[Vertebral dissecting aneurysm treated with trapping and bilateral posterior inferior cerebellar artery side-to side anastomosis; case report].

A case is reported of ruptured dissecting aneurysm of the intracranial vertebral artery (VA) operated on with VA trapping and bilateral posterior inferior cerebellar artery (PICA) side-to-side anastomosis. A 42-year-old male suddenly developed severe headache and vomiting. On admission, 3 hours later, he was in a state of moderate confusion (Japan Coma Scale 3) and had neck stiffness. Computed tomography (CT) revealed diffuse subarachnoid hemorrhage, especially thick in the posterior fossa with right side dominance. Right vertebral angiography disclosed a fusiform dilatation with proximal narr

Effect of blood flow rate and donor vessel diameter on the patency of carotid venous bypass grafts in dogs.

A saphenous vein graft was implanted from the right subclavian to the right common carotid artery in seven dogs (group I) and between the right and left common carotid arteries in another seven dogs (group II). The recipient artery was ligated proximally to augment blood flow through the graft. Immediately after the anastomoses were completed, the average blood flow through the graft was 32 +/- 25 mL/min in group I and 122 +/- 22 mL/min in group II. At sacrifice 30 days later, angiography showed that all grafts in group I were thrombosed, whereas six (87.5%) of seven grafts in group II were pa

[Extracranial surgery of vertebrobasilar insufficiency. Reconstruction of the vertebral artery in the distal first portion].

Fourteen patients with symptoms of vertebrobasilar insufficiency caused by vertebral artery stenosis in the distal first portion underwent surgical reconstruction. They ranged in age between 42 and 73 years, with a median age of 57 years. Their symptoms included vertigo, dysarthria, syncope, hemiparesis, and homonymous quadrant anopsia. The etiologies of the stenoses involved kinking in 12 cases and mechanical compression due to cervical sympathetic nerve, osteophyte, or fibrous bands in two cases. Digital subtraction angiography revealed that stenosis was maximal at systole and minimal at dia

Histological structural abnormalities of superficial temporal arteries used for extracranial-intracranial anastomosis.

Histological evaluation of the superficial temporal artery resected at the time of extracranial-intracranial anastomosis was performed in 64 consecutive patients. A neuropathologist who was not aware of the medical condition of these patients studied all specimens under light microscopy with hemotoxylin and eosin, Verhoff, and Mallory strains. Intimal proliferation was observed in 62 samples, intimal fibrosis in 56, fragmentation of the internal elastic lamina in 45, splitting of the internal elastic lamina in 41, fragmentation of the media in 38, and fragmentation of the minimal external elas

Double-barrel STA-MCA bypass with endovascular parent artery occlusion for complex anterior circulation aneurysms: indications and outcomes.

To assess the long-term clinical outcomes of a hybrid approach combining double-barrel superficial temporal artery to middle cerebral artery (STA-MCA) bypass with endovascular parent artery occlusion in the treatment of giant complex intracranial aneurysms. This retrospective observational study included patients with giant or fusiform intracranial aneurysms who underwent double-barrel STA-MCA bypass followed by endovascular occlusion of the parent artery between January 2019 and January 2025. The primary outcomes were bypass patency, aneurysm exclusion, and ischaemic complications. Secondary

A novel staged parent artery occlusion following extracranial-intracranial bypass for giant intracranial aneurysms: case series and hemodynamic insights via 4D flow MRI.

OBJECTIVE: For complex intracranial aneurysms (CIAs), extracranial-intracranial (EC-IC) bypass with immediate parent artery (PA) occlusion is an accepted strategy but is associated with a high postoperative infarction rate. This study explores the efficacy of a novel staged therapy, characterized by a first-stage EC-IC bypass followed by second-stage PA occlusion, with a focus on aneurysm change and hemodynamic outcomes. METHODS: Thirty-six patients with CIAs undergoing staged therapy from January 2018 to November 2022 were prospectively recruited. Hemodynamic analysis was performed using 4D

External carotid artery-radial artery graft-posterior cerebral artery bypass for complex vertebrobasilar aneurysms: efficacy and analysis of outcome in a single center.

The purpose of this research was to demonstrate the effectiveness and clinical outcome of an external carotid artery-radial artery graft-posterior cerebral artery (ECA-RAG-PCA) bypass in the treatment of complex vertebrobasilar artery aneurysms (VBANs) in a single-center retrospective study. An ECA-RAG-PCA bypass may be a last and very important option in the treatment of complex VBANs when conventional surgical clipping or endovascular interventions fail to achieve the desired outcome. This study retrospectively analyzed the clinical presentation, case characteristics, aneurysm location, size

Revisiting flow augmentation bypass for cerebrovascular atherosclerotic vaso-occlusive disease: Single-surgeon series and review of the literature.

OBJECTIVE: Despite advances in the nonsurgical management of cerebrovascular atherosclerotic steno-occlusive disease, approximately 15-20% of patients remain at high risk for recurrent ischemia. The benefit of revascularization with flow augmentation bypass has been demonstrated in studies of Moyamoya vasculopathy. Unfortunately, there are mixed results for the use of flow augmentation in atherosclerotic cerebrovascular disease. We conducted a study to examine the efficacy and long term outcomes of superficial temporal artery to middle cerebral artery (STA-MCA) bypass in patients with recurren

Anatomic Variation of the Superficial Temporal Artery and Posterior Auricular Artery in a Pediatric Moyamoya Disease Population.

BACKGROUND AND PURPOSE: In certain cases of pediatric patients with Moyamoya disease undergoing encephaloduroarteriosynangiosis (EDAS) treatment, the posterior auricular artery can be used as an alternative when the parietal branch of the superficial temporal artery is unavailable. In this study, anatomic variations of the superficial temporal and posterior auricular arteries in pediatric patients with Moyamoya disease and postoperative outcomes of posterior auricular artery-EDAS are explored. MATERIALS AND METHODS: Medical records of 572 patients with Moyamoya disease who underwent surgical

Persistent Local Vasogenic Edema with Dynamic Change in the Regional Cerebral Blood Flow after STA-MCA Bypass for Adult Moyamoya Disease.

We report an adult moyamoya disease (MMD) patient who developed persistent local vasogenic edema with dynamic change in the regional cerebral blood flow after left superficial temporal artery-middle cerebral artery (STA-MCA) anastomosis. A 49-year-old woman with ischemic-onset MMD underwent left STA-MCA anastomosis. Magnetic resonance (MR) imaging of fluid-attenuated inversion recovery 1 day after surgery revealed an asymptomatic local high-signal-intensity lesion at the site of anastomosis, and MR angiography demonstrated apparently patent STA-MCA bypass. Due to the increased apparent diffusi

Stent for temporary endovascular bypass and thrombectomy in major ischemic stroke.

Endovascular techniques for acute stroke have evolved from a pharmacological to a mechanical approach. We report illustrative cases of successful anterior circulation recanalization in patients with large arterial occlusions, using a stent-based technique to perform arterial recanalization and thrombectomy, without permanent stent implantation. Four patients (mean age 59 years), presented with National Institutes of Health Stroke Scale (NIHSS) scores of 18 to 24, from 2 hours to 6 hours after stroke onset, with middle cerebral artery (MCA), MCA branch, internal carotid artery (ICA) terminus, o

Bypass combined with embolization via a venous graft in a patient with a giant aneurysm in the posterior communicating artery and bilateral idiopathic occlusion of the internal carotid artery in the neck.

The authors describe the case of a patient with a symptomatic giant aneurysm of the posterior communicating artery (PCoA) associated with bilateral idiopathic occlusion of the internal carotid artery (ICA). The presence of severe tortuosity of the vertebral arteries (VAs), both at their origin from the subclavian artery and at the level of the third segment, impeded navigation of the catheter for embolization of the aneurysm with Guglielmi detachable coils (GDCs). A direct surgical approach was considered to be a high-risk procedure because of the bilateral occlusion of the ICAs and the size o

Steroid-responsive chorea in moyamoya disease.

We report the case of a 13-year-old boy with disabling chorea due to moyamoya disease. His chorea seemed to improve with steroid therapy. We conclude that steroid therapy may ameliorate moyamoya-associated chorea, and perioperative steroids can confound neurosurgical outcome. We are unable to assess the effect of cerebrovascular bypass procedures on the outcome of chorea in this patient.

[Extra- and intracranial Doppler ultrasound. Evaluation of patients with surgical anastomosis of the arteria temporalis superficialis superficialis and arteria cerebri media].

During a mean postoperative period of 16 months, 51 patients who had received an extracranial-intracranial (EC-IC) arterial bypass were examined by means of transcranial and extracranial Doppler ultrasound, with the following results: the flow velocities of the middle cerebral artery (MCA) are postoperatively significantly lower than in normal control subjects. No differences were seen if the MCA was supplied via the EC-IC bypass or via any "natural" collateral vessels. In 32 cases, blood supply of the MCA artery was provided via the EC-IC bypass. In another 13 cases the MCA was supplied via t

Extracranial-intracranial bypass to the proximal posterior cerebral artery and to the proximal middle cerebral artery for multiple occlusive cerebrovascular disease.

A 48-year-old man with a multiple occlusive cerebrovascular disease involving both the carotid and the vertebrobasilar systems is reported. Faced with noneffective conservative treatment, a surgical approach was carried out. As the patient had poorly developed posterior communicating arteries, two extracranial-intracranial bypasses were performed for both the carotid and the vertebrobasilar circulations; extracranial-intracranial bypasses with interposed saphenous vein grafts between the left external carotid artery and the proximal segment of the left middle cerebral artery, and between the r

[Cerebral blood flow dynamics of spontaneous occlusion of the circle of Willis (Part 1)--comparison between electrophysiological findings and neuroradiological findings].

Four cases (2 children and 2 adults) of spontaneous occlusion of the circle of Willis were studied using EEG, somatosensory evoked potential (SEP), cerebral angiography and CT scan. The following results were obtained. 1) Among the above described parameters, SEP was the most useful to detect the ischemic change in the thalamus when the obstruction of the posterior part of the circle of Willis extends, even before a low density lesion of the thalamus appears on CT. 2) In child cases, a decrease in the amplitude N3 of SEP was observed after hyperventilation. It was speculated that this finding
