[On translocation surgery of the tendon of the musculus peroneus brevis (M. fibularis brevis) in clubfoot (author's transl)].

The paper reports on the basis of long-term observations on an old surgical method which used to be practised in infants before arthrolysis around the anklebone (pantalar arthrolysis) was employed in the treatment of insufficient lifting of the outer edge of the foot in clubfoot recidivation. This old surgical method consisted of translocating the tendon of the m. peroneus brevis (m. fibularis brevis) before the outer ankle bone. The method is described, and its mode of action is discussed. Results obtained with 14 operations of this kind, are presented.

Postoperative Imaging of the Ankle: Ligament and Tendon Reconstruction.

Tendon and ligament disorders are common indications for ankle surgery. Numerous surgeries and related modifications, ranging from simple repairs and debridement to complex reconstructions with tendon transfers or graft, are available. Knowledge of postoperative imaging appearances of common ankle surgeries is critical to identify complications and aid assessment of clinical treatment failure. This comprehensive review article discusses the surgical indications, techniques, imaging appearances, and complications-specifically those related to minimally invasive or arthroscopic surgery-for vario…

Unusual architecture of extensor digitorum muscle of hand in conjunction with accessory belly of extensor carpi radialis brevis: a clinico-anatomical insight.

The present article highlights an unusual combination of variations of the extensor digitorum and extensor carpi radialis brevis muscles in the dorsum of forearm and hand. The extensor carpi radialis brevis exhibited an accessory musculotendinous belly which inserted into the dorsal digital expansion of index finger. The extensor digitorum showed trifurcation into three distinct parts. Resultant multiple extensor tendon insertion pattern is reported upon highlighting the embryological basis and clinical importance. Knowledge of such anatomical variations adds upon the already existing informat

Surgical treatment of type 0 radial longitudinal deficiency.

PURPOSE: The purpose of this study was 2-fold: (1) to describe the surgical anatomy associated with type 0 radial longitudinal deficiency (radially deviated hand in the presence of a normal-length radius) and (2) to report the results of a surgical procedure designed to improve the alignment of the hand and forearm. METHODS: Since 1986 there have been 6 cases of type 0 radial longitudinal deficiency in 5 children seen at the St. Louis Shriner's Hospital. These children were treated with a surgical procedure to release the radial soft tissues and correct the alignment with tendon transfers. Ag…

[Functional reconstruction of irreversible partial injury of brachial plexus].

OBJECTIVE: To assess treating results of functional reconstruction of irreversible partial injury of brachial plexus and to improve the function of injured upper extremity. METHODS: Seventy-nine cases with irreversible partial injury of brachial plexus were treated in transfer of muscle (tendon) or by functional arthrodesis (fixation of tendon) from January 1984 to June 2003. According to the evaluation criterion by American Shoulder and Elbow, Hand Association, all patients were followed up in motion of reconstructive joint and daily activities after operation for 1 year to 19 years. The eff

Functional Chimeric Double-barrel Fibula and Reinnervated Peroneus Brevis Osteomyocutaneous Flap for One-stage Forearm Reconstruction.

Reconstructing a mangled limb is complex and requires expertise in both bone and soft-tissue reconstruction, particularly when there is significant muscle loss. Typically, multistage surgery is necessary, starting with soft-tissue coverage, followed by bone grafting and tendon transfers. Sometimes, microsurgical techniques such as vascularized bone grafts and free functional muscle transfers are necessary, especially when there is a bone defect of over 6 cm; the soft-tissue environment is infected, scarred, or poorly vascularized; or there are extensive musculotendinous injuries. We treated a …

Chopart amputation with tendon balancing.

BACKGROUND: When foot necrosis occurs due to lower limb blood flow disorder caused by diabetes or peripheral arterial occlusion, many patients require lower limb amputation. The functional prognosis after lower limb amputation largely depends on whether the heel can be preserved. However, there are many reports that Chopart amputation causes varus and equinus deformity, and is functionally unfavorable. We herein report a case of Chopart amputation performed with muscle balancing. Postoperatively, the foot was not deformed and the patient was able to walk independently with a foot prosthesis.

Median to radial nerve transfer to restore wrist and finger extension: technical nuances.

BACKGROUND: Traditional methods for restoring finger and wrist extension following radial nerve palsy include interposition nerve grafting or tendon transfers. We have described the utilization of distal nerve transfers for the restoration of radial nerve function in the forearm. OBJECTIVE: We review the neuroanatomy of the forearm and outline the steps required for the implementation of this transfer. METHODS AND RESULTS: We use a step-by-step procedural outline and detailed photographs, line drawings, and video to describe the procedure. CONCLUSION: This approach is technically feasible a…

Early corrective surgery of the wrist and elbow in arthrogryposis multiplex congenita.

An early one-stage corrective surgical procedure for the elbow and wrist of patients suffering from arthrogryposis multiplex congenita is advocated. The procedure involves adequate proximal row carpectomy, tendon transfers to achieve a balanced wrist and a triceps to radius transfer for elbow flexion. Experience with 47 limbs showed that the best results were obtained in children operated on between 3 and 6 months of age.

Arthroscopically-Assisted Latissimus Dorsi Transfer for Irreparable Rotator Cuff Insufficiency: Modes of Failure and Clinical Correlation.

PURPOSE: The main objective of this retrospective study was to analyze the rate and modes of failure of latissimus dorsi transfer (LDT). The secondary objective was to evaluate whether a rupture of the transfer was associated with a worse outcome. METHODS: During a 2-year period, we performed consecutive LDTs either for irreparable posterior-superior rotator cuff tears (RCTs) or for failed prior repair. All the LDTs were performed by a single surgeon. All transfers were arthroscopically assisted and fixed as a tubularized LD tendon in a bone tunnel inside the humeral head. Three metal clips w
