Vaginal delivery of monoamniotic twins with umbilical cord entanglement. A case report.

BACKGROUND: Monoamniotic twinning is frequently complicated by umbilical cord entanglement and fetal death. Should vaginal delivery take place, this may present as an acute intrapartum emergency. CASE: A 26-year-old woman, gravida 3, para 2, presented in the second stage of labor and gave birth to a macerated, stillborn infant weighing 1,340 g. At the time of delivery it was not known that this was a twin pregnancy. Delivery was achieved only after division of a cord around the neck; it turned out to be that of a live, second twin. This infant, weighing 2,530 g, was delivered by rapid breech

Evisceration as fetal destructive operation: an art revisited.

Fetal destructive operation is a vanishing art today. In an era of increasing cesarean deliveries it has become a historic event. Incidence of destructive operation has varied from various Indian hospitals 0.09-0.28%. Evisceration is one of the rarest of all destructive operations, performed in cases of cephalopelvic disproportion with large fetal abdominal or thoracic tumors and fetal malformations, which are incompatible with life. Less than 50 cases of fetal evisceration have been reported in the literature so far. We are presenting a case of gross fetal abdominal malformation in a multigra

Prototypical autism: New diagnostic criteria and asymmetrical bifurcation model.

The current "autism spectrum" DSM 5 diagnostic criteria and autism standardized diagnostic instruments promote considerable heterogeneity or … omena sharing asymmetrical developmental bifurcations, such as twin pregnancy, left handedness, and breech presentation/delivery. Following this model, nature, trajectory, and positive/negative signs structure of autism would result from the polarized problem of whether or not language and information is processed in a socially biased manner. Third trimester abdominal pregnancy presenting as partial small bowel obstruction in a resource-limited setting: a case report.

BACKGROUND: Abdominal pregnancy is a rare and potentially life-threatening form of ectopic gestation in which the embryo implants within the peritoneal cavity. Diagnosis is often delayed, especially in low-resource settings, owing to limited access to early imaging and nonspecific clinical signs. While gastrointestinal symptoms may occur, intestinal obstruction is an extremely rare presentation of abdominal pregnancy. CASE PRESENTATION: We report the case of an 18-year-old African Black primigravida who presented to our facility at approximately 30 weeks' gestation with decreased fetal moveme

Twin pregnancy: Live fetus with complete hydatidiform molar-A case report.

Gestational trophoblastic disease encompasses a range of lesions, each culminating in varied clinical manifestations. Among these, the simultaneous occurrence of molar changes within the placenta and a viable fetus remains a particularly elusive and challenging presentation, posing intricate dilemmas for obstetricians. In this article, we delineate a distinct case involving a 21-year-old woman with a history of consanguineous marriage. She was referred from Médecins Sans Frontières to the Rafic Hariri University Hospital during her 19th week of gestation. With an unremarkable medical history a

Uterine torsion in a full-term pregnancy presenting as prolonged latent phase and fetal intolerance of labor: A case report.

BACKGROUND: Uterine torsion is defined as torsion of the uterus around its longitudinal axis exceeding 45 degrees. It is a rare obstetric complication. It is a dangerous complication that can lead to placental abruption and intrauterine fetal death. Although rare, early diagnosis is crucial to expedite intervention and optimize outcomes. While the few cases in the current literature have documented acute presentations of uterine torsion, our case is unique in that it had a slower evolution. CASE: A 38-year-old woman, G2P0, was admitted at 37 weeks 0 days of gestation for induction of labor fo

Congenital hip dysplasia: problems in the diagnosis and management in the first year of life.

Eighty-two infants less than 1 year of age who presented with a diagnosis of congenital hip dysplasia have been reviewed. Forty-one infants had a diagnosis of congenital dislocation of the hip (CDH), of which 14 were bilateral cases. Forty-one infants had a diagnosis of acetabular dysplasia, of which none was bilateral. The incidence of breech malposition was higher in the CDH group (34%) than in the acetabular dysplasia group (15%). The metaphyseal edge angle, which accurately discriminates between these two groups, is described. There was evidence of abnormal radiographic development in thos

Association Between Attempted External Cephalic Version and Perinatal Morbidity and Mortality.

OBJECTIVE: To examine whether, with fetal malpresentation at term, perinatal morbidity and mortality differ between women who undergo an external cephalic version (ECV) attempt and those who do not and are expectantly managed. METHODS: We conducted a retrospective cohort study of women with nonanomalous singleton gestations in nonvertex presentation delivering at a tertiary care institution from 2006 to 2016. Women undergoing an ECV attempt at 37 weeks of gestation or greater were compared with those with nonvertex fetuses who did not undergo an ECV attempt and delivered at 37 weeks of gestat

Is time of birth a predictor of adverse perinatal outcome? A hospital-based cross-sectional study in a low-resource setting, Tanzania.

BACKGROUND: Inconsistent evidence of a higher risk of adverse perinatal outcomes during off-hours compared to office hours necessitated a search for clear evidence of an association between time of birth and adverse perinatal outcomes. METHODS: A cross-sectional study conducted at a tertiary referral hospital compared perinatal outcomes across three working shifts over 24 h. A checklist and a questionnaire were used to record parturients' socio-demographic and obstetric characteristics, mode of delivery and perinatal outcomes, including 5th minute Apgar score, and early neonatal mortality. Malignant hypercalcemia in pregnancy: effect of pamidronate on uterine contractions.

BACKGROUND: Hypercalcemia in pregnancy can have devastating effects on both mother and fetus. Pamidronate, a bisphosphonate and inhibitor of bone resorption, has been used to treat hypercalcemia, but it is considered contraindicated in pregnancy. CASE: A 40-year-old multigravida presented at 24 weeks of gestation with metastatic breast cancer. After unsuccessful chemotherapy, at 28 weeks of gestation she had deteriorating renal function, frequent contractions, and a calcium level of 17.6 mg/dL. When conventional treatment of hypercalcemia failed, pamidronate dramatically decreased both the ca

Obstetric management and intraventricular hemorrhage in very-low-birth-weight infants.

A study was conducted to determine the effect of obstetric management on the incidence of intraventricular hemorrhage in the very-low-birth-weight infant. The study covered four years and involved 488 live-born viable infants weighing between 500 and 1500 g, electronically monitored during labor and with echoencephalograms performed within the first three days of life. The overall incidence of intraventricular hemorrhage and mortality was 43 and 21%, respectively. However, both intraventricular hemorrhage and mortality, 72 and 44%, respectively, were increased for gestations less than 1000 g w
