Maxillary sinus floor elevation using a tissue-engineered bone complex with OsteoBone and bMSCs in rabbits.

OBJECTIVES: To evaluate the effects of maxillary sinus floor elevation by a tissue-engineered bone complex with OsteoBone(trade mark) and bone marrow stromal cells (bMSCs) in rabbits. MATERIAL AND METHODS: Autologous bMSCs from adult New Zealand rabbits were cultured and combined with OsteoBone(trade mark) at a concentration of 20 x 10(6) cells/ml in vitro. Twenty-four animals were used and randomly allocated into groups. For each time point, 16 maxillary sinus floor elevation surgeries were made bilaterally in eight animals and randomly repaired by bMSCs/material (i.e. OsteoBone), material,

Use of titanium mesh in lieu of a fixation screw to stabilize an autogenous block graft: a case report.

Localized alveolar augmentation is carried out either before, during, or sometimes after implant placement. The placement of autogenous graft as a block or a particulate alone or in combination with an organic bone mineral has been practiced with a great deal of success. The block graft is secured in place with a screw and protected by a membrane. This case report describes the treatment of a female patient with a bucco-lingual bone deficiency grafted with autogenous block and a titanium mesh. Five months later, implant was placed in the grafted area. The definitive prosthesis was cemented 6 m

Immediate implantation with ridge augmentation using acellular dermal matrix and deproteinized bovine bone: a case report.

Immediate implant placement combined with hard and soft tissue grafting has been suggested because it may preclude dramatic postextraction bone loss and may decrease overall discomfort with reduction in the number of surgeries and in treatment time. In this case report, the acellular dermal matrix was used as a membrane for immediate implantation via a bone augmentation procedure in an esthetically challenging situation. The author suggests that this surgical technique provides the clinician with an option for an immediate implant therapy when primary closure is not intended. Additional random

Clinical evaluation of demineralized bone allograft in a hyaluronic acid carrier for sinus lift augmentation in humans: a computed tomography and histomorphometric study.

OBJECTIVES: Natural and synthetic graft materials are used routinely in sinus floor augmentations to help support implants in atrophic maxillary ridges. This clinical study was based on the hypothesis that the clinical effectiveness of demineralized freeze-dried bone allograft/demineralized bone matrix (DFDBA/DBM) in sinus lifts varies when used in combination bone graft substitute materials. To test this hypothesis, DFDBA was used together with one of three materials: in saline plus anorganic bone (DFDBA: Bio-Oss; in hyaluronic acid (DFDBA: HY, 32 : 68, w/w; DBX alone; DBX plus Bio-Oss; and D

Calvarial versus iliac crest for autologous bone graft material for a sinus lift procedure: a histomorphometric study.

PURPOSE: The aim of this study was to compare, through histomorphometric analysis, the use of donor autogenous bone graft from calvarial or iliac sources for maxillary sinus lift procedures. MATERIALS AND METHODS: Sixteen patients requiring maxillary sinus augmentation were included in this study. One group of 10 patients was alternatively selected to receive autologous calvarial bone particles, and another group of 6 patients received autologous iliac bone particles. Five months after surgery, bone biopsy specimens were obtained at the time of implant procedure and analyzed through histomorp

Achieving optimal esthetics in a patient with severe trauma: using a multidisciplinary approach and an all-ceramic fixed partial denture.

UNLABELLED: The treatment of traumatic injuries can be difficult, especially when the patient's expectations are too high or unreasonable. Such cases require extremely clear communication with the patient before beginning the case, including an explanation of the final results, based on the patient's goals. Taking over a case from another colleague adds more complications and requires that close attention is paid to the reason for the original dissatisfaction between the former dentist and patient to avoid repeating the same mistakes. Clinically, patients with thin, fragile gingiva are extreme

Implant-supported bar-latch overdenture for the severely atrophied, edentulous jaw: a case report.

The removable implant-supported prosthesis is the treatment of choice in some clinical situations. Despite its technically demanding fabrication and the resultant high cost, the bar-latch overdenture provides stability and is easy for patients to manage. A case report illustrating the surgical and prosthetic procedures of this treatment option, including laboratory fabrication, is presented.

Applications of coronoid process as a bone graft in maxillofacial surgery.

The coronoid process can be easily harvested as a donor bone by an intraoral approach during many maxillofacial surgery procedures. The purpose of this study was to evaluate the utility of autogenous coronoid process bone grafts for maxillofacial reconstructive surgery. Twelve patients, who underwent coronoid process grafts for reconstruction of maxillofacial deformities due to trauma, alveolar atrophy, or temporomandibular joint ankylosis, were included in the study. There were 3 orbital defects after extended maxillectomy, 1 blowout fracture of the orbit, 2 cases of reconstruction after temp

Bone level changes in dental implants with platform-switched design after immediate and delayed placement in the maxilla.

This study sought to evaluate platform-switched implants for immediate placement. A total of 136 implants were placed in 58 patients using either an immediate or delayed implant protocol. Bone level changes of the implants were measured mesially and distally and statistically analyzed with linear mixed models. No implant was lost. There was no significant difference between immediate and delayed implants in approximal bone level changes during the first year (95% confidence interval: -0.10 to -0.01 for immediate versus -0.14 to -0.07 for delayed implants). Subsequent bone resorption was neglig

The bony lid approach in pre-implant and implant surgery: a prospective study.

PURPOSE: The aim of this prospective study is to evaluate the outcome of the bony lid approach in pre-implant and implant surgery. MATERIALS AND METHODS: A total of 200 consecutive patients were treated between 2003 and 2008 with the bony lid technique in a prospective study and followed up for at least 4 years. The bony lid approach consisted in cutting and luxating a bony lid using a microsaw, which was then repositioned at the end of the surgery to avoid large bone defects in the case of osteotomies. In 146 cases the bony lid approach was used for implant explantation, in 29 cases for remo

Histologic comparison of healing after tooth extraction with ridge preservation using mineralized versus demineralized freeze-dried bone allograft.

BACKGROUND: Allografts, such as demineralized freeze-dried bone allograft (DFDBA) and mineralized freeze-dried bone allograft (FDBA) are commonly used by clinicians for ridge preservation procedures. The primary objective of this study is to histologically evaluate and compare the healing of non-molar extraction sockets grafted with DFDBA versus FDBA for ridge preservation. The secondary aim of this study is to compare dimensional changes in ridge height and width after grafting with these two materials. MATERIALS: Forty patients were randomly divided into two groups of 20. Extraction sockets

Clinical and histologic comparison of two different composite grafts for sinus augmentation: a pilot clinical trial.

BACKGROUND AND OBJECTIVES: Sinus augmentation is a procedure used for augmenting insufficient bone height that is often observed in the maxillary posterior areas. Many different techniques as well as bone graft regimens have been suggested for performing this procedure. It was the goal of this study to compare, clinically and histologically, two different composite grafting regimens used for sinus augmentation. MATERIAL AND METHODS: Five patients, needing a bilateral sinus augmentation to allow implant placement, were recruited for this study. Right sinuses were grafted with cortical bone (co

Dental implants and grafting success remain high despite large variations in maxillary sinus mucosal thickening.

BACKGROUND: Although mucosal thickening is the most common radiographic finding observed regarding sinus pathology, the knowledge regarding its clinical significance on the outcomes of dental implants and grafting in the maxillary sinuses is still limited. We hypothesized that mucosal thickening would not alter the predictability for sinus floor augmentation and dental implant placement. The purpose of this retrospective study was to evaluate the outcomes of dental implant placement in sinus-augmented areas with preexisting sinus mucosal thickening. METHODS: This study involved the review of

Free colon transfer for resurfacing large oral cavity defects.

Ideal reconstruction of the oral cavity includes a durable lining that is thin, supple, and innervated, and that provides a lubricated surface that facilitates deglutition and speech. This paper describes the use of free colon transfer for relining the oral cavity. In three patients, segments of transverse colon, split along the antimesenteric border, were transferred as free flaps on the middle colic vessels for large defects involving the alveolar ridge, buccal mucosa, floor of the mouth, tongue, and pharyngeal walls. All flaps were transferred successfully without adverse vascular events, a
