Cyanide poisoning.

In recent years, the increasing use of laetrile has been added to the traditional sources of exposure to cyanide in industry, chemistry labs, and fumigation. The events in Jonestown in 1978 were a grim reminder of the lethality of cyanide. Nonetheless, advancement in new modes of treatment has been slow. The traditional method of treatment used in the United States is effective, but not without its own morbidity and mortality. Using two case reports as models, we review here the topic of cyanide poisoning including sources of exposure, pathophysiology, clinical manifestations of both acute and

Pediatric uveitis: An update.

Because of their varied spectrum of clinical presentation and difficulty in management, pediatric uveitis remains a challenge to the ophthalmologist. Variations in clinical presentation, difficulties in eye examination, extended burden of the inflammation over quality of life, limited treatment modalities, risk of amblyopia are the main challenges in the management of pediatric uveitis. Pediatric uveitis is a cause of significant ocular morbidity and severe vision loss is found in 25-33% of such cases. This article summarizes the common causes of uveitis in children with special approach to th…

Spontaneous dural cavernous fistula in infancy.

A 4-month-old boy presented with the sudden onset of proptosis and dilated episcleral veins. CT revealed an enlarged superior ophthalmic vein and diffusely thickened extraocular muscles. MRI and angiography confirmed a dural middle meningeal--cavernous sinus fistula. Embolization was performed leading to successful resolution of the patient's signs and symptoms. Spontaneous atraumatic arteriovenous cavernous fistulae in infancy are extremely rare, but should be included in the differential diagnosis of infants with orbital congestion. Appropriate treatment leads to prevention of amblyopia, gla…

[Blindness caused by an ischemic optic neuropathy by spontaneous carotid dissection. Report of a case].

In young and middle age subjects, spontaneous carotid dissection is an increasingly recognized cause of ischemic stroke. Their usual presentation is facial pain with a Horner syndrome and a contra lateral paresis. Blindness has been reported as a presenting symptom in only a few cases. We report a 50 years old man who presented with amblyopia in the left eye, without periocular pain. Fundoscopy showed papilledema and a peripapillar hemorrhage, compatible with an ischemic optic neuropathy. A magnetic resonance angiography confirmed a left carotid dissection.

Ophthalmological problems of the premature infant.

Preterm infants are more likely than term infants to have significant abnormalities of all parts of the visual system leading to reduced vision. The most common problem is retinopathy of prematurity (ROP). The frequency and severity of this disorder is inversely related to gestational age. Damage ranges from minor to catastrophic. Preterm infants also have higher rates of amblyopia, strabismus, refractive error, and cortical visual impairment. The later problem is largely associated with neonatal brain injury. Years later, these children may develop glaucoma and retinal detachments.

Clinical suppression in monkeys reared with abnormal binocular visual experience.

To determine if monkeys exhibit clinical suppression in response to early abnormal binocular vision, we compared dichoptic to monocular luminance increment thresholds in monkeys reared with alternating monocular defocus or optically induced strabismus. In the absence of amblyopia, clinical suppression was associated with strabismus and with as little as 1.50 diopters of anisometropia. The severity of suppression was roughly correlated with the magnitude of anisometropia. The demonstration of clinical suppression in monkeys provides a model for future investigations of factors that may influenc…

[Ophthalmologic causes of traffic accidents].

The review deals with association of traffic accidents and visual state of drivers. The study in healthy professional drivers covered influence of visual defect patterns (short sight, lower visual acuity, unilateral amblyopia and single eye) on quality of car driving. The driving quality appeared to be influenced by factor causing lower visual acuity (changes in refraction and opacity of transparent eye structures). Binocular vision plays insignificant role in driving and maneuvering. Lower quality of maneuvering results mainly from diplopia, but not from loss of stereoscopic vision.

Prevalence of paradoxical anisometropia.

A sample population of patients with a certain degree of antimetropia or mixed anisometropia was selected. Binocular interference or paradoxical dominance was expected in some of them. This interference is such that the "seemingly worse eye" (the eye with the lesser visual acuity) dominates the binocular vision and leads the subject to wear corrective glasses or contact lenses even though the level of visual acuity in the other eye is adequate without correction. This is what we called paradoxical anisometropia. There was no amblyopia or strabismus present. Twenty percent of our subjects manif…

The effect of reduced viewing time on colour vision in normal subjects and those with abnormal ocular conditions.

The effect of restricting viewing time to 3.75 ms in normal subjects is to produce a tritan-like colour response on the City University colour vision test. This response pattern is consistent and is repeatable for standard illuminant C at 600 lx. Supplementary tests suggest that the effect is due to a defect of the blue/yellow opponent system rather than a true tritan defect. The same test when repeated on subjects with amblyopia or retinal pathology provides different results which may aid in the differential diagnosis of some visual disorders.

Penalization in strabismus.

Penalization includes all those methods apart from total occlusion which disturb the central vision of one eye by favouring the other eye. Penalization has a sensory action on functional amblyopia; moreover it also has a motor action, influencing the non-accommodative element of spasm in esotropia. Optical penalization is the most classical method. However, one should also be aware of penalization with partial occlusion or fogging, penalization with mydriatics, and penalization with sectors, which are very often used in association with the classical technique.

[Psychosomatic correlations in pediatric ophthalmology].

Somatopsychic disturbances encompassing both ophthalmologic as well as child-psychiatric phenomena mainly occur in ADD: Central disorders of eye motoricity (strabismus) and disorders of visual perception are encountered. Dyslexia or dysorthography often occur as secondary phenomena. The psychosomatic or psychogenic disturbances affect not only the lid mechanism but also visualization. In the lid region, nictation tics and trichotillomania of the eyebrows and eyelashes occur. Psychic impairment of vision may range from a slight amblyopia, often associated with limitation of the field of vision,…

[The late development of striae vitreae of keratoconus (author's transl)].

A young patient with corneal striae vitreae in the left eye, caused by obstetrical trauma was followed up for several years by the authors and treated for amblyopia due to sight astigmatism. When the patient was eight years old a change in the shape of the cornea was observed some months after a recession of the medial rectus and typical features of keratoconus appeared. The authors discussed the pathogenesis of the so-called traumatic keratoconus and point out that late development of striae vitreae to keratoconus is by no means unlikely, and should be taken into consideration for the prognos…

[Several functions of the visual analyzer in chronic alcoholic patients].

The authors examined 615 patients with chronic alcoholism. The study demonstrated an increased number of individuals with myopia, presbyopia, amblyopia, angiomas and angiosclerosis among them as compared to that of the general population. The frequency of optical nerve atrophies in the patients was not high (0.32%). In 205 cases the function of colour sensory and darkness adaptation was determined. The highest frequency of colour sensory was in the group of patients with the withdrawal syndrome. Eventually this function had a tendency towards restitution. The velocity of darkness adaptation wa…

[Factors influencing target refraction in children with pseudophakia after extraction of congenital cataract].

The article describes the factors affecting the target refraction of pseudophakic eyes of children after extraction of congenital cataracts. The factors include features of the echobiometric parameters of the eye, refraction, comorbidity of congenital cataracts and ocular pathologies, margins of error in calculating strength of the intraocular lens, localization and structure of the artificial lens, as well as correction of obscure or refractive amblyopia in pseudophakic eyes. Development of the algorithm for correction of residual refraction of pseudophakic eyes in children both before and af…
