Pathology of iridectomy specimens in gyrate atrophy of the retina and choroid.

Gyrate atrophy of the retina and choroid is an autosomal recessive disease characterized by progressive retinal degeneration and ornithine aminotransferase deficiency. We report here the new histological findings and ultrastructural changes in 3 iridectomy specimens from 2 Finnish patients with gyrate atrophy. The iridectomy specimens were removed during routine cataract extraction and studied with a transmission electron microscope. The dilator muscle showed atrophy, abnormal mitochondria, and tubular aggregate type structures similar to those found in skeletal muscle. Degenerative changes su…

Rheumatoid nodule formation within the choroid plexus. Report of a second case.

A woman with active systemic rheumatoid disease had striking neuropsychiatric manifestations; a rheumatoid nodule within the choroid plexus was the sole neuropathological finding. Although we found only one other report of findings of this type in rheumatoid disease, we suspect that this phenomenon may not be as rare as originally believed, and that it may represent another mechanism by which behavioral changes develop in this disorder.

Ornithine ketoacid transaminase deficiency in gyrate atrophy of the choroid and retina.

Gyrate atrophy of the choroid and retina is a chorioretinal degeneration associated with hyperornithinemia with an autosomal recessive mode of inheritance. Cultured skin fibroblasts from five affected patients showed a virtual absence of ornithine ketoacid transaminase (OKT) (L-ornithine:2-oxoacid aminotransferase E.C.2.6.1.13) activity. Fibroblasts from four carrier parents showed a 42%-65% reduction in OKT activity. Increasing the concentration of pyridoxal phosphate (vitamin B6 in the assay media resulted in partial restoration of OKT activity in fibroblasts from one out of five patients st…

Ocular and biochemical abnormalities in gyrate atrophy of the choroid and retina.

Patients with gyrate atrophy of the choroid and retina have myopia, constricted visual fields, elevated dark adaptation thresholds, small or nondetectable ERGs, and chorioretinal atrophy. Biochemical abnormalities include hyperornithinemia, hypolysinemia, hyperornithinuria, an unknown amino compound in the urine, and virtual absence of OKT activity in extracts of cultured skin fibroblasts. Extracts of cultured skin fibroblasts from one patient studied in our laboratory showed an increase in OKT activity with increasing concentrations of vitamin B6 in the assay medium; this patient also showed …

Hyperornithinemia and gyrate atrophy of choroid and retina. Report of a case.

A case of hyperornithinemia and gyrate atrophy of choroid and retina has been observed in a 3-year and 9-month-old girl. She presented also mild mental retardation, delayed language development and speech defects. The restriction of protein intake to a minimum of 0.8 g/kg/day induced a significant reduction of plasma ornithine levels. In some of the previous reports of the syndrome, a deficient ornithine-ketoacid transaminase activity has been found in cultured fibroblasts.

Gyrate atrophy of the choroid and retina: clinical, ophthalmologic, and biochemical considerations.

A case of gyrate atrophy of the choroid and retina associated with hyperornithinemia has been subjected to extensive clinical and biochemical investigation. The familial occurrence of the ocular disease and of abnormality of amino acid was unique to this 28-year-old male, being absent in parents and siblings. He presented with progressive visual loss, and was found to have cataracts and large areas of peripheral lacunar atrophy. Clinically there was no other abnormality. However, he was hyperuricemic and has an abnormal EEG. Despite otherwise normal biochemical indices of hepatic, renal, and m…

In vitro measurement of respiration of choroid plexus cells in communicating hydrocephalus.

Communicating hydrocephalus occurs as a result of inappropriate CSF production in the circumstance of diminished absorption. Ventricular enlargement does not progress as rapidly as the measured normal production of CSF would suggest. Thus, compensatory mechanisms must exist to respond in some manner to the altered pathophysiology. In this experiment the metabolic activity (O2 consumption) of the choroid plexus cells in animals with communicating hydrocephalus was compared with that of normals. The modified Cartesian diver technique of Zeuthen13 was employed. These preliminary measurements show…

Quantitative assessment of subfoveal choroidal thickness, retinal thickness, and choroidal vascularity index after upper eyelid blepharoplasty.

PURPOSE: To evaluate the quantitative changes in subfoveal choroidal thickness (CT), retinal thickness, and choroidal vascularity index (CVI) following upper eyelid blepharoplasty in patients with dermatochalasis, using spectral-domain optical coherence tomography (SD-OCT). METHODS: This retrospective study included 78 eyes of 78 patients (mean age: 57.9 ± 8.7 years) who underwent upper eyelid blepharoplasty between May 2024 and May 2025. Only right eyes with high-quality SD-OCT images obtained both preoperatively and at 3 months postoperatively were included. Retinal thickness was assessed i…

Central Macular Thickness, Subfoveal Choroidal Thickness, and Choroidal Vascularity Index After Phacoemulsification: Bromfenac versus Ketorolac.

PURPOSE: To evaluate the exploratory effects of topical bromfenac 0.1% and ketorolac 0.45% on the posterior segment following cataract surgery, focusing on the choroidal vascularity index (CVI) as a sensitive biomarker, and to explore the potential impact of different dosing regimens. METHODS: This retrospective cohort study included 105 eyes (47 in the ketorolac group and 58 in the bromfenac group) that underwent uncomplicated phacoemulsification. Subgroups were stratified by dosing duration: Pre-only (no postoperative NSAID) and Pre+Post (one-month postoperative maintenance). Central macula…

AI-Driven Optical Coherence Tomography Biomarkers for Choroidal Neovascularization Assessment in Punctate Inner Choroidopathy and Multifocal Choroiditis.

PURPOSE: To assess potential optical coherence tomography (OCT) biomarkers associated with the presence and future development of secondary choroidal neovascularization (CNV) in inflammatory lesions using artificial intelligence (AI)-based segmentation in patients with punctate inner choroidopathy (PIC) and multifocal choroiditis (MFC). DESIGN: Multicenter, retrospective cross-sectional and cohort study. SUBJECTS: The study included 208 eyes from 156 patients, comprising 112 patients with PIC (143 eyes) and 44 patients with MFC (65 eyes). METHODS: Sequential OCT scans from patients with MFC…

Assessing the Impact of Simulated Color Vision Deficiency on Ophthalmologists' Ability to Differentiate between Choroidal Melanoma and Choroidal Nevus.

Background: Color vision deficiency (CVD) is an often-overlooked issue within the medical community, and its consequences remain insufficiently explored. We aim to evaluate how CVD affects diagnostic accuracy and distinguish between malignant choroidal melanoma and benign choroidal nevus among ophthalmologists. Methods: In this cross-sectional study, we engaged ophthalmologists through a web-based survey distributed via the professional ophthalmology society's social media channels. The survey encompassed a series of three fundus images representing normal fundus, choroidal nevus, and choroida…
