Urinary exosomal miRNA profiling reveals sensitive non-invasive detection of bladder cancer.

BACKGROUND: Urinary microRNAs (miRNAs) hold substantial promise as non-invasive biomarker candidates due to their molecular stability and tumor-selective expression signatures. This study integrates the high-throughput urinary miRNA landscape with clinical data to systematically identify robust, non-invasive biomarkers that facilitate the early diagnosis and better clinical management of urinary bladder cancer (UBC). METHODS: Urine-derived miRNA sequencing was performed on UBC patients and healthy controls to delineate the miRNA expression profile. Computational filtering further excluded prec…

Obesity promotes urinary tract infection by disrupting bladder focal adhesion kinase signaling.

Obesity is a significant public health concern associated with numerous health risks. Infections are a major complication of obesity, but the mechanisms driving increased infection risk remain unclear. Using a diet-induced obesity mouse model in male and female C57BL/6 mice, we show that high-fat diet feeding heightens vulnerability to uropathogenic E. coli (UPEC) urinary tract infection. Transcriptomic analysis of bladder urothelial cells revealed sex-specific gene expression changes with the shared activation of focal adhesion and extracellular matrix pathways. Western blotting and immunosta

Autovaccine immunoprophylaxis in patients with neurogenic bladder experiencing recurrent urinary tract infections.

INTRODUCTION: Neurogenic Bladder (NB) patients are highly susceptible to recurrent urinary tract infections (UTIs), often requiring repeated hospitalizations and prolonged antibiotic use. Current preventive strategies, including long-term antibiotics, frequently fail due to resistance and limited efficacy. Autovaccination, a form of personalized immunoprophylaxis using inactivated patient-specific bacterial strains, has shown potential in reducing UTI recurrence but has not been well studied in NB populations. METHODS: A prospective, single-center study was conducted at the Hospital Universit

Montelukast Improves Urinary Bladder Function After Complete Spinal Cord Injury in Rats.

Bladder dysfunction is among the most drastic and quality-of-life-reducing conditions after spinal cord injury (SCI). Neuroinflammation in the lower urinary tract (LUT) after SCI could be a key driver of neurogenic bladder dysfunction and tissue fibrosis. Leukotrienes, a group of highly active lipid mediators, are potent inflammatory mediators. Here, we explored the potential of early montelukast (MLK) therapy, a cysteinyl leukotriene receptor 1 antagonist, on LUT function and structure four weeks after severe SCI in rats. Rats (strain Lewis, female, n = 50) received a permanent bladder cathet

Bladder Diary Feasibility and Reliability in Neurogenic Lower Urinary Tract Dysfunction Patients.

OBJECTIVES: Bladder diary (BD) is an important tool to understand patients' lower urinary tract symptoms. But few data exist on adult neurogenic lower urinary tract dysfunction (ANLUTD) patients. The aim was to assess in real-life practice the feasibility, reliability and influencing factors of BD completion in patients with ANLUTD. MATERIALS AND METHODS: All patients with ANLUTD who had a first neuro-urologic consultation between January 2022 and April 2023 were included. No patients underwent intermittent self-catheterization. We collected demographic data, validated questionnaires about sy

Preoperative Bladder Scanning Can Predict Postoperative Urinary Retention Following Total Joint Arthroplasty.

BACKGROUND: As total joint arthroplasty (TJA) moves to the outpatient setting, it is becoming clear that postoperative urinary retention (POUR) is a potential impediment to same-day discharge. Although risk factors for POUR have been widely studied, the lack of their clinical utility warrants investigation of specific preoperative factors that can assist in surgical planning and patient optimization. The purpose of the current study was to determine whether preoperative symptom surveys and bladder scanning are useful tools in identifying POUR risk. METHODS: We performed a prospective analysis

[Incidence of urinary tract infection after urodynamics among patients with neurogenic bladder].

OBJECTIVE: The indication for prophylactic antibiotic therapy during cystomanometry to prevent urinary tract infection (UTI) has long been recommended. However, limited data support this approach. Our study aims to evaluate the incidence of UTIs following cystomanometry without prophylactic antibiotics in patients with neurogenic bladder. MATERIALS AND METHODS: This prospective, monocentric study was conducted from February 2023 to March 2023 at a specialized neuro-urology center, including all consecutive patients with lower urinary tract disorders of neurogenic origin referred for cystomano

Bladder Cancer detection by urinary methylation markers GHSR/MAL: a validation study.

PURPOSE: Although cystoscopy is a reliable tool for detecting bladder cancer, it poses a high burden on patients and entails high costs. This highlights the need for non-invasive and cost-effective alternatives. This study aimed to validate a previously developed urinary methylation marker panel containing GHSR and MAL. METHODS: We enrolled 134 patients who underwent cystoscopy because of hematuria, including 63 individuals with primary bladder cancer and 71 with non-malignant findings. Urine samples were self-collected at home and sent via regular mail. Subsequently, DNA was extracted and th

Diagnostic accuracy of urinary cytokeratin fragment-19 (CYFRA21-1) for bladder cancer.

Bladder cancer (BC) is known for its high recurrence rate and requires constant patient monitoring. To confirm the diagnosis, a tissue sample from a cystoscopy is required, which the patient often avoids. Urine has the potential to be utilized as a diagnostic fluid because of its non-invasive nature and various biomarker contents. The aim of this study was to determine the diagnostic value of cytokeratin fragment-19 (CYFRA21-1) levels in urine for diagnosing BC. This single-center cross-sectional study included adults aged ≥18 years who presented with hematuria and had suspected BC based on im

Obstructive uropathy from a giant urinary bladder stone: a rare urological emergency.

A 53-year-old man presented with lower urinary tract symptoms and recurrent urinary tract infections since last 3 years without being investigated or treated properly. Examination revealed a hard mobile lump in the pelvis, and blood investigations showed raised serum creatinine of 2.9 mg/dL. Subsequent urgent ultrasound scan showed a large urinary bladder stone with bilateral hydroureteronephrosis, and X-ray kidney, ureter and bladder demonstrated a 9 cm×6 cm elliptical radio-opaque shadow in the pelvis. He underwent emergency admission followed by open cystolithotomy on the next day. He was d…

Urinary bladder stone due to retained indwelling ureteral stent: A case report.

RATIONALE: The indwelling ureteral stents is a common procedure in routine urological practice. The double-J (D-J) stent is the most common type of stents used and is indicated mainly for short-term urinary drainage and prevention of obstruction and infection. However, prolonged indwelling stents may result in disastrous complications, such as hematuria, infection, encrustation, and stone formation. In this context, the persistence of stent in situ might play a key role as a nidus in deposition of urinary sediment, then forming calculus. Although the encrustation may become more serious as tim

[Follow-up surveillance of muscle-invasive urinary bladder cancer after curative treatment].

Follow-up care of patients with muscle-invasive bladder cancer is subdivided into oncological and functional surveillance. More than 80% of local relapses and distant metastases occur within the first 2 years. Recurrences in the remnant urothelium also occur several years after radical cystectomy. Urinary cytology and a computed tomography (CT) scan of the abdomen and thorax including a urography phase are the standard diagnostics for tumor follow-up. There is no clear evidence for a survival benefit for the detection of asymptomatic vs. symptomatic recurrences. After partial cystectomy or tri

Bladder-Drained Pancreas Transplantation: Urothelial Innate Defenses and Urinary Track Infection Susceptibility.

BACKGROUND: Pancreas transplantation restores insulin secretion in type 1 diabetes mellitus. The graft also produces exocrine secretions that can be drained enterically (enteric drainage [ED]) or via the bladder (bladder drainage [BD]). We suggest that in BD transplants, such secretions destroy bladder innate immunity, specifically host defense peptides/proteins (HDPs), which increases patient susceptibility to recurrent urinary tract infections (rUTIs). MATERIALS AND METHODS: BD and ED patient records were reviewed retrospectively for UTIs. Urine samples from ED and BD transplant recipients

Lower Urinary Tract Symptoms Including Bladder Outlet Obstruction: What's New in Diagnostics?

UNLABELLED: It is well known that lower urinary tract symptoms (LUTS) have a multifactorial etiology. Therefore, there is an increased need for improved diagnostics for the optimal assessment of patients with LUTS in order to identify the various causes of symptoms and improve disease phenotyping. Research has focused on the development of improved urodynamic technologies and the development of noninvasive tests that could replace urodynamic studies. This mini-review will present the most recently published studies on the topic including new technologies in the diagnosis of bladder outlet obst
