged as a key hub due to its extensive structural and functional connections with cortical, limbic, and subcortical systems. This integrative review aimed to synthesize current evidence regarding nucleus-specific thalamic abnormalities and thalamocortical connectivity alterations in TLE.A literature search was conducted following PRISMA guidelines using PubMed and Google Sc…

How Focal Seizures Impair Consciousness: Clinical Features and Network Mechanisms in Temporal Lobe Epilepsy.

Focal impaired consciousness seizures (FICS) are among the most common seizure types in epilepsy and are a major cause of reduced QOL, occurring predominantly in temporal lobe epilepsy. Although seizure activity often remains confined to a focal temporal lobe network, patients develop widespread impairment of consciousness, indicating dysfunction of distributed brain systems rather than local seizure activity alone. Over the past two decades, converging evidence from human electrophysiological recordings, neuroimaging, and experimental animal models has substantially advanced our understanding…
