Amoebic cervicitis mimicking posterior wall fibroid: a rare presentation.

Amoebiasis of the uterine cervix is an extremely rare entity, and presentation as fibroid uterus has not been reported, to the best of our knowledge in our extensive search of the English literature. It can clinically simulate cervical malignancy by virtue of surface papillomatous and overall ulcerated and necrotic appearance. We present a case of amoebic infection of the cervix in a 45-year-old female which was suspected to be a posterior wall fibroid with degeneration until a histopathological examination of the surgical specimen revealed the presence of Entamoeba histolytica trophozoites. T…

Invasion mechanisms among emerging food-borne protozoan parasites.

Food-borne parasitic diseases, many known to be more prevalent in poor countries with deficient sanitary conditions, are becoming common worldwide. Among the emerging protozoan parasites, the most prominent is Trypanosoma cruzi, rarely reported in the past to be transmitted by the oral route but currently responsible for frequent outbreaks of acute cases of Chagas disease contracted orally and characterized by high mortality. Several other food-borne protozoans considered emerging include the apicomplexans Toxoplasma gondii and Cryptosporidium, as well as Giardia and Entamoeba histolytica. Her…

Influence of inflammation on parasitism and area of experimental amoebic liver abscess: an immunohistochemical and morphometric study.

The influence of inflammation on the number of trophozoites and on the murine amoebic liver abscess area following infection with Entamoeba histolytica and E. dispar was evaluated. Immunohistochemistry and digital morphometry were used to identify and quantify the trophozoites, neutrophils, macrophages, and lesions. Positive correlation was observed between the number of trophozoites and inflammatory cells. A significant decrease in parasitism and inflammation in groups treated with dexamethasone was observed. The scarceness or absence of trophozoites in the treated groups suggest the importan…

Antiprotozoal activities of phospholipid analogues.

The antiprotozoal activity of phospholipid analogues, originally developed as anti-cancer drugs, has been determined in the past decade. The most susceptible parasites are Leishmania spp. and Trypanosoma cruzi with activity also shown against Trypanosoma brucei spp., Entamoeba histolytica and Acanthamoeba spp. Miltefosine, an alkylphosphocholine, was registered for the oral treatment of visceral leishmaniasis (VL) in India in March 2002. This review will focus on the biological activities of phospholipid analogues. Biochemical and molecular targets and mechanism(s) of action have been studied …

How protozoan parasites evade the immune response.

Protozoan pathogens such as Plasmodium, Leishmania, Trypanosoma and Entamoeba are responsible for several of the most widespread and lethal human diseases. Their successful survival depends mainly on evading the host immune system by, for example, penetrating and multiplying within cells, varying their surface antigens, eliminating their protein coat, and modulating the host immune response. Immunosuppression is sometimes caused directly by parasite products and sometimes involves antigenic mimicry, which often appears in association with parasitic diseases. However, one of the most sophistica…

Amebiasis.

Human amebiasis is caused by the protozoan parasite Entamoeba histolytica. Invasion of the colonic mucosa by the trophozoite form of this parasite generally results in the clinical syndrome of amebic dysentery, although there is a wide range of clinical presentations. Some patients will progress to disseminated disease, with resulting amebic liver abscesses. Although it is estimated that 10% of the world's population is infected with E. histolytica, only 1% of these patients will manifest disease. The diagnosis is usually made by microscopically identifying the microorganism in a stool specime…

Massive amebic liver abscess--an unusual presentation.

A 40-year-old woman presented with a huge lump in the right half of the abdomen and irregular menses. Ultrasonography showed a cystic lump with septations, extending from the upper abdomen to the pelvis; the right ovary was not seen. On exploration, there was a large cyst arising from the right lobe of the liver; the aspirate was bilious. Since the cyst wall was adherent to retroperitoneal structures, complete excision was not possible. A roux-en-Y loop of jejunum was anastomosed to the cyst wall. Biopsy of the wall showed inflammatory granulation tissue with trophozoites of Entamoeba histolyt…

Ruptured liver abscess with fulminant amoebic colitis: case report with review.

Amoebic liver abscess is the commonest extra intestinal manifestation of amoebiasis. Intraperitoneal rupture of liver abscess and fulminant necrotizing amoebic colitis are rare occurrences which complicate a severe form of invasive disease caused by Entamoeba histolytica. These complications are associated with a high morbidity and mortality. Synchronous pathological lesions in colon and liver are rare. Still rare is the occurrence of complicated colonic and hepatic invasive amoebiasis presenting as an acute abdomen. One such presentation of ruptured liver abscess and necrotizing amoebic colit…

First report of invasive amebiasis in an organ transplant recipient.

T-cell mediated immunity is an important defense mechanism against amebiasis. However, organ transplant recipients with severe T-cell immunosuppression are not at increased risk of having Entamoeba histolytica invasive disease. The reasons are unclear and probably multifactorial, but it is likely that the absence of intestinal colonization with pathogenic strains in countries where transplants occur and the judicious intake of possible contaminated food and water are important contributing factors. We describe the first report of a liver transplant recipient with severe E. histolytica colitis …

Immunological similarity between the 170 kD amoebic adherence glycoprotein and human beta 2 integrins.

Amoebiasis is an important cause of morbidity and mortality in developing countries. Virulence of Entamoeba histolytica depends on the trophozoite's being able to bind to colonic epithelium; binding is mediated by the 170 kD subunit of a 260 kD lectin. We found, by microcytofluorometry and western blotting studies, that this 170 kD subunit protein shares an epitope with human beta 2 integrins--a subfamily of cell-surface glycoproteins expressed exclusively on leucocytes and involved in many adhesive and signalling interactions. The immunological similarity between these proteins may explain th…

Squamous cell carcinoma associated with amoebic cervicitis. Report of a case.

A case of squamous cell carcinoma associated with amebiasis is presented. Protozoa identified as Entamoeba histolytica were found in a routine Papanicolaou smear and in the cervical biopsy of a young woman following the initial diagnosis of cervical cancer and the institution of a radiation therapy regimen. No amoebae were found in repeated stool specimens. The late detection and diagnosis of amebiasis precluded any possible changes in the management of the case. Initial follow-up for the carcinoma demonstrated that the patient was not responding well to therapy; her failure to return to the c…

High-throughput phenotypic screen identifies a new family of potent anti-amoebic compounds.

Entamoeba histolytica is a disease-causing parasitic amoeba which affects an estimated 50 million people worldwide, particularly in socioeconomically vulnerable populations experiencing water sanitation issues. Infection with E. histolytica is referred to as amoebiasis, and can cause symptoms such as colitis, dysentery, and even death in extreme cases. Drugs exist that are capable of killing this parasite, but they are hampered by downsides such as significant adverse effects at therapeutic concentrations, issues with patient compliance, the need for additional drugs to kill the transmissible …

Treatment of intestinal parasites in Australian aboriginal children.

Five to nine-year-old Aboriginal children on Mitchell River Community maintained a satisfactory growth rate in the presence of multiple infections with intestinal parasites. Intensive treatment was successful in eliminating Giardia lamblia, Entamoeba histolytica, and Strongyloides stercoralis infections, but reinfection with G. lamblia was rapid. Treatment failed to produce any growth spurt in the group. This may have been due to the rapidity of reinfection, but raises the question of whether intestinal parasites contribute significantly to growth retardation or whether growth retarded childre…

In vitro amoebicidal activity of immune cells.

The amoebicidal activity of peripheral blood lymphocytes and spleen and peritoneal cells from hamsters vaccinated against or protected from hepatic amoebiasis and from those with hepatic amoebiasis was investigated. Peripheral blood lymphocytes and peritoneal and spleen cells from vaccinated or protected animals can kill trophozoites of Entamoeba histolytica in vitro. In contrast, spleen cells from infected hamsters showed no significant cytotoxic effect on the parasite. These data suggest that cellular immunity plays an important role in host defense against hepatic amoebiasis.

Double Trouble: The Perilous Intersection of Amoebic Colitis and Candida Infection Leading to Colonic Perforation.

Amoebic colitis, a parasitic infection caused by Entamoeba histolytica, can lead to severe gastrointestinal symptoms. The clinical manifestations can vary widely, from being an asymptomatic carrier to experiencing severe colitis and even colonic perforation. Surgical treatment for fulminant amoebic colitis and colonic perforation should be carried out at the earliest. Superadded fungal infections in these patients are rare and can significantly complicate the clinical course. Here, we present a unique case of amoebic colitis complicated by a superadded Candida infection, culminating in colonic…
