Central nervous system lesions in liver transplant recipients: prospective assessment of indications for biopsy and implications for management.

BACKGROUND: Precise diagnosis of central nervous system (CNS) lesions in liver transplant recipients remains problematic. Brain biopsies are … tion (43%, 26/60); central pontine myelinolysis, subdural hematoma, acute infarcts, and Aspergillus brain abscesses were the predominant etiologies during this time. All brain abscesses were fungal; 73% (8/11) of these patients concurrently had documented extraneural (pulmonary) infection as a result of the same fungal pathogen. Thus, a diagnostic brain biopsy …

Central nervous system mucormycosis in a patient with hematological malignancy: A case report and review of the literature.

Invasive mucormycosis is a refractory fungal infection. Central nervous system (CNS) mucormycosis is a rare complication caused by infiltration from the paranasal sinuses or hematogenous dissemination. Here, we present a case of a brain abscess, due to mucormycosis, diagnosed using burr craniotomy. A 25-year-old Japanese woman with relapsed-refractory acute lymphoblastic leukemia underwent cord blood transplantation (CBT). The patient experienced prolonged and profound neutropenia, and oral voriconazole was administered as primary antifungal prophylaxis. The patient received a conditioning reg…

[Multiple cerebral abscess caused by Nocardia brasilientis].

Streptococcus aureus, streptococci, and enterobacteria were major causative agents of abscesses of the brain. At present, associated infection is responsible for 60% of cerebral abscesses. When systemic and specific immunities are diminished, a risk for cerebral abscesses caused by fungi increases. Among them, there are the most common fungi Candida species or Aspergillus species. The paper gives an example of successful complex treatment of multiple cerebral abscesses caused by an association of Nocardia brasiliensis and Staphylococcus epidermidis. The specific feature of the observation was

A Unique Case of Empyema Secondary to Amoebic Liver Abscess.

An amoebic liver abscess is an extraintestinal manifestation of amoebiasis that can present with complaints such as right upper quadrant pain and fever. It might not necessarily be associated with abdominal complaints and can have many other atypical presentations. It may present with lung diseases, cardiac diseases, or brain abscesses. We present a case of a patient with empyema secondary to amoebic liver abscess whose diagnosis was delayed due to an unusual presentation. A combination of radiology, serology, and therapeutic interventions led to the accurate management of the patient.

Spontaneous evacuation of cerebellar abscess through the middle ear. A case report.

Otogenic brain abscesses are a significant cause of mortality and morbidity in developing countries. Usually they occur either by distant thrombophlebitis of cerebral veins or by direct extension in an unsafe type of chronic suppurative otitis media. This case of cerebellar abscess was prepared for surgical excision, when sudden evacuation of pus through the left middle ear resolved the abscess and clinical features in the patient. No surgical intervention was required in this patient. The case under discussion is unique and unreported for its spontaneous evacuation through the middle ear.

Multiple liver abscesses caused by S treptococcus intermedius bacteremia in the setting of a routine dental cleaning.

Streptococcus intermedius is a Gram-positive cocci, normally found in the oral cavity and gastrointestinal tract. It has been associated with deep-seated purulent abscesses commonly in the brain or liver in immunocompromised patients. Here, we discuss the case of a 21-year-old immunocompetent patient that presented with septic shock in the setting of multiple pyogenic liver abscesses with positive blood cultures for S. intermedius The patient had a dental cleaning 3 months prior to admission. The abscesses resolved with ultrasound guided drainage and antibiotic therapy.

Transient Ischemic Attack while Injecting Intravenous Saline Contrast in a Patient with Persistent Left Superior Vena Cava.

In this case report, we describe a patient who has a persistent left superior vena cava (PLSVC) manifesting as a transient ischemic attack (TIA) and a remote brain abscess. PLSCV, when draining in the left atrium, is a cause of right to left shunt, and it has been related to brain abscesses and, as reported in a few cases, to ischemic stroke. After admission, our patient experienced 2 new episodes of TIA while being injected with agitated saline contrast through the left arm, producing echocardiographic images of right to left shunt, which gave the clue to the diagnosis.

The Heart-Brain Bypass: Paradoxical Septic Embolism Through a Patent Foramen Ovale (PFO).

Brain abscesses are rare sequelae of paradoxical septic embolism from cardiac sources, particularly in the absence of valvular endocarditis. Diagnosis remains challenging, and a high index of clinical suspicion is often required. We present a 65-year-old female with a past medical history of hypertension and dyslipidemia with a chief complaint of altered mental status. She was found to have left-sided focal seizures and dysarthria. Computed tomography (CT) of the head with and without contrast, with subsequent confirmatory magnetic resonance imaging (MRI) of the head with contrast, revealed a …

[Salmonella enteritidis multifocal infection of the central nervous system. Efficacy of new cephalosporins].

Focal central nervous system infections, as abscess and empyema, due to Salmonella species are unusual. Even more unusual is the association of cerebral abscess, subdural empyema and epidural abscess we report in the present work. This infection appeared in our patient soon after a brain astrocytoma removal and was treated with cefotaxime and gentamicin during three weeks. In spite of documented genus susceptibility to the drugs the infection relapsed in few days. It was definitely cured with ceftazidime alone for six weeks. Third-generation cephalosporins are good alternatives in uncommon inf

Complications of acute frontal sinusitis.

The bone separating the frontal sinus from the anterior cranial fossa and the orbit is often quite thin. This, plus the interrelated venous drainage system of these areas, forms the anatomic basis for serious orbital and intracranial complications that must be recognized early. These include orbital cellulitis and abscess, cavernous sinus thrombosis, subdural and brain abscess and meningitis. Early hospitalization for intravenous antibiotic therapy and, occasionally, emergency surgical drainage are required.
