Comparing Outcomes of Robotic and Open Inguinal Lymph Node Dissection in Patients with Carcinoma of the Penis.

PURPOSE: We compared outcomes between robot-assisted video endoscopic inguinal lymphadenectomy and open inguinal lymph node dissection in patients without bulky nodal metastasis in a tandem contemporary cohort. MATERIALS AND METHODS: We retrospectively analyzed a prospectively maintained hospital registry of 51 patients who underwent robot-assisted video endoscopic inguinal lymphadenectomy and 100 treated with open inguinal lymph node dissection from 2012 to 2016 for groins without bulky nodal metastasis and who had a minimum 9-month followup. Complications were graded by the Clavien-Dindo cl

Diagnosis of Mesothelioma Using Flexible Bronchoscopy With Endobronchial Ultrasound-guided Biopsy of Mediastinal Lymph Nodes.

Malignant mesothelioma is a neoplasm that can arise from the mesothelial surface of the pleura and tends to be locally invasive. Various techniques have been used for diagnosis and include thoracentesis with pleural fluid cytology, closed pleural biopsy, and video-assisted thoracoscopic surgery. Thoracentesis and closed pleural biopsy can often establish the presence of malignancy, but frequently do not provide sufficient tissue to definitively diagnose malignant mesothelioma. Video-assisted thoracoscopic surgery biopsy has higher sensitivity for establishing the diagnosis, but is a more invas

Focusing the surgical microscope.

A well-focused operating microscope addresses several needs that are all secondary to the surgeon's need to see clearly at all times. These needs include: the assistant; the sharpness of the video and monitor; as well as field of view, asthenopia, and focusing issues related to zoom, accommodation, and presbyopia. We provide a practical approach to achieve optimal focus that we call the sloping paper calibration method.

Three-Dimensional, High-Definition Exoscopic Anterior Cervical Discectomy and Fusion: A Valid Alternative to Microscope-Assisted Surgery.

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) remains one of the most commonly taught procedures during residency and one of the most frequently performed by neurosurgeons. Neurosurgeons use microscopes to perform surgery and to train other surgeons. Although the microscope provides excellent illumination and magnification, its use will be limited to 2 people: the surgeon and the assistant. Consequently, the scrub nurse and residents watching 2-dimensional images on monitors will have a reduced perception of the surgical field depth and anatomical details. The exoscope has been in

Video-assisted thoracoscopic resection of a thoracic inlet schwannoma with neuroforaminal involvement: a case report.

Tumors of the posterior mediastinum, particularly when involving the neural foramina, are typically resected via thoracotomy or by a hybrid method with a combination of video-assisted thoracoscopy and open surgery. However, in the appropriate anatomic and clinical context, a video-assisted thoracoscopic approach may be feasible, and such an approach may decrease postoperative pain and hospital length of stay. We present a patient with a benign schwannoma of the thoracic inlet and posterior mediastinum with symptomatic mass effect on surrounding structures. Extensive interdisciplinary discussio

Postoperative chylothorax with a duplicated left-sided thoracic duct: a case report and review of the literature.

Postoperative chylothorax is a potentially lethal complication after esophagectomy. A 53-year-old woman underwent subtotal esophagectomy. The thoracic duct was resected due to swollen lymph nodes. Postoperative chylothorax was diagnosed but conservative treatment was ineffective. Lipiodol lymphangiography revealed leakage from a duplicated left-sided thoracic duct. Left-sided video-assisted thoracoscopic ligation of the left-sided thoracic duct was performed. Because anatomical variations in the thoracic duct contribute to refractory chylothorax, lymphangiography is useful in detecting the pos

Partial anomalous pulmonary venous connection in right lung cancer: report of a case.

A partial anomalous pulmonary venous connection (PAPVC) is an uncommon congenital anomaly which is frequently associated with congenital heart disease such as an atrial-level shunt. This report documents the case of an 81-year-old man with PAPVC which was incidentally discovered during a right upper lobectomy for lung cancer. Surgery was performed through a minithoracotomy of an auscultatory triangle using a video-assisted procedure (video-assisted thoracic surgery: VATS). Although the ramus lobi medii was connected normally, the right superior lobe vein was found to drain into the superior ve

Videoendoscopic procedures in thoracic surgery: technical aspects and report of removal of a mediastinal cyst.

Current videoendoscopic technology and percutaneous techniques of exposure and dissection have been successfully applied to abdominal surgery with favorable results. Application of this technology to our practice of thoracic surgery is the basis of this report. Video-assisted thoracic surgery was performed in 36 patients for the following indications: Raynaud's syndrome, undefined pulmonary nodule, persisting spontaneous pneumothorax, T1 bronchial carcinoma, and mediastinal cyst. Videoendoscopic surgical procedures were accomplished using double-lumen endotracheal anaesthesia and a percutaneou

Video-assisted anal fistula treatment: technical considerations and preliminary results of the first Brazilian experience.

BACKGROUND: Anorectal fistula represents an epithelized communication path of infectious origin between the rectum or anal canal and the perianal region. The association of endoscopic surgery with the minimally invasive approach led to the development of the video-assisted anal fistula treatment. AIM: To describe the technique and initial experience with the technique video-assisted for anal fistula treatment. TECHNIQUE: A Karl Storz video equipment was used. Main steps included the visualization of the fistula tract using the fistuloscope, the correct localization of the internal fistula op

What to do when the parathyroid gland is not found during a transcervical exploration?

The most frequent cause of suboptimal results in a parathyroid adenoma resection is an ectopic location, mainly in the anterior mediastinum. These cases may not always be resected through a traditional cervical access. We present 2 cases of primary hyperparathyroidism who underwent an unsuccessful bilateral cervical exploration due to parathyroid tissue located inside the thymic gland. A video-assisted thoracoscopic surgery thymectomy with intraoperative determination of blood parathormone levels was performed. A 50% reduction of intraoperatory parathyroid hormone blood from the highest basal

Extralobar sequestration with a pulmonary arterial feeding vessel.

Extralobar sequestrations are rare congenital malformations of the lung. They are usually located between the diaphragm and left lower lobe and receive their blood supply from the aorta. We report a case of extralobar sequestration in an atypical location with a pulmonary arterial feeding vessel. An 18-year-old woman presented with an abnormality on chest X-ray. Chest computed tomography revealed a lung field not communicating with the bronchus between the upper and lower right lung lobes. Three-dimensional reconstruction computed tomography demonstrated a feeding artery from the pulmonary art

[Esophageal bronchogenic cyst: an uncommon cause of dysphagia in adults. Case report and literature review].

BACKGRONUND: Bronchogenic cysts result from abnormal budding of the primitive tracheobronchial tube and are rare congenital cystic lesions. The location of the cyst depends on the embryological stage of abnormal budding. Although periesophageal bronchogenic cysts have been frequently reported, a completely intramural cyst is very rare. CLINICAL CASE: A 42-year-old female patient, a three-month course with retrosternal pain associated with food intake, accompanied by intermittent dysphagia to solids. Esophagogram, high resolution thoracic tomography and endoscopic ultrasound are performed, con

[Thoracic sympathectomy in palmar hyperhidrosis: comparison of open with thorascopic procedure].

Between 1976 and 1994 we performed 26 thoracic sympathectomies for treatment of therapy-resistant palmar hyperhidrosis. Until the end of 1992 the operation was performed using an open transaxillary approach, since 1993 sympathectomy was done by video-assisted thoracoscopy. Both procedures consisted in excision of the thoracic ganglia T2 to T5. The only complication was a pneumothorax in the open surgery group (successfully treated by drainage). Compensatory sweating occurred in 70% of our patients, compared to results in the literature of 60-90%. We did not note further complications, e.g. no

From Thoracotomy to Robotic Platforms in Thoracic Surgery: Technical Evolution, Simulation-Based Training, and Surgical Outcomes.

Thoracic surgery has progressively moved from open thoracotomy to video-assisted and robot-assisted approaches, reducing access-related trauma while changing the technical environment in which pulmonary resections are performed and taught. This narrative review examines the clinical evolution of open, video-assisted thoracic surgery (VATS), and robot-assisted thoracic surgery (RATS), with particular attention to the evidence supporting their perioperative and oncological outcomes, the learning curves associated with minimally invasive surgery, and the role of simulation-based training. Minimally invasive surgical implantation of left ventricular epicardial leads for ventricular resynchronization using video-assisted thoracoscopy.

BACKGROUND AND OBJECTIVES: Cardiac resynchronization via left ventricular or biventricular pacing is an option for selected patients with ventricular systolic dysfunction and widened QRS complex. Stimulation through a coronary vein is the technique of choice for left ventricular pacing, but this approach results in a failure rate of approximately 8%. We describe our initial experience with minimally invasive surgical implantation of left ventricular epicardial leads using video-assisted thoracoscopy. PATIENTS AND METHOD: A total of 14 patients with congestive heart failure, NYHA functional cl

Robotic resection of an esophageal leiomyoma.

The use of robotic thoracic surgery as an alternative to open resections or video assisted thoracoscopy has increased in recent years due to its many potential benefits to patients. These benefits include less pain and therefore less use of pain medication, shorter hospital stays, faster recovery, and fewer complications.  This video tutorial describes the robotic resection of a 37-mm by 30-mm esophageal leiomyoma.

Evolution of mitral valve surgery: toward a totally endoscopic approach.

BACKGROUND: Our study evaluates a series of video-assisted minimally invasive mitral operations, showing safe progression toward totally endoscopic techniques. METHODS: Consecutive patients with isolated mitral valve disease underwent either manually directed (n = 55) or voice-activated robotically directed (n = 72) video-assisted mitral operations. Cold blood cardioplegia, a transthoracic aortic clamp, a 5-mm endoscope, and a 5-cm minithoracotomy were used. This video-assisted minimally invasive mitral operation cohort was compared with a previous sternotomy-based mitral operation cohort (n
