Progress in transplantation.

Transplantation is an important and, in most cases, the only available therapeutic option for saving the lives of patients with end-stage organ failure. Furthermore, transplantation is recognized as one of the major medical achievements over the last half century. Despite outstanding short-term graft and patient survival rates, organ transplantation continues to face several major challenges. These include a severe shortage of donor grafts, poor long-term graft survival resulting from chronic vascular rejection, and major side effects from long-term immunosuppressive therapy required for preve

Liver transplantation.

Liver transplantation has become a lifesaving procedure for patients who have chronic end-stage liver disease and acute liver failure. The satisfactory outcome of liver transplantation has led to insufficient supplies of deceased donor organs, particularly in East Asia. Hence, East Asian surgeons are concentrating on developing and performing living-donor liver transplantation (LDLT). This review article describes an update on the present status of liver transplantation, mainly in adults, and highlights some recent developments on indications for transplantation, patient selection, donor and r

Pancreas transplantation.

Since the introduction of pancreas transplantation more than 40 years ago, efforts to develop more minimally invasive techniques for endocrine replacement therapy have been in progress, yet this surgical procedure still remains the treatment of choice for diabetic patients with end-stage renal failure. Many improvements have been made in the surgical techniques and immunosuppressive regimens, both of which have contributed to an increasing number of indications for pancreas transplantation. This operation can be justified on the basis that patients replace daily injections of insulin with an i

Barnard in 1967 but it was not until 1980s that this method became an e … essant therapy, a range of complications has to be dealt with. The future of care for patients with transplants will rely on the development of new immunosuppressive drugs with a minimum of adverse effects and discovery of a non-invasive technique for graft rejection diagnosis.

[Heart transplantation].

Heart transplantation remains the best treatment for terminal heat failure. Ischemic and idiopathic cardiomyopathies are the main indications. Contraindications are due to immunosuppressive treatment and fixed pulmonary hypertension. After clinical evaluation, the patient is put on the waiting list controlled by the French Biomedicine Agency. Acute rejection had decrease due to the improvement of immunosuppressive treatments. The incidence of severe acute rejection is 5%. Chronic rejection is the main cause of morbidity and mortality. The rate at 5 years is about 40-50%. Survival at one year i

[Lung transplantation].

Lung transplantation (LT) has evolved to become an important alternative in the management of patients with end-stage pulmonary disease and chronic respiratory failure. The beginnings of this technique can be traced back to the experiments of Carrel and Guthrie over a hundred years ago. However, it was not until 1963 when the first clinical experience was performed by Hardy. Clinical success did not arrive until the 1980's thanks to the works of the Toronto Lung Transplant Group. Well established criteria have been described in order to consider a patient as a potential candidate to receive a

Pancreas transplantation.

Over the last 15 years whole organ pancreas transplantation has emerged as the treatment of choice for selected patients with uremia and Type I Diabetes Mellitus. Improvements in surgical technique, better understanding of transplant related complications and advances in immunosuppressive therapy have encouraged the application of this procedure to an increasing number of patients. Pancreas transplantation occurs under three primary scenarios: simultaneous kidney pancreas transplantation, pancreas transplantation after kidney transplantation, and pancreas transplant alone. Overall results are

Heart transplantation.

Although the number of available donor hearts severely limits the epidemiologic impact of heart transplantation on patients with heart failure, patients with end-stage heart failure unresponsive to medical management currently have no other viable alternatives. Destination therapy with a ventricular assist device is the closest toward approaching clinical reality but has been plagued with problems of infection and stroke. The purpose of this review is to summarize recent developments in the field that may broaden the clinical impact of heart transplantation. For example, novel methods of cardi

[Heart transplantation].

The first heart transplantation in the Nordic countries was performed at Rikshospitalet in Oslo, Norway in 1983 and the method is now accepted as a good treatment for selected patients. In this article we present a review of heart transplantation as it is practiced in our hospital. Because of donor shortage, the selection criteria are strict. It is most important that the patients are motivated and able to cooperate. They should be in heart failure class IV (III-IV) on the best medical treatment with no other treatment alternatives. Estimated survival without transplantation should be less tha

Renal transplantation.

Renal transplantation offers the best renal replacement therapy for most children with end stage renal disease improving their potential for growth and nutrition, neurodevelopment and quality of life. Advances in organ retrieval and preservation, improved surgical techniques, newer immunosuppressive drugs and prevention and treatment of infections have significantly improved patient and graft survival. The absolute requirements for a transplant are compatible blood group and a negative cytotoxic crossmatch. The immunosuppressive drugs most often used are cyclosporin A (or tacrolimus), azathiop

[Liver transplantation].

OBJECTIVE: To review the clinical aspects and the theoretical basis of liver transplantation in children, focusing mainly pre and post surgical periods. METHODS: References were obtained from computerized search in the National Library of Medicine (Medline), recent review articles, and personal files. RESULTS: Great development has occurred in surgical techniques, in organ preservation, in postoperative care, and in immunosuppression methods after the first liver transplantation surgery took place in a child with biliary atresia in 1963. Liver transplantation has become an efficient therapy, w

[Heart transplantation].

In carefully selected patients with end-stage heart failure heart transplantation has developed from an experimental procedure to standard therapy during the last 30 years. It is currently accepted as a procedure for prolonging life and also for improving quality of life. According to the Registry of the International Society for Heart and Lung Transplantation the overall one-year actuarial survival is 79% and 10-year survival barely 50%. Nine years after the start of the Heart Transplant Program at Rigshospitalet the overall actuarial survival of 157 consecutive patients is 66%. Due to the li

[The history of transplantation].

Transplantation is a young discipline with a long history, of which this paper therefore only describes fragments. The story is created by the pioneers, who made the basis for transplantation. I have focused on those who received the Nobel prize for their work, Carrel (1912), Burnet, Medawar (1960), Snell, Dausset, Benacerraf (1980), Elion, Hitchings (1988), and Thomas and Murray (1990), however many others deserve to be mentioned. Parallelled with the development of transplantation, other disciplines like immunology have grown up. Today we can transplant kidneys, hearts, livers, and bone marr

The immunobiology of transplantation.

Having recently published A History of Transplantation Immunology, I have become acutely aware of the great revolution that has taken place in the field of organ transplantation - not only in terms of our understanding of the immunobiological mechanisms underlying the rejection process and the various means of prolonging graft survival, but also in clinical terms. Only 50 years ago, clinical kidney allo-transplantation (and it was the kidney that spearheaded clinical advances) was a highly experimental and dangerous procedure dependent on whole-body irradiation and the results were often inten

[Corneal transplantation].

The first successful corneal transplantation was done in 1906, when the corneas of a recently deceased boy were transferred to a man blinded by an alkali burn. Since then, improved equipment, microsurgical technique and better methods for treating postoperative complications have greatly improved the prognosis of corneal grafting. Immunological reactions are less frequently seen after keratoplasty than after other types of transplantation, and the graft is also directly accessible for immunosuppressive treatment. Penetrating keratoplasty accounts for more than 95% of all corneal transplantatio

[Skin transplantation].

Transplantation of skin was adopted as a clinical method for wound closure soon after Reverdin's introduction of skin grafts in 1869. Split-skin grafts are grafts which consist of epidermis and a portion of dermis, and full thickness skin grafts are grafts consisting of epidermis and the entire dermis including adenexal structures. Composite grafts with skin consist of cartilage and skin coverings. Skin grafts can also be grouped according to source in autograft, isograft, allograft and xenograft. Split-thickness grafts are used to close wounds that cannot be managed by suturing, full thicknes

[Renal transplantation].

The clinical outcome of kidney transplantation worldwide is reviewed. Total number of kidney transplantations now number 411,071, and about 26,000 have been performed annually in recent years. In Japan, the total is 11,813, and about 600 to 700 have been performed annually in recent years. Graft survival is 80% at one year and 60% at five years in cadaveric renal transplantation worldwide, and 90% at one year and 70% at 5 years, respectively, in Japan. There is no difference in the graft survival rate between Japan and Western countries. However, there have been fewer kidney transplantations i
