Pfeiffer syndrome is one of a group of craniosynostosis syndromes in which rare tracheal anomalies have been described. This group of patients have a poor prognosis, and mortality can be related to airway complications and respiratory distress. We report a case of type II Pfeiffer syndrome with tracheal cartilaginous sleeve and cricoid cartilage involvement. We discuss our strategy for the management of the airway of this patient.

We present an experience with transversal tracheal resection with end-to-end anastomosis in 3 patients with extensive subglottic stenosis because of prolonged intubation. All the patients were treated chirurgically based on the Pearson's method. In order to attein the fixation of tracheal stumbs, poroplastic splint was applied into the lumen of the trachea and patency of airways was protected by tracheotomy. There were no major complications during treatment and time of the treatment was significally shortening.

Four patients with bilateral vocal cord fixation due to isolated posterior glottic stenosis that developed following endotracheal intubation are presented. Two patients had previously undergone multiple endoscopic surgical procedures without obtaining a satisfactory airway. All four were treated by posterior cricoid split and costal cartilage interposition and were successfully decannulated. The assessment of vocal cord immobility following endotracheal intubation is presented and the surgical management of posterior glottic stenosis discussed.

Between 1965 and 1985, 47 cases of glottic and/or tracheal stenosis were diagnosed at the Rocky Mountain Regional Spinal Cord Injury System. A retrospective review of medical records identified associated injuries, pulmonary and other medical complications in this patient population. Radiographic and endoscopic reviews utilised a grading system to classify the severity of stenosis. The clinical symptoms of stenosis were multiple, including dysphonia, aspiration, dysphagia, odynophagia, dyspnea and excessive secretions. The wide spectrum of treatment modalities included endoscopy with excision

Reconstruction of the subglottis is usually required after resection of cricoid cartilage or tracheal tissue in cases of trauma, subglottic stenosis, or invasive thyroid carcinoma. There are multiple techniques available for reconstruction of the subglottic larynx, but most of them have a high rate of complications or graft failure. We have found the sternocleidomastoid myoperiosteal flap to be an ideal means of reconstruction. In an experimental study, we performed subglottic reconstruction using the sternomastoid myoperiosteal flap on 15 mongrel dogs to determine the presence of bone formati

Five cases of blunt trauma to the anterior neck were presented. It is most important to keep a high index of suspicion for the potential of total airway obstruction. Persons trained in establishing multiple types of emergency airways should be available until laryngotracheal damage has been ruled out, as the usual endotracheal tube or cricothyreotomy may be inadequate.

Vocal cord positioning produced by selective laryngeal denervation remains a controversial issue in clinical laryngology. Previous studies fail to arrive at uniform conclusions for two important reasons: 1) failure to mark a reference sagittal plane from which to assess the degree of vocal cord lateralization, and 2) failure to recognize the influence of tracheostomy and respiratory positioning of the vocal cords. The present study makes use of photographic and electromyographic documentation in the assessment of the paralyzed cord. Physiologic inactivation of the cricothyroid muscle by trache

An obese 57-year-old man with the obstructive sleep apnea syndrome had partial tracheostomy occlusion and palpitations. Continuous ECG monitoring demonstrated atrial flutter, occasional premature ventricular beats, and a high degree of atrioventricular block during apneic episodes. A 7-s period of ventricular asystole was demonstrated during one such episode. Revision of the tracheostomy produced an improvement in the patient's condition.

Respiratory distress with episodes of cyanosis, intercostal retraction and sibilant rhonchi occurred in a 2-year-old boy over a 48-hour period following serious smoke inhalation. Worsening of the child's condition accompanied these findings, culminating in sudden loss of air entry, severe respiratory distress and loss of consciousness, which necessitated endotracheal intubation for resuscitation. Pronounced improvement followed removal of two pseudomembranous bronchial casts from the airway by suctioning, and thereafter recovery was uneventful.

A fatal haemorrhage of the innominate artery complicating tracheostomy for obstructive carcinoma of the larynx is reported. The possible mechanism of the rupture is discussed and methods of management are suggested.
