Evaluation of tip fold over in 1,575 cochlear implant electrode arrays.

OBJECTIVE: Cochlear implant (CI) surgery aims to achieve atraumatic electrode array insertion to preserve residual hearing and optimize outcomes. Tip fold-over, a mechanical complication during insertion, can compromise stimulation effectiveness and may require reinsertion or revision surgery. This study investigates the incidence, influencing factors, and diagnostic approaches for tip fold-over across a large patient cohort implanted with various CI electrode array designs. METHODS: We conducted a retrospective analysis of adult patients who underwent CI surgery between 2013 and 2024 at the

The neural processing mode and development of emotional prosody in children with cochlear implants.

BACKGROUND: Prelingually deaf children with cochlear implants (CIs) receive a degraded representation of pitch and spectral cues that are critical for decoding emotional prosody. The early cortical processing modes and developmental trajectories of vocal emotion in CI children remain unclear. METHODS: Nineteen prelingually bilaterally deaf children with unilateral cochlear implants were followed longitudinally at three post-activation windows. At each visit, children were passively presented with pseudo-sentences spoken with angry, happy, fearful, or neutral prosody while cortical activity wa

The Pediatric Cochlear Implant Use Questionnaire: Assessing Habits and Barriers to Use.

OBJECTIVE: The objective was to design a questionnaire called the Pediatric Cochlear Implant Use Questionnaire (PCIUQ) to identify daily habits of pediatric cochlear implant (CI) use and the barriers to its daily application. This study also aimed to administer the PCIUQ to caregivers of pediatric CI users to determine its association with pediatric CI wear time. We hypothesized that recipients whose caregivers reported a greater number of barriers to daily CI use would show lower daily CI use. STUDY DESIGN: Questionnaire development study consisting of 2 parts. Part 1: Pragmatic item develop

Simulation of unilateral and bilateral cochlear implants on spatial speech-in-noise tasks.

The current study simulated bilateral and unilateral cochlear implant (CI) processing using a channel vocoder with dense tonal carriers ("SPIRAL") in 13 normal-hearing listeners. Their performance of recognizing spatial speech-in-noise was measured under the effects of three masker locations (0°, +90°, and -90°; target at 0°) and three types of maskers (steady-state noise, speech-modulated noise, and a single-talker interferer) where the maskers contained different levels of energetic and informational masking. The stimuli were spatialized using the head-related impulse responses recorded from

Machine-Learning Predictions of Cochlear Implant Functional Outcomes: A Systematic Review.

OBJECTIVES: Cochlear implant (CI) user functional outcomes are challenging to predict because of the variability in individual anatomy, neural health, CI device characteristics, and linguistic and listening experience. Machine learning (ML) techniques are uniquely poised for this predictive challenge because they can analyze nonlinear interactions using large amounts of multidimensional data. The objective of this article is to systematically review the literature regarding ML models that predict functional CI outcomes, defined as sound perception and production. We analyze the potential stren

Estimating Cochlear Implant Users' Sound Localization Abilities With Two Loudspeakers.

The ability to tell where sound sources are in space is ecologically important for spatial awareness and communication in multisource environments. While hearing aids and cochlear implants (CIs) can support spatial hearing for some users, this ability is not routinely assessed clinically. The present study compared sound source localization for a 200-ms speech-shaped noise presented using real sources at 18° intervals from -54° to +54° azimuth and virtual sources that were simulated using amplitude panning with sources at -54° and +54°. Participants were 34 adult CI or electric-acoustic stimul

Equitable access to cochlear implants: a perspective on social justice and international obligations.

Hearing loss is one of the most prevalent sensory conditions worldwide, affecting over 1.5 billion people and is one of the main causes of disability. For individuals with severe to profound hearing loss, cochlear implants (CIs) are one of the most effective rehabilitation options as they can significantly improve speech comprehension, language development, quality of life and social inclusion. Despite their clinical value, however, access to CIs remains highly unequal between and within countries, with implantation rates disproportionately favoring wealthier nations and people with a higher s

In Vivo Imaging of Cochlear Implant Electrode Orientation Relative to Scalar Tilt.

OBJECTIVE: Visualization of cochlear implant (CI) electrode contact position offers insights into electrode-cochlea biomechanics. Prior in vivo imaging studies have assessed electrode distances to a fixed modiolar axis, but electrodes' orientation within the cochlear canal and their relationship to the scala tympani's dynamic anatomical tilt have not been investigated. This study employed in vivo flat-panel computed tomography (FPCT) imaging to examine the flexibility and dynamic behavior of lateral wall arrays. METHODS: This retrospective cohort study included eight MED-EL FLEX28 CI users. F

A Model for Evaluating Electrode Impedance Stabilization in Pediatric Cochlear Implant Recipients.

OBJECTIVES: Electrode impedance (EI) is a key biomarker in cochlear implantation, reflecting tissue reactions and influencing stimulation efficiency. This study investigates a patient-specific model for determining impedance stabilization onset. METHODS: In this study, impedance stabilization was defined as a less than 10% difference in impedance values between two consecutive fitting sessions conducted at least 1 month apart. A time-to-event model was used to identify the stabilization onset along electrode regions and overall impedance. A comparison of stabilization times was also performed…

Long-Term Stability of Electrical Stimulation in Children with Bilateral Cochlear Implants.

The long-term stability of neural responses to cochlear implant (CI) stimulation and programmed stimulation levels remains unclear. Although smaller cohort studies suggest stabilization within months postimplant, reprogramming still consumes significant clinical time. The aim of this study was to investigate the resilience of the auditory nerve to prolonged stimulation from CIs and identify changes in the clinically provided stimulation levels over time. Stimulation parameters (n = 14,072 MAPs), electrophysiological auditory nerve thresholds (n = 23,215), and slopes of amplitude growth functio

Long-Term Cochlear Implant Sensitivity in Patients With Far Advanced Otosclerosis.

OBJECTIVE: Cochlear implantation in otosclerosis patients presents unique surgical and programming challenges. The success and satisfaction of these patients heavily depend on precise speech processor fitting. Increased bone growth and tissue mass associated with otosclerosis can hinder electrical current flow, adding resistance to stimuli transmission to ganglion cells. This study aimed to evaluate mid- and long-term cochlear implant fitting in otosclerosis patients compared to a control group. METHODS: This was a monocentric retrospective case-control study conducted at a tertiary referral …

Peripheral Neural Synchrony in Postlingually Deafened Adult Cochlear Implant Users.

OBJECTIVES: This paper reports a noninvasive method for quantifying neural synchrony in the cochlear nerve (i.e., peripheral neural synchrony) in cochlear implant (CI) users, which allows for evaluating this physiological phenomenon in human CI users for the first time in the literature. In addition, this study assessed how peripheral neural synchrony was correlated with temporal resolution acuity and speech perception outcomes measured in quiet and in noise in postlingually deafened adult CI users. It tested the hypothesis that peripheral neural synchrony was an important factor for temporal

Underlying dimensions of real-time word recognition in cochlear implant users.

Word recognition is a gateway to language, linking sound to meaning. Prior work has characterized its cognitive mechanisms as a form of competition between similar-sounding words. However, it has not identified dimensions along which this competition varies across people. We sought to identify these dimensions in a population of cochlear implant users with heterogenous backgrounds and audiological profiles, and in a lifespan sample of people without hearing loss. Our study characterizes the process of lexical competition using the Visual World Paradigm. A principal component analysis reveals t

Resting-State Functional Connectivity Predicts Cochlear-Implant Speech Outcomes.

OBJECTIVES: Cochlear implants (CIs) have revolutionized hearing restoration for individuals with severe or profound hearing loss. However, a substantial and unexplained variability persists in CI outcomes, even when considering subject-specific factors such as age and the duration of deafness. In a pioneering study, we use resting-state functional near-infrared spectroscopy to predict speech-understanding outcomes before and after CI implantation. Our hypothesis centers on resting-state functional connectivity (FC) reflecting brain plasticity post-hearing loss and implantation, specifically ta
