Abdominal tuberculosis. A case report.

Tuberculous involvement of the abdominal parenchymatous organs is usually a diffuse process. The macronodular form of hepatosplenic tuberculosis and focal involvement of the pancreas are extremely rare. This report describes CT findings of abdominal parenchymatous organ involvement in a patient with pulmonary and vertebral tuberculosis. Although CT does not confirm a diagnosis of abdominal tuberculosis, it is a valuable examination that can support the diagnosis and define the extent of disease.

[Disseminated tuberculosis of unusual locations].

We report a case of disseminated tuberculosis involving the middle ear, the central nervous system, the spine and the lung. The tuberculous epidural abscess and otomastoiditis don't have any specific imaging features. But their coexistence with an other tuberculous involvement might suggest their tuberculous nature. The epidural abscess may result from direct extension from otomastoiditis.

Intramedullary cervical spinal cord tuberculoma: A rare cause of paraparesis.

Neurological manifestations of tuberculosis remain rare, even if recent years have been marked by an increase in these pathologies, notably due to the HIV pandemic. Intramedullary tuberculoma remains an exceptional localization. Magnetic resonance imaging in diagnosing intramedullary tuberculoma and specific medical treatment are of great interest. We report a case of cervical intramedullary tuberculoma in a 45-year-old woman, with a slow spinal cord compression syndrome. The nature diagnosis had been retained on a clinical and radiological basis, and appropriate medical treatment was establis

Paradoxical Reaction to Antitubercular Treatment in a Case of Tuberculous Meningitis.

Tuberculous meningitis (TBM) is a challenging disease to treat, as Mycobacterium tuberculosis infects the meninges, which are the outer membranes of the brain and the spinal cord. The majority of patients diagnosed with TBM acquire various other central nervous system complications, and as a result, treating the disease becomes a challenging task. A paradoxical reaction to the treatment may occur in the course of managing TBM. This case study describes a 20-year-old Southeast Asian female who was diagnosed and treated for TBM and subsequently developed a resurgence of the disease due to a para

Our surgical approach towards the treatment of urethrocele and urethral fistula.

A new technique, described in the text, has been elaborated inspired by that of Monseur (1968) for urethral techniques. It has been performed with success in three paraplegics and in one incomplete tetraparesis. The plastic reconstruction of the diseased part of the urethra after excision of the stenosis and a fistula or diverticula by rotation and fixation to the subcavernal groove creates, in fact, an enlarged neo-urethra rendering the recurrence of the primary lesion practically impossible. This technique, first applied to spinal cord injuries, has been extended with permanent success to ot

Paradoxical reactions in pediatric HIV-negative CNS tuberculosis: A longitudinal cohort study.

OBJECTIVE: Paradoxical reactions (PR) in pediatric HIV-negative CNS tuberculosis remain underexplored. This study prospectively analyzes incidence and pattern of PR and its impact on functional outcome. METHODS: Children (6 months-14 years) with newly diagnosed CNS Tuberculosis using the Lancet Consensus Scoring System, were enrolled. All participants received standard anti-tubercular therapy (ATT). Clinical evaluation, CSF analysis and neuroimaging were performed at baseline, during clinical worsening, or at 8 weeks. PR patterns defined as worsening or new tuberculosis lesions or symptoms fo

Acute myelopathy or cauda equina syndrome in HIV-positive adults in a tuberculosis endemic setting: MRI, clinical, and pathologic findings.

BACKGROUND AND PURPOSE: Cape Town is the center of an HIV-tuberculosis coepidemic. This study's aim was to highlight the importance and to describe the MR imaging features of tuberculosis in acute myelopathy and cauda equina syndrome in HIV-positive adults. To accomplish this we retrospectively reviewed the MR imaging and clinico-pathologic findings of HIV-positive patients presenting to our hospital with recent onset paraplegia and sphincter dysfunction over a 4-year period, 2008-2011. MATERIALS & METHODS: MR imaging, CD4 count, and CSF analysis and pathology were correlated in 216 cases. R

The application of endoscopic debridement combined with metagenomic next-generation sequencing technology in primary spinal infections: a retrospective study.

PURPOSE: Spinal endoscopy is a novel minimally invasive spinal surgery technique used in recent years to treat various degenerative spinal diseases. Metagenomic next-generation sequencing (mNGS) is a new method for identifying infectious microorganisms in infectious diseases. We aim to evaluate the application effect of combining spinal endoscopy with mNGS in diagnosing and treating spinal infections. METHODS: The clinical data of 62 patients with suspected spinal infectious diseases admitted from January 2020 to December 2023 were retrospectively analyzed. All patients underwent spinal endos

Spinal intramedullary tuberculoma in a 3-year-old girl.

BACKGROUND: Spinal intramedullary tuberculoma (IMT) is a rare manifestation of extrapulmonary tuberculosis (TB). Presentation of TB in the pediatric age group is a significant contributor to mortality. CASE DESCRIPTION: A young vaccinated girl presented to the neurosurgery department with difficulty walking and urinary incontinence. A magnetic resonance imaging performed outside the hospital showed a hyperintense intramedullary lesion extending from T6 to T9. The patient underwent T6-T9 laminoplasty with intramedullary lesion decompression under neuromonitoring. The dense adherence of the les

Central Nervous System Tuberculoma Complicated with Spinal Arachnoiditis in Immunocompetent Patient.

BACKGROUND: Central nervous system (CNS) tuberculosis (TB) can manifest as meningitis, abscess, tuberculoma or other manifestations. CNS TB is a rare TB complication, and its diagnosis remains a challenge since it has clinical and imaging features that resemble other diseases. The antituberculosis treatment has a significant role in determining a patient's outcome and prevent complications and mortality. CASE PRESENTATION: Here, we report a case of CNS TB manifested as tuberculoma in immunocompetent patient complicated with spinal arachnoiditis. Despite a treatment delay, the patient still sh

Tuberculous spondylitis in patients with end-stage renal disease undergoing chronic hemodialysis therapy.

Tuberculosis of the spine is not rare in immunocompromised patients and particularly in those with end-stage renal disease (ESRD). Furthermore, the possible vascular compromise of the spinal cord in patients with diabetic nephropathy may result in symptoms of neurological involvement that could lead to deterioration and paralysis. We report a series of 4 patients with ESRD undergoing dialysis that developed tuberculous spondylitis of the thoracic spine. Diabetic nephropathy was the primary cause for chronic kidney disease in 2 patients; 3 of these patients were treated conservatively with anti

Immunodiagnosis of tuberculous meningitis: detection of antibody reactivity to antigens of Mycobacterium tuberculosis and Cysticercus cellulosae in cerebrospinal fluid tuberculous meningitis patients by ELISA.

Enzyme-linked immunosorbent assay (ELISA) was standardized and evaluated for detection of antibody response in cerebrospinal fluid (CSF) to antigens of Mycobacterium tuberculosis and Cysticercus cellulosae. Sonicated extracts of heat killed M. tuberculosis H37Rv and C. cellulosae were prepared and used in ELISA to detect respective antibody response in CSFs for a definitive diagnosis as to tuberculous meningitis (TBM)/neurocysticercosis (NCC). ELISA was performed in a total of 201 CSF samples, which include Group I: chronic infections of the central nervous system (CNS) with possible diagnosis

Management of Craniovertebral Junction Tuberculosis Presenting with Atlantoaxial Dislocation.

Tuberculosis (TB) rarely involves the craniovertebral junction (CVJ). Atlantoaxial dislocation (AAD) is one of the most commonly encountered lesions in craniocervical TB. The incidence of TB and its craniovertebral manifestation is increasing even in developed countries because of intercontinental migration and increased prevalence rates of immunosuppression conditions. While the treatment of craniovertebral TB is well standardized and relies on conservative measures, the treatment of TB with AAD is disputable. In this paper we present a review of the literature and elucidate our approach to c

Pott's disease is a health problem in developing countries and its diagnosis in children is a challenge. Here we present the case of a two-year-old boy with Pott's disease involving T1 to T3 thoracic vertebrae. The clinical presentation was characterized by difficulty walking, fever, cough, and dyspnea. At physical examination, kyphosis and bony prominence were observed in the cervicodorsal area. A positive tuberculin test was obtained, and Mycobacterium tuberculosis was isolated via culture of the gastric aspiration sample. The spine MRI showed a chronic abscess, destruction of two vertebrae,
