The role of Doppler echocardiography in the diagnosis, follow-up, and management of ventricular septal defects.

The defects of the ventricular septum have received special attention from investigators working in echocardiography. The method showed an incomparable capability to identify all the morphological features of the defects. The increasing improvement in definition of transducers associated with conventional and color Doppler contributed significantly to the reliability to detect most of the defects. The great majority of associated lesions can be easily identified and serial examinations allow prediction of which defect may become smaller or even close spontaneously as well as which have acquire

Modified Konno operation for diffuse subaortic stenosis.

The modified Konno operation is designed to provide relief of diffuse subaortic stenosis, while preserving the native aortic valve. The aorta and the right ventricular infundibulum are opened. The upper part of the subaortic stenosis is incised through the aortic orifice. The conal septum is incised and the septotomy is extended across the stenotic area. The obstructive tissue is removed (mainly from the left-handed rim of the septotomy) and the conal septum is enlarged with a prosthetic patch. The aorta is closed and the right ventricular infundibulum is enlarged. Early and late mortality rat

Transseptal BATMAN for High-Risk Valve-in-Ring Procedures: A Case Series.

Valve-in-ring procedures represent a feasible solution for high-risk patients with surgical repair failure. The risk of left ventricular outflow tract obstruction increases the challenge, and transcatheter approaches to prevent it are technically demanding and often do not resolve it. We demonstrate the feasibility and safety of a transseptal balloon-assisted translocation of the anterior mitral leaflet for valve-in-ring implantation.

Systolic anterior motion of mitral valve with calcified annulus in octogenarians.

Systolic anterior motion of the mitral leaflet causing left ventricular outflow tract obstruction is commonly seen in hypertrophic cardiomyopathy and also in patients with advanced mitral valve disease with excessive anterior leaflet tissue or a reduced aortic-mitral angle. We describe 2 octogenarians who presented with systolic mitral leaflet anterior motion in advanced mitral valve disease with severe mitral annular calcification and associated asymmetrical septal hypertrophy.

Recurrent subaortic membrane causing subvalvular aortic stenosis 13 years after primary surgical resection.

This case demonstrates the use of cardiac CT angiography as part of the preoperative evaluation of a recurrent subaortic membrane in a 23-year-old women. A subaortic membrane is a rare finding on coronary CT angiography and typically consists of a thin fibrous membrane in the left ventricular outflow tract (LVOT). The resulting turbulent flow may lead to LVOT obstruction and aortic insufficiency.

Results of operation for obstructive hypertrophic cardiomyopathy in the elderly. Septal myotomy and myectomy in 20 patients 65 years of age or older.

The results of operative treatment in 20 patients with hypertrophic cardiomyopathy 65 years of age or older are described. All 20 patients were severely symptomatic before operation. Seventeen of the 20 patients had marked left ventricular outflow tract obstruction under basal conditions (average pressure gradient 107 mm Hg); the other three patients had outflow gradients of more than 50 mm Hg only with provocation. One patient died at operation, and one died 4 years after operation of circumstances unrelated to hypertrophic cardiomyopathy. Sixteen of the 18 long-term survivors have had persis

Novel insights into the clinical manifestations and treatment of hypertrophic cardiomyopathy.

Knowledge of the pathophysiology, clinical manifestations, and treatment of hypertrophic cardiomyopathy continues to evolve. In subaortic obstructive hypertrophic cardiomyopathy, the importance of elongated mitral leaflets in causing the outflow obstruction and concomitant mitral regurgitation has been further elucidated, and marked worsening of the obstruction during upright exercise has been clearly demonstrated. Treatment options now include dual chamber pacing as well as medical and surgical therapy. The syndrome of midventricular obstruction with apical infarction and aneurysm formation m

Successful correction of double-outlet right ventricle with a ventricular D-l-malposition of the great arteries, bilateral conus, pulmonary stenosis and subaortic ventricular septal defect.

The authors present the case of a fifteen-year old girl with double outlet right ventricle with ventricular d-loop and l-malposition of the great arteries, bilateral conus, pulmonary stenosis and subaortic ventricular septal defect, who was operated on successfully. This is the fourth case of double outlet right ventricle with l-position of the aorta that has been surgically corrected. The subaortic position of the interventricular defect favours the creation of the tunnel connecting the left ventricle with the aorta without obstructing the right ventricular outflow tract. The patient was doin

Paradoxical response to exercise in asymptomatic hypertrophic cardiomyopathy: a new description of outflow tract obstruction dynamics.

OBJECTIVES: The purpose of this study was to analyze left ventricular obstruction in patients with hypertrophic cardiomyopathy (HCM) during exercise echocardiography. BACKGROUND: Despite the association of symptoms with left ventricular outflow tract obstruction in HCM, there exist paradoxical situations in which significant intraventricular gradients (>50 mm Hg) at rest occur in conjunction with excellent exercise tolerance. METHODS: To examine this phenomenon, we performed exercise echocardiography and analyzed the clinical status of 107 HCM patients with and without resting obstruction.

Adverse effects of nifedipine therapy on hypertrophic obstructive cardiomyopathy.

A 62-year-old man with obstructive hypertrophic cardiomyopathy was given sublingual nifedipine, 10 mg, during invasive hemodynamic monitoring. After 15 minutes, his left ventricular outflow gradient increased from 22 to 80 mm Hg while arterial pressure fell from 152/70 to 122/64 mm Hg. Left ventricular end-diastolic pressure increased from 15 to 22 mm Hg. These adverse hemodynamic responses may have been a result of vasodilation of the peripheral circulation induced by nifedipine. Thus, some patients with hypertrophic obstructive cardiomyopathy may develop serious hemodynamic compromise when t

[Pulmonary valve aplasia with ventricular septal defect. Hemodynamics and embryology].

The combination of absent pulmonary valve with ventricular septal defect, subvalvular pulmonic stenosis and aneurysm of the pulmonary artery presents a characteristic heamodynamic and clinical profile. It should be differentiated from other morphologically related cardiovascular malformations. According to the degree of right ventricular outflow tract obstruction, the circulatory pattern and the clinical picture is varying widely. We are differentiating an obstructive and a congestive form of the disease. An own case is demonstrated. After a short review of the literature the clinical and haem

Myectomy with secondary chordal resection in hypertrophic obstructive cardiomyopathy: the mid-term outcomes.

BACKGROUND: It is not clear whether septal myectomy with secondary chordal resection (SM-SCR) can achieve similar postoperative outcomes among hypertrophic obstructive cardiomyopathy (HOCM) patients exhibiting different severity of septal hypertrophy. METHODS: This was a single-center retrospective cohort study with a nested case-control study. HOCM patients received SM-SCR between 2014 and 2023 were retrospectively included and were grouped based on their septal thickness. The main outcome was the relief of the left ventricular outflow tract (LVOT) obstruction. Besides, predictors for recurr

Corrected transposition of the great arteries with isolated aortic coarctation: In utero echocardiographic diagnosis.

Physiologically corrected transposition of the great arteries (cTGA), defined by discordant atrioventricular and ventriculoarterial connections, is an uncommon congenital cardiac malformation. It rarely exists without associated cardiac anomalies, the most common of which are ventricular septal defect, pulmonary outflow obstruction, tricuspid valve (systemic) deformity, and rhythm disturbances. Conversely, hypoplasia of the systemic ventricle and systemic inflow or outflow obstructions have seldom been reported, although their recognition may significantly influence surgical repair and the pat
