Six patients with perioral dermatitis were successfully treated with a combination of 1.5 percent erythromycin topical solution twice a day and topically applied hydrocortisone valerate cream. Application of erythromycin solution seems to be an effective topical therapy. Hydrocortisone valerate cream, when used in a controlled, tapered regimen seems to prevent acute rebound flare of perioral dermatitis from previous high-potency steroid use or abuse.

An 84-year-old man was admitted to the hospital with a diabetic foot ulcer and osteomyelitis of the calcaneum. While being treated with cefoxitin sodium, he experienced generalized exfoliative dermatitis, which subsided on discontinuation of therapy. To our knowledge, this is the first report of exfoliative dermatitis with a cephalosporin or related drug.

A patient with a suspected allergic contact dermatitis produced by ethylene oxide (EO), is described. Epicutaneous tests in the patient and in controls with materials treated with ethylene oxide, and ethylene chlorhydrin supported the allergic mechanism. Chromatography studies of different hospital samples sterilized with EO showed strong evidence of abnormal amounts of irritant substances.

Liver function tests were performed in 60 patients with dermatitis herpetiformis. Abnormalities of bilirubin or elevated liver enzyme levels were found in 17% of the patients. 13% had abnormal liver enzymes or elevated bilirubin levels not due to haemolysis. In 8% of patients bilirubin levels were elevated as a result of dapsone-induced haemolysis. The incidence of liver function abnormalities was higher (19%) in those on a normal diet than those on a gluten-free diet (9%). Twenty-one patients with dermatitis herpetiformis were typed for HLA-ABC and -DR determinants. The incidence of HLA-AI, -B8 and -DR3 antigens was found to be significantly higher (P: = 10(-3), less than 10(-6) and less than 10(-6), respectively) among patients with dermatitis herpetiformis than among the normal population. HLA-DR3 was found in 85.7% of patients, HLA-B8 in 66.7% and HLA-AI in 61.9% only. Three out of 4 patients with coexistent diabetes mellitus and dermatitis herpetiformis (DH) developed severe renal failure which resulted in death in 2 of them. Diabetes mellitus and DH may coexist more frequently than would occur by chance, and such patients may run an increased risk from severe renal complications of diabetes.

The rare association of lepromatous leprosy and allergic contact dermatitis due to chromium in cement was confirmed clinically and biologically in a male patient. The apparent lack of influence of lepromatous leprosy on the development and maintenance of this dermatitis dependent upon cell-mediated immune mechanisms is discussed.

A patient had a localized, persistent vesicular eruption of the feet that was clinically identical to dyshidrosiform dermatitis. Direct and indirect immunofluorescent studies revealed a bullous pemphigoid (BP) pattern. Antibody blocking experiments showed the patient's circulating basement membrane zone antibody to be identical to that of classical BP. Possible pathogenetic mechanisms of localized BP are discussed.

The prevalence of contact allergic dermatitis and nonallergic irritant reactions to neomycin sulfate was determined in a large group of young adults who had a noteworthy exposure to topical antibiotics. Only two subjects (0.09%) had contact allergy proved by closed patch testing, while 20 (0.9%) had nonallergic irritant reactions to 20% neomycin sulfate and 17 (0.78%) had nonallergic reactions to one of the hydrophilic ointment bases. Patients with dermatitis herpetiformis were found to have an increased incidence of atopic disorders compared to an age and sex matched control group. The increase in incidence was statistically significant for atopic eczema and any atopic disorder. Serum immunoglobulins (including IgE) were estimated in forty-five dermatitis herpetiformis patients and no significant abnormalities were found.
