Myval Beyond the Aortic Valve: Device-Anatomy Interaction, Procedural Strategy, and Clinical Evidence in Mitral, Tricuspid, and Pulmonary Positions.

The Myval balloon-expandable transcatheter heart valve was developed for transcatheter aortic valve implantation, but its broad 20-32 mm size matrix and controlled deployment have prompted use in non-aortic landing zones. This narrative review critically synthesizes Myval-specific case reports, case series, and observational cohorts, together with relevant platform-level evidence, addressing device design, anatomical selection, imaging, procedural strategy, clinical outcomes, and evidence gaps. Reported applications include mitral valve-in-valve, valve-in-ring, and valve-in-mitral annular calc

Efficacy of Combined Cox-Maze IV and Ventricular Septal Myectomy for Treatment of Atrial Fibrillation in Patients With Obstructive Hypertrophic Cardiomyopathy.

Atrial fibrillation (AF) has important clinical consequences in hypertrophic cardiomyopathy (HC). Safety and efficacy of the Cox-Maze IV procedure (when combined with ventricular septal myectomy) in patients with obstructive HC and paroxysmal AF is largely unresolved. Records of 395 consecutive HC patients (age 55 ± 13 years) who underwent septal myectomy for heart failure symptoms between 2004 and 2015 were reviewed. Sixty-two patients also had concomitant complete biatrial Cox-Maze IV for a history of symptomatic paroxysmal AF (3.0 ± 3.6 episodes) combined with myectomy comprise the study co

The Central Role of Echocardiography in Mavacamten Therapy for Obstructive Hypertrophic Cardiomyopathy: A Comprehensive Review.

Mavacamten is a cardiac myosin inhibitor used in adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM). Echocardiography is central to selecting suitable patients, confirming left ventricular outflow tract (LVOT) obstruction, excluding inappropriate use when left ventricular ejection fraction (LVEF) is < 55% and guiding dose titration during treatment. This narrative review summarises the practical echocardiographic approach to mavacamten therapy, with emphasis on reproducible LVOT gradient assessment, goal-directed Valsalva manoeuvres, LVEF monitoring, mitral valve and systoli

[Quantitative evaluation of right heart obstructions in children using Doppler echocardiography].

To assess the accuracy of pulsed and continuous-wave Doppler echocardiography (DE) for estimating pressure drops across right ventricular outlet obstruction, we examined 27 children (valvular pulmonary stenosis n = 16, complex congenital heart lesion with prior pulmonary banding n = 11). The Doppler evaluation was performed within 24 hours of cardiac catheterization in 17 patients, in 10 it was done 2-24 months (mean 12.2 months) after catheterization. We found a good correlation (r = 0.93) with slight underestimation of pulmonary valve gradient. Using three windows (left parasternal, supraste

A Ball Valve Thrombus in a Young Obese Male With Hyperlipidemia.

A ball valve thrombus is a common entity in rheumatic mitral stenosis where a large clot forms in the left atrial appendage, the clot detaches from the left atrial appendage, swirls in the left atrium in a clockwise manner, obstructs the mitral inflow producing cyclic acute pulmonary edema, and likely causes left ventricular outflow tract obstruction producing transient syncope or embolism to the brain producing dense hemiplegia. Besides rheumatic mitral stenosis, ball valve thrombus has been described in post-mitral valve replacement patients and restrictive cardiomyopathy. Left atrial myxoma

Persistent troponin elevation in the heart of Fabry disease due to coronary microvascular dysfunction.

Fabry disease is a rare X-linked lysosomal storage disorder caused by GLA variants leading to α-galactosidase A deficiency. The heterozygous phenotype predominantly affects the heart. We report a heterozygous woman in her 70s with known Fabry disease presenting with chest pain and palpitations despite well-controlled cardiovascular risk factors. High-sensitivity troponin remained persistently elevated. Coronary angiography showed non-obstructive arteries, and echocardiography revealed left ventricular hypertrophy without outflow tract obstruction. In the absence of epicardial disease, troponin

Clinical and echocardiographic variables fail to predict response to dual-chamber pacing for hypertrophic cardiomyopathy.

BACKGROUND: Although dual-chamber pacing therapy for obstructive hypertrophic cardiomyopathy has been relegated to a limited role, there remains a subset of patients who are not good candidates for more definitive therapies. OBJECTIVE: The goal of this investigation was to determine whether preprocedural variables could help identify patients most likely to respond. METHODS: Retrospective review of 84 patients with hypertrophic cardiomyopathy who underwent dual-chamber pacing for obstructive physiology at Mayo Clinic, Rochester, MN, included baseline demographics and echocardiographic featur

The Ross procedure in infants and young children.

BACKGROUND: This study reviews our experience with the Ross procedure in infants and young children. METHODS: From September 1993 to September 2004, 52 children less than 15 years of age underwent a Ross procedure. The patients ranged in age from 4 days to 15 years old (median, 5 years). Fifteen patients (29%) were less than 2 years of age. The predominant indication for the Ross procedure was aortic stenosis. Sixteen patients underwent a Ross-Konno procedure for severe left ventricular outflow tract obstruction. Thirty-four patients had 48 previous interventions. Preoperatively, 6 patients s

Transapical Beating-Heart Septal Myectomy for Obstructive Hypertrophic Cardiomyopathy: Lessons Learned After the Learning Curve Period.

BACKGROUND: The transapical beating-heart septal myectomy (TA-BSM) procedure was developed to enhance efficiency and reduce surgical trauma compared with conventional septal myectomy in treating obstructive hypertrophic cardiomyopathy. The current study aimed to delineate a refined TA-BSM surgical technique and summarize the midterm outcomes of the cohort immediately after the learning curve period. METHODS: Employing a beating-heart myectomy device and guided by real-time transesophageal echocardiography, TA-BSM was conducted through a left mini-thoracotomy. Multiple tailored resections were

[Cardiac pacing in obstructive hypertrophic cardiomyopathies].

Up until recently, the first-line treatment of obstructive hypertrophic cardiomyopathy was pharmacological and surgical in refractory cases. However, the immediate beneficial effect of a cardiac pacing on infra-aortic obstruction has been known for many years. The development of sophisticated two-chamber pacemakers and their use in patients with obstructive hypertrophic cardiomyopathy has confirmed their beneficial effect both in terms of haemodynamic and clinical parameters. There appears to be many reasons for this advantageous effect and only some of them have been elucidated, including alt

Biventricular repair of double-outlet left ventricle by handmade trileaflet-valved conduit: A case report.

RATIONALE: Double-outlet left ventricle (DOLV) is a rare congenital cardiac malformation in which both great arteries arise entirely or predominantly from the left ventricle. An extracardiac conduit is the first surgical option for repairing DOLV, specifically because its placement of the extracardiac conduit can be customized to accommodate all possible anatomical variations. The bovine jugular veins and homograft valves are often used as conduits. There have been no reports on the use of handmade trileaflet-valved conduits for correcting DORV. PATIENT CONCERNS: A 1-year old male was admitte

Idiopathic hypertrophic subaortic stenosis.

The clinical and laboratory findings in 29 patients with idiopathic hypertrophic subaortic stenosis are presented. Dyspnoea during exercise, angina pectoris, syncope combined with left ventricular hyperthrophy on ECG and chest X-ray and a systolic ejection murmur at the apex and the left sternal border are the most important findings. The findings were different in patients below and above 30 years of age. Most of the patients below 30 were in function group I, had a normal heart volume on chest X-+ray, and syncope was related to exercise. All patients above 30 had symptoms, nearly all were in

Concomitant treatment of sustained ventricular tachycardia and hypertrophic cardiomyopathy with transcoronary ethanol ablation: a case report.

BACKGROUND: The recommended treatment for recurrent ventricular tachycardia in patients with hypertrophic cardiomyopathy that is not amenable to defibrillator implantation due to shock burden is radiofrequency ablation. In patients with deeply intramural foci of ventricular tachycardia, traditional unipolar ablation has a lower probability of success. CASE SUMMARY: A 66-year-old Caucasian man was admitted with ventricular tachycardia, which recurred despite antiarrhythmic drugs. On cardiac magnetic resonance imaging, he was discovered to have septal hypertrophic cardiomyopathy, which was not

Progress in perioperative echocardiography: focus on safety, clinical outcomes, 3-dimensional imaging, and education.

Gastric decompression with an orogastric tube after anesthetic induction does not appear to enhance image quality for routine cases. The insertion of a transesophageal echocardiographic (TEE) probe can cause significant upper-airway trauma, which can be minimized with rigid laryngoscopy. Limited TEE imaging without transgastric views appears to be safe and clinically adequate in patients with advanced liver disease and esophageal varices. Although esophagogastric perforation because of transesophageal echocardiography is rare, the risk is significantly higher with advanced age and female sex.

Ventricular septal defect following mitral valve replacement.

Mitral valve surgery (MVS), with repair preferred to replacement, is a common procedure for the treatment of severe primary mitral regurgitation related to leaflet prolapse. Structural complications after MVS include left ventricular outflow obstruction, paravalvular leak and atrial septal defect. Intraoperative transoesophageal echocardiography and predischarge transthoracic echocardiography (TTE) specifically screen for these complications. Ventricular septal defect (VSD), a known complication after aortic valve surgery, is rarely reported after MVS. Recently, unsuccessful valvuloplasty prio

A review of the Nunn modified single patch technique for atrioventricular septal defect repair.

Atrioventricular septal defect (AVSD) is a common congenital cardiac surgical problem. Over the years, younger and smaller infants are having operations for this condition before irreversible cardio pulmonary changes occur. Traditionally a single or two patch techniques have been used to repair this defect. However, in the past two decades an innovative method of modified single patch technique popularized by Dr. Graham Nunn has gained worldwide popularity. This review discusses the origin, surgical principles, technique and outcomes of this method, popularly known as Nunn or Australian techni
