Maternal psychological distress on delayed onset of lactation II: A meta-analysis.

BACKGROUND: Existing studies suggest a potential association between maternal psychological distress and delayed onset of lactation II (DOLII), but the evidence remains inconclusive. METHODS: A systematic literature search was conducted in PubMed, Web of Science, Embase, Cumulative Index to Nursing and Allied Health Literature, Cochrane Library, China National Knowledge Infrastructure, WANFANG, and VIP databases, from database inception to August 30, 2024. Two independent reviewers screened articles, extracted data, and assessed study quality. Discrepancies were resolved through discussion. M

Seeking mental health support for feelings of perinatal depression and/or anxiety during the COVID-19 pandemic: A qualitative descriptive study of decision-making.

BACKGROUND: Rates of perinatal depression and anxiety increased during the COVID-19 pandemic. It remains unclear how the COVID-19 pandemic influenced risk perception and help-seeking behaviours among pregnant and postpartum individuals. OBJECTIVES: To explore pregnant and postpartum individuals' decision-making process about when and how to seek support for feelings of depression and/or anxiety during the COVID-19 pandemic. DESIGN: A qualitative descriptive design was used. METHODS: The current study is a secondary analysis of qualitative data collected for a larger mixed-methods project th

The association of maternal mental health with prenatal smoking cessation and postpartum relapse in a population-based sample.

To examine the association between mental health factors with smoking cessation during pregnancy and postpartum relapse. We used data from 1,416 women who participated in the Minnesota Pregnancy Risk Assessment Monitoring System survey in 2004-2006 and reported smoking immediately prior to pregnancy. Maternal mood during pregnancy, stressful life events and postpartum depression were the mental health variables. We used multivariate logistic regression to examine the association between these variables and two outcomes, smoking cessation during pregnancy and postpartum relapse. Stressful life

Less depressed or less forthcoming? Self-report of depression symptoms in women preparing for in vitro fertilization.

While depression has been associated with infertility treatments, it is not routinely assessed in women prior to undergoing in vitro fertilization (IVF) treatment. Findings are mixed regarding the degree to which women report depression prior to IVF. The purpose of this study was to (1) examine response profiles in women preparing for IVF and (2) compare responses to those of postpartum, primary care, and general population groups. Female IVF patients (n = 321; 19-45 years) completed the Patient Health Questionnaire-9 (PHQ-9) at their first visit. Clinical and demographic characteristics and i

Neurosteroids and translocator protein 18 kDa (TSPO) ligands as novel treatment options in depression.

Recently, the gamma-aminobutyric acid (GABA) system has come into focus for the treatment of anxiety, postpartum depression, and major depressive disorder. Endogenous 3α-reduced steroids such as allopregnanolone are potent positive allosteric modulators of GABAA receptors and have been known for decades. Current industry developments and first approvals by the U.S. food and drug administration (FDA) for the treatment of postpartum depression with exogenous analogues of these steroids represent a major step forward in the field. 3α-reduced steroids target both synaptic and extrasynaptic GABAA r

Mental health outcomes among Chinese prenatal and postpartum women after the implementation of universal two-child policy.

BACKGROUND: Poor mental health status among both pregnant and postpartum women is commonly reported worldwide. The associations between mental health outcomes and giving birth to the second child since the implementation of China's universal two-child policy have not been identified. METHODS: A large-scale based mental health survey was conducted between March 2017 and December 2018 in Suzhou, China. The survey evaluated the symptoms of anxiety, hypomania, depression and poor sleep quality among both pregnant and postpartum women. RESULTS: A total of 3113 questionnaires were collected, the p

Web-based interventions for prevention and treatment of perinatal mood disorders: a systematic review.

BACKGROUND: Perinatal depression is strikingly common with a prevalence of 10-15%. The adverse effects of perinatal depression on maternal and child health are profound with considerable costs. Despite this, few women seek medical attention. E-health, providing healthcare via the Internet is an accessible and effective solution for the treatment of depression in the general population. We aimed to conduct a systematic review of web-based interventions for the prevention and treatment of mood disorders in the perinatal period, defined as the start of pregnancy to 1 year post-partum. METHODS: S

The Impact of Potentially Traumatic Perinatal Complications on Postpartum Psychological Distress and Cardiometabolic Risk: Implications for Women's Heart Health.

OBJECTIVE: Perinatal complications are well-documented cardiovascular disease risk factors. However, the potential for these complications to be psychologically traumatic due to their life-threatening nature-and their association with postpartum psychological distress and cardiometabolic risk-remains underexplored. This gap is important, as perinatal complications, trauma and related distress, and cardiometabolic risk factors are independently linked to cardiovascular disease and may have a compounded impact when co-occurring. METHODS: Data were analyzed from 2510 predominantly low-income, ra

Trajectories of depressive symptoms of mothers and fathers over 11 years.

Aims. Parental postpartum depressive symptoms have been extensively studied, but the combined longitudinal depression trajectories of parents and their long-term development beyond the postpartum period remain largely underexplored. We identified dyadic longitudinal depressive symptom trajectories in new parents, followed over an 11-year period, and compared parental characteristics, as well as child temperament and mental health factors, across different parental trajectory classes. Methods. A prenatal cohort of 5,518 couples was studied. Depressive symptoms were measured using the Edinburgh

Increased postpartum anxiety symptoms after perinatal SARS-CoV-2 infection in a large, prospective pregnancy cohort in New York City.

Numerous studies reported an increase of postpartum mood symptoms during the COVID-19 pandemic. Yet, the link between severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and perinatal mental health is less well understood. We investigated the associations between prenatal SARS-CoV-2 infection and postpartum depressive and anxiety symptoms, including examinations of infection timing and pandemic timeline. We included 595 participants from Generation C, a prospective pregnancy cohort in New York City (2020-2022). Prenatal SARS-CoV-2 infection was determined via laboratory or m

Prediction of Postpartum Weight in Low-Income Mexican-Origin Women From Childhood Experiences of Abuse and Family Conflict.

OBJECTIVES: The postpartum period represents a crucial transition period in which weight gain or loss can affect lifetime obesity risk. This study examined the prevalence of obesity and the influence of childhood abuse and family conflict on postpartum weight among low-income Mexican-origin women. Depressive symptoms and partner support were evaluated as mediators. METHODS: At a prenatal assessment, low-income Mexican-origin women (N = 322; mean [SD] age, 27.8 [6.5]) reported on childhood abuse and family conflict. Weight was measured 7 times between 6 weeks and 2 years postpartum and calcula

Sex after childbirth: postpartum sexual function.

Pregnancy and childbirth bring many changes to the health and well-being of new mothers. Postpartum sexual health is a common concern that is often not discussed during prenatal or postpartum care and has received little attention from either clinicians or researchers. In this article, we review current theories of female sexual response, the epidemiology of postpartum sexual dysfunction, and the use of screening tools to identify women with sexual health concerns. Specifically, we present a review of published data regarding the effect of mode of delivery, perineal lacerations, postpartum dep

Genetic associations of perinatal pain and depression.

Underlying genetic influences may affect perinatal pain, depression, or both. We investigated the role of 59 single-nucleotide polymorphisms on 20 quantitative traits measured in perinatal women. Moreover, 183 pregnant women (28–37 weeks’ gestation) were prospectively genotyped for single-nucleotide polymorphisms with known prior associations with either pain or depression in nonpregnant populations. Prenatal saliva samples were collected. Phenotypic data were gathered during prenatal, labor and delivery, and postpartum (six weeks and three months) periods, capturing labor pain, Edinburgh Post

Suicidal ideation across pregnancy and the postpartum: Trajectories and assessment with commonly used screening tools.

Suicidal ideation (SI) is a public health concern that remains poorly characterized during the perinatal period. It is most often assessed by the final items in two depression screening instruments, the Edinburgh Postnatal Depression Scale (EPDS) and the Patient Health Questionnaire-9 (PHQ-9). We sought to examine and compare SI trajectories identified by these measures and their changes from pregnancy to postpartum. This study reports on data from a supplemental study to the Understanding and Preventing Women's Relapse of Depression (UPWARD) study on perinatal depression (NIMH, NCT03623620; s

Patterns of respectful care and mistreatment during childbirth in relation to perinatal mental health: a secondary analysis of listening to mothers in California.

PURPOSE: Despite mental health conditions being the leading cause of maternal mortality in the United States, the relationship between perinatal mental health and mistreatment during childbirth has been insufficiently examined. The objective was to identify patterns of respectful care and mistreatment during childbirth and examine associations with perinatal mental health. METHODS: We conducted a cross-sectional secondary analysis of Listening to Mothers in California (N = 2,539). We conducted latent class analysis of indicator variables describing rough or rude care, discriminatory treatment,
