Coccidioidomycosis in transplant recipients: a primer for clinicians in nonendemic areas.

PURPOSE OF REVIEW: Coccidioidomycosis is a fungal infection caused by Coccidioides spp., a fungus endemic to the southwestern USA. Immunocompromised patients with coccidioidomycosis often present with atypical manifestations. We provide clinical, diagnostic, and management approaches for clinicians in nonendemic areas who might encounter coccidioidomycosis in transplant recipients. RECENT FINDINGS: With preemptive screening, heightened awareness, and targeted prophylaxis, the incidence of coccidioidomycosis in transplant recipients has decreased to 1-3% in endemic areas. Dissemination and mor…

High levels of interleukin-10 impair resistance to pulmonary coccidioidomycosis in mice in part through control of nitric oxide synthase 2 expression.

We have shown previously that there is a direct correlation between IL-10 levels and susceptibility to Coccidioides immitis peritonitis in C57BL/6 (B6), DBA/2, and BXD recombinant inbred mice. We now show that B6 mice are also more susceptible to C. immitis pneumonia and that interleukin-10 (IL-10)-deficient (IL-10-/-) B6 mice are more resistant to C. immitis pneumonia. In addition, we established that high levels of IL-10 are sufficient to make genetically resistant mice susceptible to both C. immitis peritonitis and pneumonia by infecting h.IL-10 transgenic mice. Infected h.IL-10 transgenic …

[Mycoses of the head and neck].

OBJECTIVES: The ever growing number of immunocompromized patients and increasing travel to areas where fungal diseases are endemic explain why in recent years mycoses have emerged as important infections in clinical practice. It is essential that pathologists and clinicians be knowledgeable about them. RESULTS: Head and neck infections are common in disseminated mycoses. The most frequent causative yeasts or yeast-like organisms include Candida albicans, Cryptococcus neoformans, Histoplasma capsulatum var capsulatum, Blastomyces dermatitidis, Paracoccidioides brasiliensis and Coccidioides imm…

Treatment of coccidioidomycosis with SCH 39304.

A new oral triazole antifungal, SCH 39304, was administered to 54 patients with progressive infections due to Coccidioides immitis from six collaborating centers. Patients were grouped according to site of infection including chronic pulmonary (25), bone/joint (17) and skin/soft tissue (12). The median age was 40 years; 83% were male, 52% white, 13% HIV-infected and 35% had failed previous therapy. The majority of patients were treated with either 100 mg or 200 mg day-1. One patient on renal dialysis received 300 mg day-1. Baseline abnormalities were reassessed for evidence of efficacy every 4…

Pulmonary fungal infections in the age of biologics and climate change.

PURPOSE OF REVIEW: This review examines the evolving epidemiology of invasive fungal infections, with emphasis in pulmonary presentations, in the context of climate change and the expanding immunomodulatory therapy use. RECENT FINDINGS: Fungal infections represent a growing global health threat, with epidemiological patterns increasingly extending beyond traditional immunocompromised populations. Climate-driven thermal adaptation and geographic range expansion of endemic fungi, particularly Coccidioides and Histoplasma species, are exposing immunologically naive populations to infection. The …

Simultaneous Laryngeal and Mastoid Coccidioidomycosis: A Case Report and Literature Review.

A 72-year-old Caucasian woman from the Southwestern United States presented with chronic right ear pain, hearing loss, and progressive hoarseness. Prior treatments, including balloon sinuplasty and tympanostomy tube placement, failed to resolve her symptoms. Imaging revealed right mastoid opacification, while flexible laryngoscopy demonstrated erythematous and polypoid changes of the right larynx. Pathology from biopsies of both the middle ear and larynx revealed spherules consistent with Coccidioides infection. She was treated with systemic fluconazole and underwent tympanomastoidectomy with …

Primary Cutaneous Coccidioidomycosis Presenting on the Leg of an Immunocompetent Patient: A Case Report.

Coccidioidomycosis is a fungal infection that primarily manifests as an asymptomatic condition caused by inhalation of fungal spores. Less commonly, patients can develop dissemination to extrapulmonary locations such as the skin or primary cutaneous inoculation of the fungus at a site of trauma. These cutaneous complications are primarily found among immunosuppressed individuals. Here, we present a patient with no relevant past medical history that, after living in an area to which Coccidioides is endemic, developed a slowly growing, pruritic plaque on his right thigh with well-demarcated area…

Mapping the environmental risk of coccidioidomycosis in Sonora, Mexico: a Bayesian spatiotemporal perspective (2010-2022).

Coccidioidomycosis, a fungal disease caused by Coccidioides spp. is endemic in arid regions of the Americas. Although northern Mexico shows increasing incidence, its environmental determinants remain understudied. This research examines the association between coccidioidomycosis and environmental factors in Sonora, Mexico, from 2010 to 2022. We integrated municipal-level hospital discharge data with mean annual temperature, total annual precipitation, wind speed and direction, soil moisture, vegetation index and land use (human settlements and agriculture). Using a Bayesian hierarchical spatio…

Evaluation of nikkomycin Z with frequent oral administration in an experimental model of central nervous system coccidioidomycosis.

UNLABELLED: We evaluated the in vivo activity of nikkomycin Z against central nervous system coccidioidomycosis. Mice were inoculated intracranially with arthroconidia of Coccidoides immitis, and treatment with nikkomycin Z (50, 100, or 300 mg/kg orally TID) or fluconazole (25 mg/kg orally BID) began 2 days later. Each dose of nikkomycin Z and fluconazole significantly improved survival and reduced brain fungal burden compared with vehicle control. Further studies of nikkomycin Z against coccidioidomycosis are warranted. IMPORTANCE: Coccidioides species are endemic fungi that are capable of c…

Δcps1 vaccine protects dogs against experimentally induced coccidioidomycosis.

Coccidioidomycosis is a significant health problem of dogs and humans in endemic regions, especially California and Arizona in the U.S. Both species would greatly benefit from a vaccine to prevent this disease. A live avirulent vaccine candidate, Δcps1, was tested for tolerability and efficacy to prevent pulmonary coccidioidomycosis in a canine challenge model. Vaccine injection-site reactions were transient and there were no systemic effects observed. Six of seven vaccine sites tested and all draining lymph nodes were sterile post-vaccination. Following infection with Coccidioides posadasii, …

A preliminary study of the plasma concentrations of orally administered fluconazole in alpacas (Vicugna pacos).

Alpacas residing in the region endemic for Coccidioides spp. are susceptible to serious, disseminated coccidioidomycosis that may result in death. There is currently no oral antifungal dose recommendation for this species. We used a steady-state study design to assess the pharmacokinetics of oral generic fluconazole in alpacas dosed q 24 h for 14 days. Cohorts of 2-3 animals received fluconazole from 6 to 15 mg/kg/day, and pharmacokinetic analysis was performed after each group of animals in order to make dose adjustments for the next group. The final three animals were used as confirmation of…

Interleukin-8 Receptor 2 (IL-8R2)-Deficient Mice Are More Resistant to Pulmonary Coccidioidomycosis than Control Mice.

The pathology of human coccidioidomycosis is granulomatous inflammation with many neutrophils surrounding ruptured spherules, but the chemotactic pathways that draw neutrophils into the infected tissues are not known. We previously showed that formalin-killed spherules (FKS) stimulate mouse macrophages to secret macrophage inflammatory protein 2 (MIP-2), which suggested that CXC ELR+ chemokines might be involved in neutrophil recruitment in vivo To test that hypothesis, we intranasally infected interleukin-8R2 (IL-8R2) (Cxcr2)-deficient mice on a BALB/c background with Coccidioides immitis RS.…

Opportunistic Invasive Mycoses in AIDS: Cryptococcosis, Histoplasmosis, Coccidiodomycosis, and Talaromycosis.

PURPOSE OF REVIEW: The goal of this review is to provide an update on the epidemiology, diagnosis, and treatment of opportunistic fungal infections in patients with human immunodeficiency virus (HIV) infection including Cryptococcus spp., Histoplasma spp., Coccidioides spp., and Talaromyces marneffei, formerly Penicillium marneffei. RECENT FINDINGS: In many settings, despite increasing roll out of antiretroviral therapy (ART), opportunistic invasive mycoses produce a substantial burden of disease. The prevalence of specific fungal pathogens depends on their endemicity. Viral suppression achie…

Rapidly progressive quadriparesis heralding disseminated coccidioidomycosis in an immunocompetent patient.

Coccidioides species are dimorphic fungi endemic to southwestern USA and northern Mexico. Disseminated coccidioidomycosis is rare with an estimated incidence of 1% in affected individuals and usually presents as meningitis when the central nervous system is involved. Spinal involvement with coccidioidomycosis, though not uncommon, predominantly manifests as osseous involvement leading to osteomyelitis and epidural abscess formation. Progressive quadriparesis as a presenting symptom secondary to intramedullary spinal cord coccidioidomycosis is very unusual and to our knowledge has not been desc…

Pulmonary fungal infections in HIV-infected persons.

Fungal infections account for a large number of AIDS-index diagnoses and complicate the course of most patients with HIV disease. Infection … more common in the American heartland, it has overlapped areas endemic for Histoplasma capsulatum, Coccidioides immitis, and Blastomycosis dermatitidis. Disease from these deep-seated fungal pathogens, whether from de novo exposure or reactivation, has protean manifestations. Fungal infections in AIDS. Histoplasmosis and coccidioidomycosis.

Histoplasma capsulatum and Coccidioides immitis are two fungi that are regional in occurrence and cause opportunistic fungal infections in patients with AIDS. Many cases of histoplasmosis have been reported in patients months or years after they have been in an endemic area. These are obviously cases of reactivation of latent infections. With coccidioidomycosis, the cases have been reported from endemic areas, but some also appear to be reactivation infections, and we should anticipate such cases in nonendemic areas just as with histoplasmosis. The clinical presentations may be atypical, even …

Therapy of disseminated or pulmonary coccidioidomycosis with ketoconazole.

Oral therapy with ketoconazole for active disseminated or progressive pulmonary coccidioidomycosis was evaluated according to defined criteria of objective improvement for 39 patients, most of whom had received other antifungal chemotherapy. Minimal inhibitory concentrations of ketoconazole for isolates of Coccidioides immitis were below mean peak serum concentrations. Eighteen patients responded at all sites of disease, one patient failed to respond, and the others either are being evaluated or cannot be evaluated. Most patients who responded to therapy required more than three months of trea…

Coccidioidomycosis in compromised hosts. Experience at Stanford University Hospital.

To determine the frequency and clinical characteristics of infection with Coccidioides immitis in immunosuppressed patients at Stanford University Hospital, clinical records of 14 years were examined. Thirteen cases met the diagnostic criteria. Half had Hodgkin's disease. In six the infection was disseminated; five of the six died early in the course of their infectious illness, frequently without diagnosis. Conclusions include: 1. The occurrence of coccidioidomycosis in immunosuppressed patients seen at institutions in or adjacent to the endemic area is not as rare as the literature suggests.…

The spectrum and presentation of disseminated coccidioidomycosis.

PURPOSE: Extrapulmonary dissemination of Coccidioides species is associated with significant morbidity and mortality. The clinical manifestations vary widely according to the host, the severity of illness, and location of dissemination. The morbidity and mortality can be reduced by early recognition and treatment, which in turn depends on understanding the spectrum and presentation of disease. METHODS: We performed a retrospective analysis of 150 cases with extrapulmonary nonmeningeal disease seen from 1996 to 2007 at a referral medical center in an endemic region. RESULTS: Hematogenous diss…
