Barriers to clinical implementation for novel therapies and biomarker testing in the community practice oncology setting: A CLDN18.2 case study.

PURPOSE: Community-based practices often experience extended adoption times when implementing novel targeted agents, due to practical barriers. Delayed implementation of new therapies can potentially increase patient burden. This study aimed to objectively identify and quantify practice challenges to the implementation of new therapies in the community setting. METHODS: In this pilot study, challenges associated with zolbetuximab, a potential first-in-class claudin 18.2 (CLDN18.2) monoclonal antibody and companion diagnostic, was assessed. Between April and August 2024, 53 data-collection eve

High-fructose diet-induced gut dysbiosis as a mechanistic driver of chronic inflammation and accelerated biological aging: a comprehensive review.

High-fructose diet (HFD) has emerged as a critical dietary risk factor for metabolic syndrome, non-alcoholic fatty liver disease (NAFLD), and cardiovascular disease. This review delineates the molecular mechanisms underlying the pathological cascade linking high-fructose intake, gut dysbiosis, chronic inflammation, and accelerated aging. At the metabolic level, excess fructose is rapidly phosphorylated by ketohexokinase (KHK), depleting intracellular ATP while driving overproduction of uric acid and reactive oxygen species (ROS). These events activate the NLRP3 inflammasome and NF-κB signaling

Periodontitis Exacerbates Insulin Resistance via Gut Barrier Dysfunction and TLR4/NF-κB Activation.

INTRODUCTION AND AIMS: Periodontitis and type 2 diabetes mellitus (T2DM) are interrelated, potentially linked through intestinal barrier dysfunction. Yet, whether their comorbidity is associated with worsened barrier integrity and how this relates to insulin resistance remain unclear. This study examined associations between periodontitis, gut barrier disruption, Toll-like receptor 4 (TLR4)/nuclear factor kappa-B (NF-κB) activation, and metabolic parameters in a T2DM mouse model. METHODS: Twenty-four C57BL/6 mice were randomised into 4 groups (n = 6): healthy control, chronic periodontitis, T

Sodium butyrate alleviates high ambient temperature-induced oxidative stress, intestinal structural disruption, and barrier integrity for growth and production in growing layer chickens.

BACKGROUND: This study was conducted to evaluate the effects of dietary sodium butyrate (SB) supplementation on the antioxidant status, intestinal morphology, functional damage, and barrier integrity of heat-stressed Hy-Line Sonia (HYS) layer chicks. A total of 240 female HYS at 35 days of age with average body weights (415 ± 35 g) were divided into 6 groups with 10 replicates/group and 4 chickens per replicate. A 2 × 3 factorial design study was performed, including two conditions of ambient temperature (25 °C and 35 °C) and three dietary levels of SB (0, 0.5, and 1.0 SB g/kg diet). RESULTS:

Yuzhi Zhixue Granules regulate the JAK/STAT pathway to affect T-cell differentiation in the colon of ulcerative colitis.

PURPOSE: Yuzhi Zhixue (YZZX) granules, a classic Chinese herbal remedy, have demonstrated efficacy in managing hemorrhagic and inflammatory conditions, and are clinically used to treat uterine bleeding. Building upon their documented clinical applications and phytochemical composition, we hypothesized that YZZX may also possess therapeutic potential against ulcerative colitis (UC). This research aimed to examine the immunomodulatory effects of YZZX granules on Th1/Th17-mediated immune reactions in a murine UC model induced by dextran sulfate sodium (DSS). METHODS: The major small-molecule con

Cenicriviroc prevents dysregulation of astrocyte/endothelial cross talk induced by ischemia and HIV-1 via inhibiting the NLRP3 inflammasome and pyroptosis.

Blood-brain barrier (BBB) breakdown is one of the pathophysiological characteristics of ischemic stroke, which may contribute to the progression of brain tissue damage and subsequent neurological impairment. Human immunodeficiency virus (HIV)-infected individuals are at greater risk for ischemic stroke due to diminished immune function and HIV-associated vasculopathy. Studies have shown that astrocytes are involved in maintaining BBB integrity and facilitating HIV-1 infection in the brain. The present study investigated whether targeting astrocyte-endothelial cell signaling with cenicriviroc (

Toxoplasma gondii Dissemination in the Brain Is Facilitated by Infiltrating Peripheral Immune Cells.

Despite recent advances in our understanding of pathogenic access to the central nervous system (CNS), the mechanisms by which intracellular pathogens disseminate within the dense cellular network of neural tissue remain poorly understood. To address this issue, longitudinal analysis of Toxoplasma gondii dissemination in the brain was conducted using 2-photon imaging through a cranial window in living mice that transgenically express enhanced green fluorescent protein (eGFP)-claudin-5. Extracellular T. gondii parasites were observed migrating slowly (1.37 ± 1.28 μm/min) and with low displaceme

Effects of Lipotoxicity in Brain Microvascular Endothelial Cells During Sirt3 Deficiency-Potential Role in Comorbid Alzheimer's Disease.

Silence information regulator 3 (SIRT3) is an NAD+ dependent deacetylase enzyme that enhances the function of key mitochondrial proteins. We have earlier demonstrated that deletion of Sirt3 gene leads to downregulation of metabolic enzymes, mitochondrial dysfunction and neuroinflammation in the brain, the major causes of Alzheimer's disease (AD). We also reported recently that Sirt3 gene deletion in Alzheimer's transgenic mice leads to exacerbation of neuroinflammation, amyloid plaque deposition and microglial activation. AD often coexists with other brain lesions caused by comorbidities which

Effects and interaction of dietary electrolyte balance and citric acid on the intestinal function of weaned piglets.

Fifty-six piglets (6.26 ± 0.64 kg BW) were weaned at 21 d and randomly assigned to one of the eight dietary treatments with seven replicate pens for a 14-d experimental period. The eight experimental diets were prepared via a 2 × 4 factorial arrangement with citric acid (CA; 0% and 0.3%) and dietary electrolyte balance (dEB, Na + K - Cl mEq/kg of the diet; -50, 100, 250, and 400 mEq/kg). Varying dEB values were obtained by altering the contents of calcium chloride and sodium bicarbonate. An interaction (P < 0.05) between dEB and CA in diarrhea score and the number of goblet cell in jejunum wer

Primary Appendicular Malignancy Presenting as Abdominal Wall Abscess with Secondary Tuberculosis Infection: an Interesting Case Report.

Gastric cancer remains a major cause of cancer-related mortality worldwide. Systemic chemotherapy is the primary treatment strategy for unresectable or metastatic gastric cancer, but long-term survival remains poor. Zolbetuximab, a monoclonal antibody targeting claudin-18 isoform 2 (CLDN18.2), has demonstrated efficacy in combination with chemotherapy for CLDN18.2-positive gastric and gastroesophageal junction (GEJ) adenocarcinomas. In select cases, systemic therapy can lead to a significant reduction in tumor burden, enabling conversion surgery, which has been associated with favorable outcom

Blood-brain barrier disruption and microglia-associated synaptic pruning contribute to cognitive decline after electroconvulsive therapy.

BACKGROUND: Modified electroconvulsive therapy (MECT) is a highly effective intervention for major depressive disorder (MDD), yet its broader clinical application is constrained by poorly understood cognitive side effects. We hypothesized that MECT disrupts hippocampal blood-brain barrier (BBB) integrity, permitting extravasation of peripheral factors that activate microglia, drive synaptic phagocytosis, and precipitate cognitive decline. METHODS: We employed a multi-modal translational approach integrating: (1) clinical neuroimaging (rs-fMRI and DCE-MRI) and cognitive assessments in 41 MDD p

Alginate Oligosaccharides Enhance Gut Microbiota and Intestinal Barrier Function, Alleviating Host Damage Induced by Deoxynivalenol in Mice.

BACKGROUND: Alginate oligosaccharides (AOS) exhibits notable effects in terms of anti-inflammatory, antibacterial, and antioxidant properties. Deoxynivalenol (DON) has the potential to trigger intestinal inflammation by upregulating pro-inflammatory cytokines and apoptosis, thereby compromising the integrity of the intestinal barrier function and perturbing the balance of the gut microbiota. OBJECTIVES: We assessed the impact of AOS on mitigating DON-induced intestinal damage and systemic inflammation in mice. METHODS: After a 1-wk acclimatization period, the mice were divided into 4 groups.

A meta-analysis of the effects of early life stress on the prefrontal cortex transcriptome suggests long-term effects on myelin.

BACKGROUND: Early life stress (ELS) refers to exposure to negative childhood experiences, such as neglect, disaster, and physical, mental, or emotional abuse. ELS can permanently alter the brain, leading to cognitive impairment, increased sensitivity to future stressors, and mental health risks. The prefrontal cortex (PFC) is a key brain region implicated in the effects of ELS. METHODS: To better understand the effects of ELS on the PFC, we ran a meta-analysis of publicly available transcriptional profiling datasets. We identified five datasets (GSE89692, GSE116416, GSE14720, GSE153043, GSE12
