Humeral metastases: an unusual presentation of pancreatic cancer.

In patients presenting with symptomatic bone metastases, the usual primary malignancies are breast, prostate, thyroid, lung and kidney. Pancreatic cancers are an uncommon cause of bone metastasis and, when they are, it is typically to the axial skeleton. We present the case of a 77-year-old woman who presented to our emergency department with right arm pain. Investigation demonstrated a metastatic lesion with impending pathological fracture. Further investigation found a necrotic pancreatic mass in the tail of the pancreas. Histopathology from the humeral metastasis was consistent with metasta

Management of a large post-traumatic skin and bone defect using an Ilizarov frame.

The authors report the case of a 28-year old male who presented with a compound diaphyseal fracture of the tibia, which was treated with intramedullary nailing. Postoperatively he required an extensive fasciotomy for an acute compartment syndrome. The fracture evolved towards post-traumatic osteomyelitis, growing methicillin-resistant Staphylococcus aureus (MRSA), combined with a large overlying soft tissue gap. An Ilizarov frame was used to treat both the bone and the skin defect. The infected fracture was treated by resection and longitudinal bone transport. Meanwhile, the skin was gradually

Engelmann's disease of bone--a systemic disorder?

A patient with Engelmann's disease of bone (progressive diaphyseal dysplasia) also suffered from Raynaud's phenomenon, multiple nail fold infarcts, anaemia, leucopenia, hepatosplenomegaly, and a raised erythrocyte sedimentation rate. Her mother, who also had this inherited bone disease, was known to have Raynaud's phenomenon, necrotising vasculitis, and digital gangrene. Review of other published cases of Engelmann's disease indicates that some of these features are not uncommon. It is argued that Engelmann's disease is not primarily a metabolic bone disease but may be a systemic disorder whic

Intramedullary Nailing for Pilon Nonunions.

Nonunions are uncommon sequelae of tibial pilon fractures, but they can present a challenging problem to the treating surgeon. Treatment strategies can range from isolated bone grafting to revision plating depending on the type of nonunion and patient factors. Here, we present our technique and clinical results for intramedullary nailing of tibial pilon nonunions.

Residual interfragmentary gap after intramedullary nailing of fragility fractures of the humeral diaphysis: short and midterm term results.

The purpose of this retrospective study was to evaluate the short and midterm radiological outcome with regards to bone healing, correlated with humeral shaft fractures treated with anterograde intramedullary nailing, when interfragmentary gap was significant. Here, we critically review our experience of short and midterm term results in over 65 year-old patients. Inclusion criteria in the study were: (1) patients over 65 years old (2) patients with displaced humeral mid shaft fractures (AO/OTA 12 A B C type fractures); (3) patients treated with closed reduction and internal fixation with intr

Elastic nailing of tibia shaft fractures in young children up to 10 years of age.

INTRODUCTION: Although tibia shaft fractures in children usually have satisfactory results after closed reduction and casting, there are several surgical indications, including associated fractures and soft tissue injuries such as open fractures. Titanium elastic nails (TENs) are often used for pediatric tibia fractures, and have the advantage of preserving the open physis. However, complications such as delayed union or nonunion are not uncommon in older children or open fractures. In the present study, we evaluated children up to 10 years of age with closed or open tibial shaft fractures tre

Radius and ulna locking intramedullary nails are safe and reliable after adolescent forearm fractures: a case-controlled study in adolescents.

The aim of this study is to compare the radiological and functional results of newly designed radius and ulna intramedullary nails (IMNs) with those of titanium elastic nails (TENs) for forearm fractures in adolescents requiring surgery. Patients aged between 10 and 17 years who underwent surgery for forearm fractures between January 2011 and January 2018 were retrospectively evaluated. Of the 45 patients who met the study criteria, 18 patients received IMNs and 27 patients received TENs. The clinical results were evaluated according to the Price criteria. The mean age, mean follow-up period,

Behavior of Locked Intramedullary Implants Spanning the Distal Femoral Growth Plate: Results From an Ovine Model.

INTRODUCTION: Safe retrograde femoral nail placement across the distal femoral growth plate could broaden fracture and reconstructive management options for skeletally immature patients, especially in resource-poor settings. Previous data suggested up to 7% of the distal femoral physis can be violated using a retrograde implant without subsequent growth arrest or inhibition. When the intramedullary implant is locked in the metaphysis, the bone will grow with the nail passing from the epiphysis to the metaphysis. However, the consequences of this growth across the physis are not well understood

External fixators and lengthening systems in pediatric upper limb.

The use of external fixators is part of the cultural background of orthopedic surgeons in treating numerous clinical conditions. Over the years, fixator design and biomechanical knowledge have led to different solutions and techniques, and bone lengthening and its better understanding come together with the development of external fixators and the application of the biological principle of distraction osteogenesis. The authors conducted a literature review about using external fixators and lengthening systems in pediatric upper limbs. Despite the applications of external fixators in upper limb

[Fractures of the shaft of the femur].

The femur is the largest, longest and strongest bone in the human skeleton. Fractures of the shaft of the femur can result from high energy as well as low energy trauma and 30% of patients have multiple injuries. In the clinical diagnostic special attention must be paid to the peripheral neurovascular status as well as the possibility of a compartment syndrome. Fractures of the femur shaft are defined according to the AO classification. Treatment is as a rule operative, except for children up the end of 4 years old. Medullary nailing is nowadays the method of choice and the nails can be implan

Vitamin E Phosphate Coating Stimulates Bone Deposition in Implant-related Infections in a Rat Model.

BACKGROUND: Implant-related infections are associated with impaired bone healing and osseointegration. In vitro antiadhesive and antibacterial properties and in vivo antiinflammatory effects protecting against bone loss of various formulations of vitamin E have been demonstrated in animal models. However, to the best of our knowledge, no in vivo studies have demonstrated the synergistic activity of vitamin E in preventing bacterial adhesion to orthopaedic implants, thus supporting the bone-implant integration. QUESTIONS/PURPOSES: The purpose of this study was to test whether a vitamin E phosp

Impact of fractured tibia implant fixation devices on bone stiffness during bending test.

This study focuses on evaluating the failure resistance of a previously reduced tibia with internal fixation implants as PLate (PL) or InterMedullary Nail (IMN), subjected later to a tibial lateral trauma. To replicate this type of trauma, which can be caused by a road accident, a three-point bending test is considered using experimental tests and numerical simulations. The withstand evaluation of the tibia-PL and tibia-IMN structures was conducted by following the load transfer through, the bone and the used implants. The analysis, up to tibia failure, required the use of an elasto-plastic be

Intramedullary nailing of tibial nonunions.

Fifteen tibial nonunions, ranging nine months to 19 years after original injury, were treated with the Brooker intramedullary nail (IMN). The average number of previous failed surgeries was two. The fracture location ranged from several centimeters below the tubercle to 3 cm above the plafond. Twelve of 15 had bone grafting, and all 15 had fibular osteotomy at the time of the IMN procedure. Thirteen of 15 were reamed, and 11 of 15 were nailed open. Patients required an average of five postoperative days in the hospital, and 14 of 15 tibial nonunions healed uneventfully at an average of eight m

Biomechanical analysis of fixation strength at different nailing angles for femoral neck fracture with insufficient reduction.

To analyze the fixation strength of cannulated screws fixation in the treatment of femoral neck fracture with posterior tilt due to insufficient reduction. Two sets of digital models of anatomical reduction and 15° tilting reduction were established by CT data. Each group of models was modeled with two different fixation methods. One fixation method was fixed according to the standard cannulated screws recommended by AO. Another fixation method is to tilt the screw posterior tilt 15°. The final four groups of models were obtained: AO principle nailing posterior tilt model (Group A), posterior

Minimally invasive treatment of forearm double fracture in adult using Acumed forearm intramedullary nail: A case report.

BACKGROUND: Currently, open reduction internal fixation is the conventional surgical method for treatment of double ulna and radius fracture. However, open reduction is associated with a high risk of complications. This case of forearm double fracture involved a patient treated using an Acumed intramedullary nail. The patient experienced good follow-up outcomes. The Acumed forearm intramedullary nail enables early functional exercise and hastens healing of the fracture. Few studies have reported on the use of this approach for the treatment of fractures. CASE SUMMARY: A 23-year-old male patie

Fragility fractures of the ankle in the frail elderly patient: treatment with a long calcaneotalotibial nail.

Conventional methods of treating ankle fractures in the elderly are associated with high rates of complication. We describe the results of treating these injuries in 48 frail elderly patients with a long calcaneotalotibial nail. The mean age of the group was 82 years (61 to 96) and 41 (85%) were women. All were frail, with multiple medical comorbidities and their mean American Society of Anaesthesiologists score was 3 (3 to 4). None could walk independently before their operation. All the fractures were displaced and unstable; the majority (94%, 45 of 48) were low-energy injuries and 40% (19 o
