[Arteritis in aged patients].

Gangrene is the usual reason for admission to a surgical unit in elderly patients with peripheral vascular disease. It is often the first sign of arterial disease occurring in a foot with satisfactory skin and may be triggered off by trauma or infection. Obstruction at various levels of the profunda femoris artery, of the main leg arteries with calcification of the media, are the usual characteristics, whether the patient is diabetic or not. This explains why reconstructive vascular surgery is rarely possible but this, nevertheless, remains the best chance for obtaining healing of trophic diso

Clinical practice guideline for nonpharmacological prevention and management of patient agitation in the adult intensive care unit (CALM ICU).

BACKGROUND: Agitation affects 32-70% of adult patients in the intensive care unit (ICU) and is associated with disruption of life-saving treatment, prolonged hospitalisation, and psychological trauma. While nonpharmacological interventions are increasingly encouraged to reduce the reliance on sedatives, existing guidelines predominantly focus on pharmacological management. This contributes to inconsistent practices and underutilisation of effective person-centred nonpharmacological alternatives. OBJECTIVE: The objective of this study was to develop evidence-based recommendations for the nonph

Role of Strong Opioids in an Effective Discharge for Lower-Limb Large Joint Arthroplasty Patients: A Patient-Based Analysis.

BACKGROUND/OBJECTIVE: Adequate postoperative analgesics are an essential element in the recovery and rehabilitation of large joint lower-limb arthroplasty patients in their acute postoperative phase. In this study, we will establish that strong opioids like morphine should be included as postoperative analgesics to improve patient satisfaction.  Material: This retrospective cross-sectional study was conducted in the Arthroplasty Ward, Trauma, and Orthopaedics Department in a district general hospital of the United Kingdom. Fifty patients operated in January 2024 were enrolled in this study, ou

An Evaluation of the Effect of the Use of N95 Respirators by Surgical Teams on Early Surgical Site Infections in Orthopedic Cases.

Background Surgical site infections (SSIs) are seen in the postoperative period in orthopedic and traumatology clinics. Just as in all surgical clinics, SSIs lead to patient dissatisfaction with the results, prolong the length of stay in the hospital, and increase treatment costs. SSIs are known to occur as a result of wound contamination through inoculation of microorganisms found mainly in the air or in the surgical area. Because of the coronavirus disease 2019 pandemic, N95 masks have been widely used in the operating rooms of our hospital by nurses, residents, and surgeons since March 2020

Death Cafés for prevention of burnout in intensive care unit employees: study protocol for a randomized controlled trial (STOPTHEBURN).

BACKGROUND: Burnout is an occupational syndrome that leads to mental health problems, job turnover, and patient safety events. Those caring for critically ill patients are especially susceptible due to high patient mortality, long hours, and regular encounters with trauma and ethical issues. Interventions to prevent burnout in this population are needed. Preliminary studies suggest debriefing sessions may reduce burnout. This study aims to assess whether participation in regular debriefing can prevent burnout in intensive care unit (ICU) clinicians. METHODS: A randomized controlled trial will

The Cedar Project: a comparison of HIV-related vulnerabilities amongst young Aboriginal women surviving drug use and sex work in two Canadian cities.

BACKGROUND: In Canada, Aboriginal women and youth continue to be overrepresented amongst new cases of HIV, and are considered at increased risk for sex and drug-related harm. Young women involved in sex work are particularly vulnerable. The purpose of this study is to determine HIV-related vulnerabilities associated with sex work amongst young Aboriginal women in two Canadian cities. METHODS: This study is based on a community-based cohort of Aboriginal young people (status and non-status First Nations, Inuit and Métis) between the ages of 14 and 30 who used injection or non-injection illegal

How do real tumors become resistant to cisplatin?

CONTEXT: Indigenous children and adolescents in Australia and globally bear the burden of acute rheumatic fever (ARF). It has been virtually eliminated in well-resourced, developed settings. ARF is an autoimmune response to infection with group A Streptococcus. The mainstay of management is long-acting intramuscular penicillin injections to prevent recurrence of ARF and development of rheumatic heart disease (RHD), comprising valvular pathology and attendant complications. In Australia, penicillin injections are currently prescribed every 28 days for 5-10 years after diagnosis of ARF, dependin

Harm reduction services for veterans in supportive housing: A pharmacist-led, interdisciplinary, street medicine approach.

Background: United States Veterans are disproportionately impacted by homelessness. Substance use, mental health conditions, and trauma increase the risk for homelessness and contribute to challenges in maintaining housing. Street Medicine utilizes a trauma-informed approach to provide low-barrier health and social services for those at risk for and experiencing homelessness. Limited evidence exists describing pharmacist roles on Street Medicine teams, including provision of harm reduction services. Objective: To implement a pharmacist-led Harm Reduction Program Street Medicine intervention at

Cold-water immersion (cryotherapy) for preventing and treating muscle soreness after exercise.

BACKGROUND: Many strategies are in use with the intention of preventing or minimising delayed onset muscle soreness and fatigue after exercise. Cold-water immersion, in water temperatures of less than 15°C, is currently one of the most popular interventional strategies used after exercise. OBJECTIVES: To determine the effects of cold-water immersion in the management of muscle soreness after exercise. SEARCH METHODS: In February 2010, we searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register, the Cochrane Central Register of Controlled Trials (The Cochrane Library (20

Home-Based Exercise to Improve Functional Outcomes in Veterans With a Recently Healed Diabetic Foot Ulcer: Protocol for a Pilot Randomized Controlled Trial.

BACKGROUND: Foot ulcers are a common complication of diabetes, often resulting from peripheral neuropathy and inadvertent trauma. Poor healing is exacerbated by peripheral arterial disease and poor glycemic control. Off-loading, a key treatment, leads to prolonged immobility. Patients rarely regain baseline mobility. Mobility is crucial to improve glycemia, promote vascular health, and improve immobility as it leads to nursing home admissions. There is limited research on exercise during ulcer remission. OBJECTIVE: This pilot study will assess the feasibility and acceptability of a home-based

Optimal Resources for Children's Surgery 2: Tiered Recommendations for Minimum Essential Children's Surgical Procedures Across Health System Levels.

BACKGROUND: In 2015 the World Health Assembly (WHO), based on recommendations by the Lancet Commission on Global Surgery, passed resolution (68:15) on strengthening emergency and essential surgical care and anesthesia as a component of universal health coverage. However, children's surgery was not adequately represented and in response, the Global Initiative for Children's Surgery (GICS) published the Optimal Resources for Children's Surgical Care (OReCS) in 2019 providing insight into procedures which should be performed at different facility types. OReCS 2 addresses a gap in both policy and

Barriers and facilitators to implementation of peer support after miscarriage: a systematic review using thematic synthesis methods.

OBJECTIVES: Miscarriage, defined in the UK as loss of pregnancy prior to 24 weeks gestation, can have long-term psychological implications. Clinical guidelines for perinatal bereavement care do not provide guidance on how best to support the mental health of women, and their partners, after miscarriage. Peer support (support from those who share common characteristics) is often sought, but there is little understanding of its access and use. We conducted a systematic review to understand the barriers to and facilitators of the implementation of peer support to improve mental health outcomes fo

Short-term outcome of fragility fractures of the pelvis in the elderly treated with screw osteosynthesis and external fixator.

BACKGROUND: The treatment of fragility fractures of the pelvis is rising challenge for orthopedic trauma surgeons. Operative treatment should allow immediate full weight bearing and early mobilisation but should also be as minimal invasive as possible. Sacroiliac (SI) or transsacral transiliac screws (TSTI) alone or depending on the fracture in combination with an external fixator meets both of these criteria. MATERIAL AND METHODS: The outcome of 121 operatively treated patients with fragility fractures of the pelvis were evaluated in this retrospective study. Depending on the type of fractur
