Epidemiology and control of virus infections in the laboratory.

Viruses cause some of the most dangerous infections. Laboratory work with viruses entails hazards which should be faced rationally. Data are scarce but studies, particularly of hepatitis, are in progress and are extended to cover other infections also. Surveillance can provide facts with which to correct theories and guide control measures. A simple and informal surveillance scheme operated through membership of a professional association has proved useful and is commended as a model.

Vaccination gaps and infection risk at dialysis initiation.

In this editorial, we comment on the study by Puri et al published in the World Journal of Nephrology, which highlights a neglected gap in preventive care at dialysis initiation: Inadequate protection against vaccine-preventable and endemic infections in advanced chronic kidney disease. In an incident hemodialysis cohort from a tertiary centre in North India, the authors report major deficiencies in adult vaccination against hepatitis B virus, pneumococcus, and influenza at dialysis entry, with more than half of patients having received none of the recommended immunizations. Concurrently, a su

Hemophagocytic Lymphohistiocytosis Secondary to Extrapulmonary Tuberculosis in an HIV Patient: A Case Report.

Hemophagocytic lymphohistiocytosis (HLH) is a severe hyperinflammatory syndrome caused by unregulated immune activation, leading to a "cytokine storm" and multisystem failure. In people living with HIV (PLWH), it is frequently triggered by opportunistic infections like Mycobacterium tuberculosis. This report describes a 28-year-old transgender woman with HIV (on ART, undetectable viral load) admitted with a 20-day history of high fever, diarrhea, and hepatosplenomegaly. Admission labs showed severe pancytopenia (Hb 6.2 g/dL, WBC 1,590/µL, platelets 102,000/µL) and a CD4 count of 164 cells/mm³.

Whole exome sequencing identifies rare variants involved in PPIX-metabolizing pathway in anti-TB drug-induced hepatitis.

AIMS: To screen and validate rare variants in the protoporphyrin IX (PPIX)-metabolizing pathway associated with anti-tuberculosis drug-induced hepatitis (AT-DIH). METHODS: A two-stage matched case-control study was conducted. Firstly, a 1:1 matching design (50 cases and 50 controls) was adopted to screen for rare variants using whole exome sequencing (WES). Secondly, a 1:4 matching design (132 cases and 528 controls) was employed to validate these rare variants via TaqMan genotyping. The association between these variants and AT-DIH risk was evaluated using odds ratios (ORs) with 95% confiden

Profiling infectious diseases in Turkey after the influx of 3.5 million Syrian refugees.

BACKGROUND: Since 2011, the conflict in Syria has led to over five million refugees. Turkey hosts the highest number of Syrian refugees in the world. By February 2019 over 3.6 million people had fled to Turkey to seek safety. Only 6.1% of Syrian refugees live in temporary shelters. Owing to the disrupted healthcare services, many children coming from the conflict zones are less likely to have received vaccination. In temporary shelters immunization coverage is >95% and the refugee population is receptive to vaccination. AIMS: The objective of this study was to review the infectious diseases s

Delayed BCG immunization does not alter antibody responses to EPI vaccines in HIV-exposed and -unexposed South African infants.

BACKGROUND: Bacille Calmette-Guérin (BCG) is routinely given at birth in tuberculosis-endemic settings due to its protective effect against disseminated tuberculosis in infants. BCG is however contraindicated in HIV-infected infants. We investigated whether delaying BCG vaccination to 14 weeks of age affected vaccine-induced antibody responses to Haemophilus influenzae type b (Hib)-conjugate, pertussis, tetanus and Hepatitis B (HBV) vaccines, in HIV-exposed uninfected (HEU) and -unexposed uninfected (HUU) infants. METHODS: Infants were randomized to receive BCG at birth or at 14 weeks of age.

Educating international students about tuberculosis and infections associated with travel to visit friends and relatives (VFR-travel).

BACKGROUND: International students in Victoria, Australia, originate from over 140 different countries. They are over-represented in disease notifications for tuberculosis and travel-associated infections, including enteric fever, hepatitis A, and malaria. We describe a public health initiative aimed to increase awareness of these illnesses among international students and their support staff. METHODS: We identified key agencies including student support advisors, medical practitioners, health insurers, and government and professional organisations. We developed health education materials tar

Continuing challenge of infectious diseases in India.

In India, the range and burden of infectious diseases are enormous. The administrative responsibilities of the health system are shared between the central (federal) and state governments. Control of diseases and outbreaks is the responsibility of the central Ministry of Health, which lacks a formal public health department for this purpose. Tuberculosis, malaria, filariasis, visceral leishmaniasis, leprosy, HIV infection, and childhood cluster of vaccine-preventable diseases are given priority for control through centrally managed vertical programmes. Control of HIV infection and leprosy, but

Miliary tuberculosis in children. A clinical review.

Eighty cases of miliary tuberculosis admitted to our hospital between January 1981 and December 1984 were reviewed. The age of the patients ranged from 3 months to 12 years, with an average of 2 years 2 months (26.5 months). Nine cases (11.25%) died during hospitalization due to the severe condition at the time of admission. Only 8 patients (10%) were in good nutritional condition. Seventy-two patients (90%) had been visiting the primary health care clinic for several times since 2-3 months but were never diagnosed as suffering from tuberculosis. Fever or recurrent fever were found in 78 cases

Silent Renal Autodestruction: Incidental Discovery of End-Stage Genitourinary Tuberculosis (Putty Kidney) 14 Years After Treatment for Pulmonary Tuberculosis With Multisystem Sequelae.

Genitourinary tuberculosis (GUTB) is a prevalent yet frequently underdiagnosed form of extrapulmonary tuberculosis (TB), owing to its characteristically indolent and asymptomatic clinical course. In its most advanced form, renal TB culminates in complete destruction and dystrophic calcification of the renal parenchyma, a condition known as "putty kidney" or autonephrectomy. This entity can develop silently over many years, even in patients who have completed antitubercular therapy for pulmonary disease. A 44-year-old woman with a remote history of pulmonary TB treated 14 years prior presented
