A different perspective on cervical X-ray parameters in nTOS: could it play a role in pathophysiology?

INTRODUCTION: Neurogenic Thoracic Outlet Syndrome (nTOS) is characterized by compression of neural structures within the interscalene triangle of the thoracic outlet, with anatomical variations frequently implicated in its etiology. In patients with TOS, spinal nerves pass through a narrow anatomical space as they exit the foramen and traverse the thoracic inlet. The tension of the suprapleural bands extending between the C7 transverse process and the first rib, along with changes in neck position and certain anatomical variations, can lead to compression of the lower trunk. The condition of t

Cervical Rib Synostosis to the First Rib: A Rare Anatomic Variation.

BACKGROUND: Congenital anatomic variations exist in human anatomy, which create both diagnostic and treatment challenges. Understanding the osteologic and radiographic anatomy of supernumerary ribs arising from the cervical spine and recognizing the morphologic variations thereof is of great importance to clinicians, radiologists, and surgeons alike. CASE DESCRIPTION: This case study describes osteologic morphology and radiologic characteristics of a rare anatomic variant of a cervical rib (CR): a unilateral, right-sided CR synostosis to the first thoracic rib of a 50-year-old South African m

Computerized Tomography Analysis of Young Children for Prevention of Aspiration Injuries.

OBJECTIVE: The Small Parts Test Fixture (SPTF) (16 CFR 1501) was developed from cadavers of young children and foreign body (FB) data. Recent FB studies reveal that the SPTF misses outliers. Computerized tomography (CT) provides detailed dimensional data for young children. Our null hypothesis is that the SPTF (31.75 mm) is smaller than relevant portions of the aerodigestive tract. METHODS: A 3-year retrospective review (2011-2014) of head/neck CT data for infants and children (N = 106) aged 6 months to 6 years was completed. Six measurements (mm) were recorded: 1) maxillary incisors to poste

[Measurement of parapharyngeal space using CT images].

Parapharyngeal space can be defined as a potential space surrounded by deglutitional and masticator muscles and their covering, superficial and middle layer of deep cervical fascia. It is easily recognized in images provided by modern scanning devices, such as CT and MRI. Parapharyngeal space has traditionally been divided by styloid process and fascia of tensor veli palatini muscle (nasopharyngeal level) or fascia of stylopharyngeus muscle (oropharyngeal level) into two compartments, prestyloid and postsyloid spaces. The latter is often called as carotid space. Prestyloid portion exclusively

A paracondylar process and early onset cervical osteoarthritis in a California Amerind adult.

Burial 65 is a 25-39-year-old male from a hunter-gatherer California Amerind collection with a large bone spur called a paracondylar process. The process measures 11.92mm in width and 13.32mm in length. It is located on the left anteromedial region near the foramen magnum and articulates with a pseudarthrosis to the atlas. The atlas, too, has a pseudarthrosis1 on the left transverse process. The paracondylar process may be the result of caudal occipitalization, which is a congenital variant that results from a lack of fusion of the occipital bone to the foramen magnum. Trauma is another possib

Direct Hemi-Hypoglossal Nerve and Hypoglossal Nerve for Suprascapular Nerve/Proximal Brachial Plexus Neurotization: A Cadaveric Feasibility Study.

Introduction Partial restoration of shoulder function is important in upper brachial plexus lesions, and the suprascapular nerve is often the target for such neurotization procedures. Although there is an extensive history of peripheral nerve surgeons using the hypoglossal nerve for various local nerve transfers, some have reported using this nerve as a donor for upper brachial plexus grafting procedures. We discuss our anatomical findings for the use of a direct hypoglossal to suprascapular nerve transfer. Materials and methods Fifteen adult cadavers (30 separate sides) were dissected to reve

Utility of intraoperative neuromonitoring and outcomes of neurological complication in lower cervical and upper thoracic posterior-based three-column osteotomies for cervical deformity.

OBJECTIVE: For severe and rigid adult cervical deformity, posterior-based three-column osteotomies (3COs) are warranted, but neurological complications are relatively high with such procedures. The performance measures of intraoperative neuromonitoring (IONM) during cervicothoracic 3CO have yet to be studied, and there remains a paucity of literature regarding the topic. Therefore, the authors of this study examined the performance of IONM in predicting new neurological weakness following lower cervical and upper thoracic 3CO. In addition, they report the 6-month, 1-year, and 2-year outcomes o

The Great Auricular Nerve: Anatomical Study with Application to Nerve Grafting Procedures.

BACKGROUND: When it comes to autogenous nerve grafting, the sural and great auricular nerve (GAN) are the 2 nerves predominately used for trigeminal and facial nerve repair. Arising from the second and third cervical ventral rami, the GAN emerges from the posterior border of the sternocleidomastoid coursing superiorly and anteriorly toward the ear. METHODS: Eleven sides from 5 Caucasian and 1 Asian cadaveric heads (all fresh-frozen) were used. One man and 5 women were used with an age at death ranging from 57 to 91 years, with a mean of 80.3 years. Measurements were made from the inferior bor

The role of clivus length and cranial base flexion angle in basilar invagination and Chiari malformation pathophysiology.

BACKGROUND: The craniovertebral junction is an anatomically well-defined transitional zone located between the skull and the cervical spine. Multiple malformations can affect this region with the most prominent being basilar invagination (BI) and Chiari malformation (CM). Despite numerous studies, the origin, pathophysiology, and classification of these pathologies remain controversial. The objective of this study was to evaluate the implication of cranial base flexion angle and clivus length in the development of these conditions. METHODS: Midline tomography and magnetic resonance imaging of
