[Condyloma acuminatum in children].

A retrospective study of eleven children affected of condyloma acuminatum is presented. Authors evaluated: age of presentation and consulting, sex, social history, predisposing factors, characteristic and extension of disease, previous venereal diseases, microscopic pathology, treatment and follow up. Age presentation ranged between 1.5 months and 14 years old. Two were male and nine were female. In two cases obvious sexual abuse was demonstrated; close contact without sexual goals in four; in two transmission during labor existed; early sexual activity in one and they didn't evidence mechanis

Papillomavirus antigens in anorectal condyloma and carcinoma in homosexual men.

Squamous cell carcinoma of the cervix and vulva has been associated with infection by human papillomavirus. Recent epidemiologic studies have observed more anal carcinoma among single than married men. To investigate this association between human papillomavirus and squamous proliferative lesions of the anus, we studied eight homosexual or bisexual men with recent diagnoses of anorectal carcinoma and six older patients of unknown sexual orientation with similar diagnoses 20 to 30 years ago. Using an immunohistochemical stain for papillomavirus, we found evidence of its presence in the carcinom

Squamous-cell carcinoma of the anus in condyloma acuminatum. Successful treatment with preoperative chemotherapy and radiation.

A 40-year-old black male had unresectable giant condylomata acuminata of the anorectum with invasive, squamous-cell carcinoma. He was treated with intravenous 5-fluorouracil and mitomycin-C and with extended field radiation to the primary lesion. An abdominoperineal resection was performed, and the surgical specimen contained no residual cancer. Three years after diagnosis, the patient has no recurrent cancer. This case illustrates that otherwise unresectable giant condylomata acuminata with invasive carcinoma may be rendered operable with chemotherapy and radiation.

Treatment of epidermoid anal canal cancer.

Between 1968 and 1979, 183 patients with invasive epidermoid cancer of the anal canal were treated at Institut Curie. There was 156 women, 27 men with a mean age of 67 +/- 11 years (range 40 to 85 years). The initial height of the tumor was less than 4 cm (65 patients), 4 to 6 cm (98 patients), and more than 6 cm (20 patients). All the patients received radiotherapy, either preoperatively or as curative procedure. Twenty-five patients received preoperative radiotherapy, and there was no residual tumor in 9 biopsies obtained at operation. Four local recurrences were observed. One hundred fifty-

Giant and recurrent condyloma acuminatum: appraisal of immunotherapy.

Eighty-four cases of condyloma acuminatum were presented, with conventional treatment, e.g., surgical excision and fulguration with or without Podophyllin application. Recurrence rate was 28 per cent to 65 per cent, depending on the number and duration of treatments. Immunotherapy was used on 17 patients with recurrent and giant condyloma acuminatum. Condylomata were successfully eradicated in 94.1 per cent of patients, which indicates that immunotherapy is a very effective alternative in the treatment of recurrent and giant condyloma acuminatum.

Second primary anal and oropharyngeal cancers in cervical cancer survivors.

BACKGROUND: Human papilloma virus infection is responsible for approximately 31,500 new cancers in the United States annually. Almost all cervical cancers are linked to human papilloma virus infection. As early identification and treatment of cervical cancer improve, the incidence of cervical cancer has decreased and survival has improved. However, survivors continue to remain at risk for other human papilloma virus-related malignancies. The purpose of this study was to assess the risk of primary anal and oropharyngeal cancers among women with a history of squamous cell carcinoma of the cervix

EUS-guided fine needle aspiration and trucut needle biopsy for examination of rectal and perirectal lesions.

OBJECTIVE: Few studies have assessed the diagnostic efficacy of endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) and/or trucut biopsy (TCB) in patients with rectal and perirectal lesions. We aimed to evaluate the diagnostic utility of EUS-FNA and/or TCB in patients with rectal and perirectal lesions. We also assessed their influence on the management and clinical course of these patients. MATERIAL AND METHODS: We performed EUS-FNA and/or TCB in 11 consecutive patients (4 men and 7 women, 33-69 years) with rectal and perirectal lesions for whom conventional diagnostic tools could

Beyond cervical cancer: burden of other HPV-related cancers among men and women.

Human papillomavirus (HPV) infection is a necessary cause of cervical cancer, and is etiologically associated with a subset of cancers of the anus, oropharynx, penis, vagina, and vulva. Current data indicate that HPV infection is potentially associated with 90%-93% of anal cancers, 12%-63% of oropharyngeal cancers, 36%-40% of penile cancers, 40%-64% of vaginal cancers, and 40%-51% of vulvar cancers. HPV infection accounts for up to 492,800 cervical cancers and 97,215 cases of noncervical HPV-related cancers worldwide during 2002, including up to 50,780 cancers among men (13,485 anal cancers, 2

HPV-related anogenital disease and HIV infection: not always 'ordinary' condylomata acuminata.

Human papillomavirus (HPV) is responsible for various diseases in the anogenital region which range from benign condylomata acuminata to anal carcinoma. Buschke-Loewenstein tumour is a clinically 'intermediate' condition which is histologically benign but due to extensive destruction of the local tissues can show malignant behaviour. Its early recognition as a different clinical entity to 'ordinary' condylomata acuminata is important for its adequate management. Immunocompromised persons, such as those with HIV infection, have a higher incidence of HPV-related anogenital disease. Different asp

BRAF mutations distinguish anorectal from cutaneous melanoma at the molecular level.

BACKGROUND & AIMS: Anorectal melanoma (AM) is a rare but highly malignant tumor, displaying histologic and immunohistochemical features very similar to cutaneous melanoma (CM). Because BRAF mutations were recently identified in the majority of CM and nevi, we investigated AM for BRAF mutations and mutations of NRAS , an additional component of the MAPK-signalling pathway. METHODS: DNA from formalin-fixed and paraffin-embedded AM was PCR amplified and sequenced. RESULTS: We detected BRAF mutations in 2 of 19 cases and NRAS mutations in none of the cases. Mutations in exon 15 of BRAF were pres

Interferon alpha-2b injections used as an adjuvant therapy to carbon dioxide laser vaporization of recalcitrant ano-genital condylomata acuminata.

We retrospectively analyzed our treatment of men with anogenital condylomata acuminata (CA) at Hennepin County Medical Center and the University of Minnesota. Between October 1985 and August 1989, 48 patients with CA were treated with laser vaporization, and 27 were treated with laser vaporization followed by a series of intralesional interferon (IFN) alpha-2b injections. The patients in the IFN group were more likely to have perianal CA (44% vs 24%), had larger numbers of CA, and were more likely to have had previous treatment, including previous laser vaporization. The CA in the laser group

Intraepithelial neoplasia in the anal canal. The appearance and relation to genital neoplasia.

Anal canal intraepithelial neoplasia (ACIN) in the form of squamous dysplasia and carcinoma in situ is a rare condition. In this paper we present all 19 cases diagnosed in Denmark in a two-year period. The incidence was 0.2/10(5)/year compared to 0.7/10(5)/year for variants of squamous carcinoma in the anal canal. Most cases of ACIN were incidental findings at minor anal surgery. Histologically, one half of the lesions were located in the anal transitional zone above the dentate line. Two of the 19 patients died from colo-rectal carcinoma and 17 were alive 5 years later. Nine of the patients w

Nonavalent HPV vaccine to prevent recurrent anal or vulvar high-grade squamous intraepithelial lesions (VIVA trial): A randomized, double-blind, placebo-controlled trial.

The nonavalent human papillomavirus (9vHPV) vaccine protects against HPV infection and high-grade squamous intraepithelial lesions (HSIL) when administered prior to exposure, but evidence supporting its potential therapeutic benefit has been inconsistent. This randomized, double-blind, placebo-controlled trial evaluated whether the 9vHPV vaccine reduces recurrence of HSIL or HPV persistence in 27-69-year-old persons previously treated for anal or vulvar HSIL. Participants were HSIL-free at enrollment and received 9vHPV or placebo at months 0, 2, and 6. High-resolution anoscopy or vulvoscopy wa

What cancers explain the growing rural-urban gap in human papillomavirus-associated cancer incidence?

PURPOSE: Human papillomavirus (HPV) can cause cancers of the genital system, anus/rectum, and oropharynx. Prior research showed that HPV-associated cancer incidence was rising faster in nonmetro than in metro populations. Our study identified which cancers contributed to the widening disparity. METHODS: Representing ∼93% of all cancers in the United States, we analyzed data from the North American Association of Central Cancer Registries (2000-2019). Restricting the analysis to HPV-associated cancers, we compared 5-year average age-adjusted incidence rates (per 100,000 population) for nonmetr

Transperineal retropubic approach in total pelvic exenteration for advanced and recurrent colorectal and anal cancer involving the penile base: technique and outcomes.

BACKGROUND: Complete pathological resection of locally advanced and recurrent anorectal cancer is considered the most important determinant of survival outcome. Involvement of the retropubic space with cancer threatening or involving the penile base poses specific challenges due to the potential for margin involvement and blood loss from the dorsal venous plexus. In the present study we evaluate a new transperineal surgical approach to excision of anterior compartment organs involved or threatened by cancer which facilitates exposure and visualisation of the bulbar urethra and the deep vein of

Tumour diameter is a predictor of mesorectal and mesenteric lymph node metastases in anorectal melanoma.

AIM: Anorectal melanoma is a rare neoplasm with poor prognosis. The aim of this study was to investigate what clinicopathological factors predict lymph node metastases and to investigate their association with survival. METHOD: Patients undergoing surgery with curative intent for primary anorectal melanoma in Fudan University Shanghai Cancer Center between 1989 and 2011 were studied retrospectively. The associations between clinicopathological factors and lymph node metastases and prognosis were determined. RESULTS: Forty-three patients underwent a potentially curative resection with a media
