Like lumbago, pains and stiffness of the shoulder are a crux medicorum. Here, however, unlike lumbago, promising treatment is possible if a correct diagnosis made. The cardial symptoms are classified for the diagnosis; conservative and surgical treatment of the individual disease pictures are described.

INTRODUCTION: Fibrous connections between the suboccipital muscles and the spinal dura mater (DM), known as myodural bridges (MBs), have been extensively described in adults, but their embryological development and morphogenesis remain incompletely understood. This study examines the development of these structures, along with the posterior atlanto-occipital and atlanto-axial membranes (AOM and AAM), in human fetuses, to clarify their origin and potential functional relevance.
METHODS: This study examined the histological sections of 40 human embryos and fetuses from 8 to 14 weeks of developm

BACKGROUND: For immediate facial nerve reconstruction during head and neck tumor resection, a great auricular nerve (GAN) graft can be potentially harvested in the same surgical field during tumor resection. However, it is often avoided because a GAN graft is only approximately 5 cm long without any branches, and a sural nerve graft is recommended for larger defects. We investigated the length of the GAN that can be harvested in 18 patients, along with the evaluation of postoperative facial nerve palsy.
METHODS: We retrospectively analyzed 18 cases of immediate facial nerve reconstruction usi

The continuously growing mouse incisor provides a fascinating model for studying stem cell regulation and organ renewal. In the incisor, epithelial and mesenchymal stem cells assure lifelong tooth growth. The epithelial stem cells reside in a niche known as the cervical loop. Mesenchymal stem cells are located in the nearby apical neurovascular bundle and in the neural plexus. So far, little is known about extracellular cues that are controlling incisor stem cell renewal and guidance. The extracellular matrix protein tenascin-W, also known as tenascin-N (TNN), is expressed in the mesenchyme of

BACKGROUND: Infusion testing is a common procedure to determine whether shunting will be beneficial in patients with normal pressure hydrocephalus. The method has a well-developed theoretical foundation and corresponding mathematical models that describe the CSF circulation from the choroid plexus to the arachnoid granulations. Here, we investigate to what extent the proposed glymphatic or paravascular pathway (or similar pathways) modifies the results of the traditional mathematical models.
METHODS: We used a compartment model to estimate pressure in the subarachnoid space and the paravascul

INTRODUCTION: Posterior C1-C2 fusion is a highly successful treatment for atlantoaxial instability and other pathologies of the cervical spine, with fusion rates approaching 95%-100%. However, poor visualization of the lateral masses of C1 secondary to the course of the C2 nerve root along with blood loss from the venous plexus and compression of the C2 nerve from lateral mass screws are technical obstacles that can arise during surgery. Thus, sacrifice of the C2 nerve root has long since been debated in fusions involving the C1 and C2 vertebral bodies.
METHODS: Cadaveric dissections on four

This article presents the history of the discovery of compression of the median nerve in the carpal tunnel without an identifiable cause as a distinct clinical entity. By analyzing primary sources, we show that, at the beginning of the twentieth century, physicians described patients with paresthesias and numbness in the hands, most prominent at night, accompanied by bilateral symmetrical atrophy along the radial side of thenar eminence. At the time, the 2 most influential hypotheses regarding etiology were, first, compression of the lower trunk of the brachial plexus by a cervical or first ri

The coexistence of molluscan cardioexcitatory neuropeptide (FMRFAMIDE) and luteinizing hormone-releasing hormone (LHRH) was studied in the nervous system of the big brown bat, Eptesicus fuscus, with immunocytochemistry. Within mammals, this is the first report of the coexistence of these neuropeptides in the terminal nerve. In juvenile and adult bats, both neuropeptides are distributed identically throughout the terminal nerve (tn), and they coexist in many parts of the prosencephalon from the olfactory bulb as far caudally as the interpeduncular nucleus. Peripherally, on the basal surface of

OBJECTIVE: Cervical spinal cord injury (SCI) and lower trunk brachial plexus injury (BPI) commonly result in hand paralysis. Although restoring hand function is complex and challenging to achieve, regaining volitional hand control drastically enhances functionality for these patients. The authors aimed to systematically review the outcomes of hand-opening function after supinator to posterior interosseous nerve (PIN) transfer.
METHODS: A systematic literature review was performed according to the PRISMA guidelines.
RESULTS: A total of 16 studies with 88 patients and 119 supinator to PIN tran

INTRODUCTION AND IMPORTANCE: Thoracic outlet syndrome (TOS) includes disorders caused by compression of the neurovascular structures in the upper thoracic outlet (Roos and Owens, 1996 [1]; Bürger, 2014; Curuk, 2020 [3]). Depending on the compressed structure, it is categorized into neurological, arterial and venous TOS. SAPHO syndrome (synovitis-acne-pustulosis-hyperostosis-osteitis syndrome) is a rare chronic inflammatory disease of unknown etiology. With its typical involvement of sternoclavicular joint and clavicle, complication due to hyperostosis in this region, leading to thrombosis of t

Sexual medicine experts and sexologists must spread certainties on the biological basis of the female orgasm to all women, not hypotheses or personal opinions. Therefore, they must use scientific anatomical terminology. The anatomy of the clitoris and the female orgasm are described in textbooks, but some researchers have proposed a new anatomical terminology for the sexual response in women. The internal/inner clitoris does not exist: the entire clitoris is an external organ. The clitoris is not composed of two arcs but of the glans, body, and crura or roots. "Clitoral bulbs" is an incorrect

Thoracic outlet syndrome (TOS) is a nonspecific label. When employing it, one should define the type of TOS as arterial TOS, venous TOS, or neurogenic TOS. Each type has different symptoms and physical findings by which the three types can easily be identified. Neurogenic TOS (NTOS) is by far the most common, comprising well over 90% of all TOS patients. Arterial TOS is the least common accounting for no more than 1%. Many patients are erroneously diagnosed as "vascular" TOS, a nonspecific misnomer, whereas they really have NTOS. The Adson Test of noting a radial pulse deficit in provocative p

Phrenic nerve transfer has been widely used in treating brachial plexus avulsion injury. However, the present method crosses the thoracic part of the phrenic nerve, and nerve graft is needed, resulting in a long period of regeneration and partly irreversible muscle atrophy. We present our early experience of using video-assisted thoracic surgery to harvest a full length of phrenic nerve for transfer. Fifteen patients (mean age, 28 years) were treated. The thoracic part of the phrenic nerve was freed by means of video-assisted thoracic surgery and taken out of the thoracic cavity, and a full-le

OBJECTIVE: The objective of this study was to identify electrodiagnostic and anatomic distinctions between true neurogenic thoracic outlet syndrome and median sternotomy-related brachial plexopathy, in reference to the pattern of abnormality of the medial antebrachial cutaneous sensory nerve conduction study (NCS) response.
BACKGROUND: Neurogenic thoracic outlet syndrome and sternotomy-related brachial plexopathy are both lower trunk brachial plexopathies, but their clinical and electrodiagnostic presentations are distinct. The anatomic differences distinguishing these disorders from each oth

PURPOSE OF THE STUDY: Combined fractures of the atlas and epistropheus account for 3 % of all acute injuries to the cervical spine. In relation to all C1 and C2 injuries this is 43 % and 16 %, respectively. The aim of this study is to evaluate a group of patients with combined C1-C2 fractures and to suggest an effective therapeutic procedure.
MATERIAL: In the years 1996 to 2003, a total of 16 patients with trauma to the atlantoaxial complex were treated at the Orthopedic Department of the Third Faculty of Medicine, Charles University, Prague (1996-2001) and the Department of Spinal Surgery of

In this study a new method of reinnervation for unilateral recurrent laryngeal nerve paralysis was performed in canines, producing physiologic vocal fold motion in each of a small series of animals. During the procedure the left anterior division of the recurrent laryngeal nerve was reinnervated with axons from the thyroarytenoid branch of the contralateral recurrent laryngeal nerve. The posterior branch of the left recurrent laryngeal nerve was divided and sutured to the ansa cervicalis to maintain tone in the posterior cricoarytenoid muscle. In all four animals, the right distal vocalis stum

BACKGROUND: We reported phonatory recovery in the majority of 88 patients after recurrent laryngeal nerve (RLN) reconstruction. Here we analyzed factors that might influence the recovery, in a larger patient series.
METHODS: At Kuma Hospital, 449 patients (354 females and 95 males) underwent RLN reconstruction with direct anastomosis, ansa cervicalis-to-RLN anastomosis, free nerve grafting, or vagus-to-RLN anastomosis; 47.4 % had vocal cord paralysis (VCP) preoperatively. Maximum phonation time (MPT) and mean airflow rate during phonation (MFR) were measured 1 year post surgery. Forty patient

BACKGROUND: Recently, the supraclavicular artery island flap has gained popularity as a regional flap for head and neck reconstruction. During clinical follow-up, some patients report referred sensation to the shoulder when there is contact with the flap skin island surface. The authors examine the anatomical origin/characteristics of the supraclavicular nerves (C3-4) to this flap and its relationship to the flap pedicle and anatomical boundaries.
METHODS: SAI flap harvest and nerve dissection was performed in seven fresh frozen cadavers (n = 10) using loupe magnification in order to further

BACKGROUND AND OBJECTIVES: Cervical spinal cord injury (SCI) and lower trunk brachial plexus injury (BPI) commonly result in hand paralysis. The supinator to posterior interosseous nerve (SPIN) transfer is a well-established surgical approach that can achieve restoration of hand opening with similar outcomes in SCI and BPI. Most often, the radial nerve has 2 branches to the supinator. Studies have reported utilization of either single or double supinator branches as donors for the SPIN transfer. The aim of this study was to study whether using one or both supinator branches can affect recovery
