Risk of HIV transmission within marriage in rural China: implications for HIV prevention at the family level.

BACKGROUND: Little is known about HIV transmission at the family level in China. GOAL: : We examined the risks for HIV transmission between husbands and wives and from parents to children in a rural area where HIV spread among former commercial blood donors. STUDY: A cross-sectional study was conducted among 605 (302 couples) marriage license applicants. RESULTS: More males (64.6%) than females (52.1%) reported having had premarital sex and multiple sex partners (12.6% and 6.9%, respectively). Among those having had multiple sex partners, 8.5% reported often or always using condoms. Only 36…

Family planning in traditional markets in Nigeria.

Operations research on the Ibadan Market-Based Distribution Project in Nigeria investigated the feasibility of a contraceptive distribution system using traders in the traditional markets to sell pills, condoms, and foaming tablets. Two hundred and thirty-five female and male traders were trained and supplied with contraceptives, malaria treatments, and oral rehydration salts to sell at low prices in 39 markets. This article presents findings from qualitative and quantitative research conducted in 1985-89 to determine if the sale of contraceptives in the marketplace is acceptable to participat…

Experiences and Perceptions of Black Men Who Have Sex with Men About Acquiring HIV: A Qualitative Narrative Perspective.

In the United States, Black men who have sex with men (MSM), between the ages of 18 and 34 years, have the highest rates of new HIV infections. The prevalence of HIV in this population is three to four times higher than their White MSM counterparts. Twelve Black MSM from the Bay Area, nine with HIV and three without HIV, were interviewed regarding their experiences and perceived risks of acquiring HIV. Narrative analysis revealed these themes: (a) tested regularly for HIV, (b) HIV knowledge varied before arriving in San Francisco, (c) condom use typically nonexistent when under the influence o

Women and HIV in the Twenty-First Century: How Can We Reach the UN 2030 Goal?

Women have always been part of the HIV/AIDS epidemic. As with other populations affected by HIV, for many years the only prevention strategy available was behavior change. Behavioral interventions for women were developed and evaluated, with some success. Because women did not control the use of male condoms, efficacious interventions needed to build skills for partner negotiation. Woman-controlled technologies such as the female condom and vaginal spermicide were unable to solve the problem of male control of the condom and enable the development of safe methods for women to protect themselve…

HIV knowledge and sexual risk behavior among pregnant couples in South Africa: the PartnerPlus project.

In sub-Saharan Africa, 60 % of people living with HIV are women and most are of childbearing age. Alarmingly, seroconversion rates during pregnancy are high and increase as pregnancy progresses, highlighting the importance of increasing HIV-knowledge among pregnant women and their partners. This study compared sexual risk behavior, HIV knowledge and condom use pre- to post-partum among South African couples (n = 239 couples) randomly assigned to an intervention or an enhanced standard of care with the PMTCT protocol at rural community health antenatal clinics. Consistent condom use and HIV-rel

HIV infection and related risk behaviours among female sex workers in greater Cairo, Egypt.

Although illegal in Egypt, prostitution exists. The prevalence of HIV infection among female sex workers (FSWs) in Cairo is not precisely known. This cross-sectional study investigated the high-risk behaviour for HIV infection and HIV prevalence among FSWs in greater Cairo. A total of 431 FSWs were interviewed about their sexual history with paid and unpaid partners, condom use and risky behaviour for HIV infection; all were tested for HIV. Use of alcohol and drugs was reported by 39.9% and 49.0% of the women respectively; 37.6% only used such substances while with a client. Male condoms were …

Changes in sexual behavior during a safety and feasibility trial of a microbicide/diaphragm combination: an integrated qualitative and quantitative analysis.

If proven effective, vaginal microbicides and diaphragms will likely be part of a larger HIV prevention model that includes condoms and other prevention strategies. It is, therefore, important to understand how introducing new prevention methods may affect overall patterns of sexual risk behavior. Data presented were collected as part of a safety and feasibility study of ACIDFORM gel with a diaphragm among 120 women in South Africa. Interviews were administered at enrollment and months 1, 3, 5, and 6 of the trial. Focus groups were conducted at trial exit. Frequency of sex increased significan…

Clinical evaluation of the easy-flow catheter.

A clinical evaluation of the Easy-Flow External Condom Catheter (Delphi Medical Products) was performed on incontinent patients of the Salt Lake City VA Medical Center. All 20 subjects were experienced users of condom catheters prior to entering the study. Use of the experimental device reduced frequency of change for 10 patients, increased daytime dryness for 13 patients, and improved nighttime dryness for 10 patients. All except one of the 20 subjects found the Easy-Flow Catheter easier to apply and all rated it easiest to remove. Overall, 13 of the 20 patients indicated that they preferred

Attitudes and practices of hemophilia care providers involved in HIV risk-reduction counseling.

Hemophilia physicians, nurses, and social workers attending a national conference were asked to complete a questionnaire assessing their attitudes and practices regarding HIV risk-reduction counseling. All of the 150 respondents reported recommending the use of condoms to their clients, but only two-thirds felt comfortable demonstrating a condom, while fewer could explain condom choices or how to make safe sex more pleasurable. Less than half questioned their clients about history of STDs, sexual practices, or level of sexual satisfaction. Those who devoted 50 percent or more time to HIV risk-…

STD knowledge and behaviours among clients of female sex workers in Bali, Indonesia.

This study investigated knowledge, beliefs, and practices related to STDs and AIDS among clients of low price sex workers in Bali, Indonesia. These men are at high risk of STD and HIV transmission. They have poor knowledge of the basic concepts of STD and HIV transmission and prevention, and they practice a variety of ineffective prevention strategies including partner selection and the prophylactic use of antibiotics. They report a mean of 1.9 paid sexual partners in the previous week and very low frequencies of condom use. Over 25% had experienced an STD symptom in the previous 6 months, wit

HIV Prevention Patterns Among a Population-Based Sample of HIV Negative Young Women in Eswatini.

Adolescent girls and young women (AGYW; aged 15-24 years) in Sub-Saharan Africa have shouldered a disproportionate share of the global HIV burden for decades and currently account for 78% of incident AGYW HIV cases worldwide. Condoms remain the predominate tool for HIV protection, but antiretroviral pre-exposure prophylaxis (PrEP) is increasingly available in Southern Africa. The DYnamics of Contraception in Eswatini (DYCE) study conducts bi-weekly phone surveys about sexual and reproductive health with a nationally representative panel of AGYW. We employ sequence analysis to classify longitud…

HIV risk perceptions and post-exposure prophylaxis among men who have sex with men in five Brazilian cities.

In the current context of the HIV epidemic, multiple prevention strategies including biomedical interventions have been presented as alternatives for vulnerable groups. This study investigated homosexuals' and bisexuals' perceptions of the risk of HIV infection and their experiences of using HIV post-exposure prophylaxis (PEP). We conducted a qualitative study with 25 men who have sex with men (MSM) in five Brazilian cities using semi-structured interviews. The results showed that the use of condoms was the main HIV prevention strategy employed by the respondents. In addition, condom failure, …

Homelessness and unstable housing associated with an increased risk of HIV and STI transmission among street-involved youth.

The role that environmental factors play in driving HIV and STI transmission risk among street-involved youth has not been well examined. We examined factors associated with number of sex partners using quasi-Poisson regression and consistent condom use using logistic regression among participants enroled in the At Risk Youth Study (ARYS). Among 529 participants, 253 (47.8%) reported multiple partners while only 127 (24.0%) reported consistent condom use in the past 6 months. Homelessness was inversely associated with consistent condom use (adjusted odds ratio [aOR]=0.47, p=0.008), while unsta

Contraceptive use in women with bacterial vaginosis.

The aim of the study was to investigate if bacterial vaginosis (BV) is associated with use of specific contraceptives. Women at family planning and youth clinics (n = 956), among whom 131 had BV, were subjects for structured in-depth interviews including current and previous contraceptive use. Variables measuring sexual risk-taking were ascertained. Current users of contraceptives were compared with non-users. Both oral contraceptive (OC) and condom use showed a significant protective effect against BV, adjusted for possible confounders (odds ratios were 0.4 and 0.3, respectively). Intrauterin

Urban minority youth: alcohol and marijuana use and exposure to unprotected intercourse.

This article focuses on the use of alcohol and marijuana in the context of sexual relationships and the impact of these substances on the consistency of condom use by urban, minority youth. Regression models are presented to test for the effects of alcohol and marijuana use with sexual partners while controlling for the effects of respondents' sexual history. The results indicated that when alcohol or marijuana was used with a partner, the consistency of condom use decreased even when other variables related to sexual history were controlled for. Alcohol use had a greater negative effect on co
