The forequarter lateral implosion injury: A diagnostic paradigm and Ch-Sh (chɛ-shɑ) classification.

BACKGROUND: The lateral impaction force against the shoulder often causes multiple lesions to the forequarter including the chest wall. The purpose of this study is to describe the injury patterns and associated pathology in patients with combined chest wall and shoulder girdle injuries- forequarter lateral implosion injury and organize the findings into a classification framework to predict morbidity. METHODS: Three hundred fifty-four consecutive patients with two or more rib fractures and ipsilateral shoulder girdle injury from a Level 1 trauma center, between 2018-2022, were retrospectivel

Blunt cerebrovascular injury in children: A prospective multicenter ATOMAC+ study.

BACKGROUND: The incidence of blunt cerebrovascular injury (BCVI) in children remains largely unknown, with only 16.5% of children receiving appropriate screening. This study sought to determine the impact of a screening guideline on injury detection and outcomes in children with BCVI. METHODS: This was a prospective, multi-institutional observational study of children younger than 15 years with blunt trauma to the head, face, or neck (Abbreviated Injury Scale score, >0) at any of six level 1 pediatric trauma centers. All patients were screened using the Memphis criteria. Head/neck computed to…

Head injury patterns in helmeted and non-helmeted cyclists admitted to a London Major Trauma Centre with serious head injury.

BACKGROUND: Cycle use across London and the UK has increased considerably over the last 10 years. With this there has been an increased interest in cycle safety and injury prevention. Head injuries are an important cause of mortality and morbidity in cyclists. This study aimed to ascertain the frequency of different head injury types in cyclists and whether wearing a bicycle helmet affords protection against specific types of head injury. METHODS: A retrospective observational study of all cyclists older than 16 years admitted to a London Major Trauma Centre between 1st January 2011 and 31st

Preparing Japanese surgeons for potential mass casualty situations will require innovative and systematic programs.

INTRODUCTION: The ongoing state of global geo-political instability means that it is prudent to prepare civilian surgeons to manage major military-type trauma. Japan has enjoyed a prolonged period of peace and consequently it is unlikely that surgeons will have been exposed to a sufficient volume of cases. This study reviews the state of trauma training and preparedness in Japan and reviews the trauma workload of a major Japanese emergency medical center and compared with a major South African trauma center with the intention of quantifying and comparing the time needed to gain adequate exposu

Impact of Helicopter Emergency Medical Service in Traumatized Patients: Which Patient Benefits Most?

INTRODUCTION: The Helicopter Emergency Medical Service (HEMS) was established for the prehospital trauma care of patients. Improved rescue times and increased coverage areas are discussed as specific advantages of HEMS. We recently found evidence that HEMS exerts beneficial effects on outcomes for severely injured patients. However, it still remains unknown which group of trauma patients might benefit most from HEMS rescue. Consequently, the unique aim of this study was to reveal which patients might benefit most from HEMS rescue. METHODS: Trauma patients (ISS ≥9) primarily treated by HEMS or

Trauma activation patients: evidence for routine alcohol and illicit drug screening.

BACKGROUND: Statistics from the National Trauma Data Bank imply that discretionary blood alcohol and urine drug testing is common. However, there is little evidence to determine which patients are appropriate for routine testing, based on information available at trauma center arrival. In 2002, Langdorf reported alcohol and illicit drug rates in Trauma Activation Patients. METHODOLOGY/PRINCIPAL FINDINGS: This is a retrospective investigation of alcohol and illicit drug rates in consecutive St. Elizabeth Health Center (SEHC) trauma patients. SEHC Trauma Activation Patients are compared with th

Predicting risk-adjusted mortality for trauma patients: logistic versus multilevel logistic models.

BACKGROUND: Theoretical advantages of random-intercept multilevel (ML) logistic regression (LR) modeling over standard LR include separating variability due to patient-level and hospital-level predictors, shrinkage of estimates for lower-volume hospitals toward the overall mean, and fewer hospitals falsely identified as outliers. STUDY DESIGN: We used Nationwide Inpatient Sample data from 2002 to 2004 to construct LR models of hospital mortality after admission with a principal ICD-9 Clinical Modification injury diagnosis (ICD-9 Clinical Modification codes 800 to 904, 910 to 929, 940 to 957,

Multiple laparotomies are a predictor of fascial dehiscence in the setting of severe trauma.

The purpose of this study was to determine the incidence of wound dehiscence after repeat trauma laparotomy. We performed a retrospective analysis of adult trauma patients who underwent laparotomy at an urban level 1 trauma center during the past 5 years. Patients were divided into single (SL) and multiple laparotomy (ML) groups. Demographic, clinical, and outcome data were collected. Data were analyzed using chi2, t testing, and ANOVA. Overall dehiscence rate was 0.7 per cent. Multiple laparotomy patients had damage control, staged management of their injuries, or abdominal compartment syndro

The role of site and severity of injury as predictors of mental health outcomes following traumatic injury.

The aim of this study was to investigate the influence of injury site and severity as predictors of mental health outcomes in the initial 12 months following traumatic injury. Using a multisite, longitudinal study, participants with a traumatic physical injury (N = 1,098) were assessed during hospital admission and followed up at 3 months (N = 932, 86%) and at 12 months (N = 715, 71%). Injury site was measured using the Abbreviated Injury Scale 90, and objective injury severity was measured using the Injury Severity Score. Participants also completed the Hospital Anxiety and Depression Scale a

Trauma screening in students attending a medical university.

Clinicians seldom assess trauma history in patients who seek treatment for psychological problems, yet trauma exposure is often related to psychological distress. Assessing trauma history can provide valuable information for treatment conceptualization and provision, although patients may not spontaneously share their histories because of embarrassment, avoidance, or other concerns. The authors compared 73 students at a southeastern US medical university who sought counseling and psychological services and completed intake paperwork without a trauma screen with 130 students whose intake proced

Home away from home: factors associated with current functioning in children living in a residential treatment setting.

OBJECTIVE: While recent research has focused on the impact of abuse and other interpersonal traumas in childhood, little attention has been given to the experiences of children who have been removed from their homes. In addition to trauma, these children are likely to have had a number of experiences that may impact their current functioning. The purpose of this study was to provide descriptive information pertaining to the unique characteristics of children in residential treatment centers, and to examine preliminary factors believed to be associated with current functioning. METHODS: Partic

Successful limb reperfusion by temporary arterial shunt during a 950-mile air transfer: case report.

Plastic vascular shunts successfully reperfused a trauma victim's severely ischemic leg for 16 hours during a 950-mile transfer to a regional trauma center. Although the limb ultimately was lost, this case suggests the potential value of temporary shunts in managing complex extremity vascular injuries in settings remote from the trauma center.
