Testing prediction capabilities of an 131I terrestrial transport model by using measurements collected at the Hanford nuclear facility.

A model describing transport of 131I in the environment was developed by SENES Oak Ridge, Inc., for assessment of radiation doses and excess lifetime risk from 131I atmospheric releases from Oak Ridge Reservation in Oak Ridge, Tennessee, and from Idaho National Engineering and Environmental Laboratory in southeast Idaho. This paper describes the results of an exercise designed to test the reliability of this model and to identify the main sources of uncertainty in doses and risks estimated by this model. The testing of the model was based on materials published by the International Atomic Ener…

Left-sided liver and right sided Polysplenia: a 77-year-old patient with hetrotaxy syndrome.

Hetrotaxy syndrome is defined as an abnormality where the internal thoraco-abdominal organs demonstrate abnormal arrangement across the left-right axis of the body. Although it is a known condition among physicians taking care of pediatric patients, it is rarely seen in adult day-to-day medicine and most physicians involved in care of adult patients are unaware of it. It is important to recognize this anomaly based on clinical findings, due to its reported association with various medical conditions. We report a case of a 77-year-old patient diagnosed with Hetrotaxy syndrome.

Case report and review of the literature: spontaneous aortobronchial fistula.

We present a fatal case of aortobronchial fistula due to ruptured atherosclerotic aneurysm of the aorta into the left lower lobe, bronchus. Also, review of the pertinent literature is presented. Fistulas between the aorta and tracheobronchial tree are rare but usually lethal if not treated promptly and timely, as they can cause fatal hemoptysis. Aortobronchial fistulas occur most often in patients who have a history of thoracic vascular surgery. Nevertheless, few cases without previous thoracic surgery, trauma or infectious process of the aorta have been described in the literature. [corrected

Isolated pyocele of anterior clinoid process presenting as a cavernous sinus syndrome.

A 37-year-old man presented with fever, decreased vision in the left eye, a partial left cranial nerve III paresis, and a left cranial nerve VI paresis. Neuro-imaging showed an opacification of a left pneumatised anterior clinoid process. After failing a course of intravenous antibiotics, a craniotomy was performed with exenteration of the cavity and resolution of symptoms. Although rare, a pyocele of a pneumatised anterior clinoid process may cause ocular morbidity and require surgical intervention.

Treatment of cerebral malaria and acute respiratory distress syndrome (ARDS) with parenteral artesunate.

Infection with Plasmodium Falciparum can cause a severe form of malaria with multiorgan involvement. Cerebrum is one of the organs involved in the P. Falciparum malaria, which can lead to coma, convulsions, and other neurological sequel. The sequestration of cerebral vasculature with parasitized red blood cells is one of the proposed mechanisms for the development of cerebral malaria. We present a case of malaria with multi organ involvement. Cerebral malaria should be suspected in any febrile patient from a malaria-endemic region with loss of consciousness. Compared with quinine, intravenous

Intravenous gammaglobulin as rescue therapy in a patient with sickle cell and septic shock.

INTRODUCTION: We present a case involving a patient with sickle cell and hyposplenism, in which refractory septic shock quickly responded after the infusion of intravenous gammaglobulin (IV-GG) given as an adjuvant-rescue therapy CASE DESCRIPTION: A 30-year-old African-American female with history of Sickle Cell disease was admitted for acute chest syndrome, septic shock and respiratory failure. Despite aggressive therapy the patient remained on two vasopressors and with persistent bacteremia. Within one day of starting IV-GG, both vasopressors (norepinephrine and vasopressin) were able to be

Improvement in ocular myasthenia gravis during CPAP therapy for sleep apnea.

We describe the case of a 73-year-old male with a history of ocular myasthenia gravis (OMG) who was followed through diagnosis and treatment of sleep-disordered breathing (SDB). Therapeutic use of continuous positive airway pressure (CPAP) led to improvement of not only SDB but also OMG. To our knowledge, this is the first paper describing this treatment benefit. This case report emphasizes the need to obtain a sleep history in patients with ocular myasthenia gravis in order to screen for SDB.

Unusual presentations of disseminated histoplasmosis.

Histoplasmosis is considered to be the most prevalent endemic mycosis in United States that can present as a disseminated infection. The initial presentation of Disseminated Histoplasmosis (DH) can be atypical. We report three cases with such atypical presentation. Our first patient presented with bowel perforation, the second with left-sided pleural effusion and the third with submandibular abscess. Blood cultures as well as biopsy of perforation site, culture of pleural fluid and submandibular abscess were positive for Histoplasma Capsulatum (HC). We encourage clinicians to look for HC even

A case report of acute interstitial nephritis associated with antibiotic-impregnated orthopedic bone-cement spacer.

Local antibiotics are effective adjunct therapy for the treatment of orthopedic infections and antibiotic loaded bone cement is successfully used in revision procedures of infected hip and knee arthroplasties. Different antibiotics in isolation or in combination can be added into bone cement. The theory behind the use of antibiotic loaded cement is that anti-infective drug incorporated into the cement diffuses into surrounding tissues over a period of weeks to months, thereby providing prolonged high local concentration of antibiotic while minimizing systemic drug toxicities. Although kidney t

Asymptomatic colonic histoplasmosis in a patient receiving methotrexate.

Disease due to infection with Histoplasma capsulatum usually manifests as a disseminated infection in patients with the Acquired Immunodeficiency Syndrome (AIDS). However, it may also present with focal disease in patients with or without AIDS. Unusual presentations or less well-recognized risk factors may make diagnosis of histoplasmosis more difficult. We describe the case of a patient who presented for screening colonoscopy and was found to have isolated asymptomatic colonic histoplasmosis.

Diabetic ketoacidosis presenting with acute pancreatitis and visceral vein thrombosis.

Abdominal pain is a frequent manifestation in patients presenting with Diabetic Ketoacidosis (DKA). Usually it is attributed to severe metabolic acidosis but it can be due to underlying abdominal pathologies (i.e., pancreatitis, appendicitis). We report a case of a 19-year-old female who presented with DKA and severe abdominal pain and was found on further examination to have underlying pancreatitis and visceral vein thrombosis. The patient improved with treatment for the mentioned co-morbidities, including anticoagulation.

Lactic acidosis in restrained cocaine intoxicated patients.

Severe lactic acidosis has been reported in patients struggling against restraints, especially in association with the use of stimulant drugs, such as cocaine. Profound acidosis occurring under these conditions can lead to cardiac arrhythmias, autonomic instability and cardiac arrest, a syndrome known as restraint associated asphyxia. Early recognition of this condition and removing the stimulus for lactic acid production (excessive muscle activity) by aggressive sedation and ventilatory assistance, coupled with fluid administration to improve tissue perfusion and lactate metabolism, can be li

The occurrence of stiff person syndrome in a patient with thymoma: case report and literature review.

Stiff-person syndrome (SPS) is a rare disorder that affects the central nervous system, being characterized by intermittent episodes of muscle spasms, rigidity, and a significant increase in morbidity and mortality. We report a 29-year-old patient, with left-sided chylothorax and thymoma, who presented with truncal, lower extremities stiffness and painful cramps alleviated by diazepam and opiates. His clinical picture was suggestive of SPS secondary to invasive thymoma. This syndrome is best approached by its early recognition and prompt treatment since a missed diagnosis can have deleterious

Disseminated histoplasmosis presenting with biliary obstruction and duodenal ulcer.

Disseminated histoplasmosis is an AIDS-defining illness which usually involves the liver and gastrointestinal tract, most commonly the small bowel. Abdominal pain, diarrhea, GI bleeding, hepatosplenomegaly and small bowel obstruction are well described presentations. Still gastrointestinal histoplasmosis often results in either vague symptomatology or no symptoms. Pancreaticobiliary disease related to disseminated histoplasmosis is not well characterized. We report the case of a young female with advanced HIV infection and biliary obstruction and a periampullary duodenal ulcer due to dissemina

Aortic thrombosis in infancy.

A 33-day-old female was admitted to hospital with the diagnosis of viral bronchiolitis. The physical examination performed after treatment with bronchodilators revealed a grade 2/6 systolic ejection murmur at the left upper sternal edge. The arterial pulses and blood pressure in all extremities were normal. The two-dimensional echocardiogram revealed a mass measuring 3.5 x 3.5 cm at the ductus level consistent with a thrombus; there was flow acceleration at the level of the proximal descending aorta with no significant gradient. All other laboratory tests were normal except for an elevated pla

Persistent hyperprolactinemia following mammoplasty: a rare association.

Breast augmentation surgery is one of the most commonly performed plastic surgery procedures. Several reported cases linked onset of transient hyper prolactinemia and galactorrhea to breast implant surgery. These prior studies did not corroborate that bioactive (monomeric prolactin) was responsible for the prolactin excess. We report a patient with galactorrhea and hyperprolactinemia following breast augmentation, with several unique features. We used monomeric prolactin in the diagnosis of hyperprolactinemia and macro prolactinemia was specifically excluded. Common causes of prolactin excess
