Throughout the last century, the incidence of diverticular disease of the colon has increased tremendously in industrialized countries; neve … similar in all Scandinavian countries. Computed tomography is the first choice for imaging of acute diverticulitis and its complications. Furthermore, the use of antibiotics in uncomplicated diverticulitis is nearly abandoned in Scandinavia, whereas several international guidelines still recommend their use. There is a broad consensus that abscesses secondary to …

Sigmoidal diverticulitis is a regular reason for emergency referral. Although the treatment is well established, we describe an unusual complication that raises concerns about its etiopathogeny and treatment: an isolated pneumoretroperitoneum complicating a sigmoidal diverticulitis.

A recent review of the surgical management of diverticular disease of the colon included 73 patients. One-stage resections were accomplished in 67%, primarily in patients operated on electively for recurrent diverticulitis or fistula, and in the group of patients with cecal diverticulitis operated on for presumed appendicitis. Single-stage resection with ileorectal anastomosis is also preferred in patients with massive diverticular bleeding. For patients with large abscesses or diffuse peritonitis a two-stage procedure which removes the site of disease in the initial operation is recommended. …

BACKGROUND: Patients who are suspected of having acute appendicitis usually undergo surgery in order to avoid life-threatening complications such as perforation and peritonitis. However, acute appendicitis is difficult to distinguish from other sources of right-sided abdominal pain. The clinical picture is almost indistinguishable from appendiceal diverticulitis, which is a rare entity and remains a difficult diagnostic problem.
PATIENTS AND METHODS: We describe the case of a 39-year-old male with perforated appendiceal diverticulitis. The patient was admitted to our surgical unit with acute …

We report a rare case of perforated jejunal diverticulitis after an unrelated surgery. A 60-year-old male presented with severe abdominal pain one day after robot-assisted radical prostatectomy. This was diagnosed as perforated jejunal diverticulitis and managed via immediate partial jejunal resection. Key points are as follows. First, surgery may have indirect impacts on diverticulitis perforation because of postoperative intestinal hypomobility and immunosuppression. Second, the diagnosis becomes harder when the perforation occurs after surgery because postoperative laboratory and imaging fi…

BACKGROUND: Recent genetic studies identified common mutations between diverticular disease and connective tissue disorders, some of which are associated with abdominal wall hernias. Scarce data exists, however, shedding light on the potential clinical implications of this shared etiology, particularly in the era of laparoscopic surgery.
METHODS: The New York Statewide Planning and Research Cooperative System database was used to identify adult patients undergoing elective sigmoid and left hemicolectomy (open or laparoscopic) from January 1, 2010, to December 31, 2016, for diverticulitis or d…

The co-occurrence of diverticulitis with pregnancy is incredibly rare and the management of recurrent complicated diverticulitis may not be feasible in a pregnant patient. Adding cases to the incredibly sparse literature may highlight similarities and create potential recommendations for at-risk populations. We present a case of a female at 18 weeks' gestation who presented with left lower quadrant pain. The initial physical exam and clinical findings revealed severe abdominal tenderness without signs of generalized peritonitis, leukocytosis with predominant neutrophils, and fundal height with…

BACKGROUND: There are various procedures to be considered in the surgical treatment of complicated diverticulitis, which must be selected depending on the classification of diverticular disease (CDD) type and the condition of the patient.
OBJECTIVE: Comparison of surgical procedures with respect to aspects such as morbidity, mortality, reconstructive surgery and postoperative quality of life.
MATERIAL AND METHODS: Evaluation, analysis and assessment of the current literature on surgical treatment of diverticular disease.
RESULTS: Laparoscopic sigmoid resection with primary anastomosis is no…

CASE: Diverticular disease and its associated complications are an increasing problem among Western populations. Perforated diverticulitis results in intra-abdominal sepsis, which rarely causes a life-threatening necrotizing soft-tissue infection. We present the case of a 70-year-old man with a sigmoid diverticulitis perforation presenting as necrotizing fasciitis of the thigh and calf requiring serial debridement and major limb reconstruction.
CONCLUSION: Clinicians should be aware of index presentations of diverticulitis and its recognized complications in this demographic group. For the or…

INTRODUCTION: Hepatic abscesses have been reported as a rare complication of diverticulitis of the bowel. This complication is recognized more commonly at the time of the diagnosis of diverticulitis, or ruptured diverticula, but also can be diagnosed prior to surgery, or postoperatively.
CASE PRESENTATION: This report describes a man who developed an hepatic abscess within a simple hepatic cyst, two months after operation for ruptured diverticula of the sigmoid colon. The abscess was drained surgically and the patient made a complete recovery.
CONCLUSION: The development of an hepatic absces…

OBJECTIVE: To examine morbidity, mortality, conversion rates, and disease recurrence after laparoscopic resection of complicated and uncomplicated diverticular disease in a single center.
SUMMARY BACKGROUND DATA: In contrast to colorectal cancer, there are few large studies of laparoscopic or open resection for diverticular disease.
METHODS: This study represents a retrospective analysis of a prospectively collected database of all laparoscopic resections for uncomplicated and complicated diverticulitis from a single center.
RESULTS: Five hundred patients (305 female) were identified (media…

Some 10 to 25 percent of all patients with diverticulosis will come down with diverticulitis in the course of their lives. Apart from physical examinations, sonography and--if results are still unclear--computer tomography are seen as the most important diagnostic devices. 75 percent of patients with acute diverticulitis can initially be treated conservatively, and only one in four ever falls ill again with another inflammation. According to those numbers a conservative method is justified when dealt with a primary uncomplicated diverticulitis. If a second attack occurs, should an elective res…

OBJECTIVE: The epidemiology and the aetiology of inflammatory diseases of the vermiform appendix remain poorly understood. The prevalence of appendiceal diverticulosis and diverticulitis in patients undergoing appendectomy for suspected acute appendicitis was investigated.
METHODS: A retrospective study was completed on patients who underwent appendectomy for suspected acute appendicitis. Pathology reports of all patients were screened for diverticula of the vermiform appendix. Patients with either diverticulitis of the vermiform appendix or normal appendicitis were compared.
RESULTS: Out of…

BACKGROUND: We identified areas of controversy in the management of patients with diverticular disease to develop key questions addressing the management of symptomatic uncomplicated diverticular disease (SUDD), uncomplicated diverticulitis, and complicated diverticulitis.
METHODS: We performed a systematic review to identify randomized trials and cohort studies addressing our key questions. Informed by an updated evidence synthesis, an international, multidisciplinary panel of gastrointestinal surgeons, gastroenterologists, patient partners, and methodologists applied the Grading of Recommen…

INTRODUCTION: To prevent an acute surgery, classic indications for elective sigmoid resections concerning diverticulitis have usually been based on the number of recurrent episodes. Since 2005 these indications have been challenged, primarily because the majority of patients first present themselves with acute complications at their first episode.
METHODS: Between 1990 and 2000, a cohort analysis was conducted involving all patients admitted to the VU University Medical Center with the diagnosis of diverticulitis, with a follow-up until January 2009. To identify those patients who might benef…

Diverticulosis is extremely common in Western societies and is associated with complications in up to 15%of cases. Altered motility is an important feature of the pathogenesis of diverticular disease, and serotonin (5-HT) release is a primary trigger of gut motility. This study aims to determine whether colonic 5-HT signaling is altered in patients with diverticulosis or diverticulitis, and whether differences in serotonin signaling may distinguish patients with asymptomatic diverticulosis from those who develop disease specific complications. Sigmoid colon biopsies were obtained from healthy …

Diverticular disease affects more than 50% of the population over the age of 60 years in the west and becomes even more common as the population ages. Diverticulitis is one of the complications of diverticular disease and can culminate into colonic perforation. Though perforated diverticular disease is not uncommon, synchronous colonic perforations in diverticulitis is rare. Our patient was admitted with acute abdomen and exploratory laparotomy revealed two side-by-side perforations of the sigmoid colon. A Hartmann's procedure was performed. Macro- and microscopic evaluation confirmed the pres…
