A Case Series of Bosworth Fracture-Dislocations and Review of 129 Reported Cases.

INTRODUCTION: Bosworth fracture dislocations of the ankle are rare injuries of the ankle caused by extreme external rotation of the supinated foot where the proximal fibula fracture fragment is posteriorly dislocated and entrapped behind the posterior-lateral ridge of the tibia. This case series aims to document three such cases treated in our institution over a nine year period. We also provide a review of 129 cases in the existing literature. MATERIALS AND METHODS: Medical records and relevant radiographs for each patient were analysed and collected from the time of presentation till the po

ChronOS inject in children with benign bone lesions--does it increase the healing rate?

OBJECTIVE: Benign bone lesions in children although rare, can result in a pathological fracture. Although their etiology and pathogenesis are not yet entirely clear, the phenomenon of spontaneous healing is well known. Nevertheless, some benign bone lesions are unlikely to heal spontaneously due to the patient's age or high risk of fracture and deformity due to the lesion's location or size. The following study presents our results after treatment of these bone cysts with chronOS Inject. METHODS: From June 2004 to May 2007 23 patients with 24 benign bone cysts were treated with chronOS Inject

Ilizarov Circular Fixation for High-Velocity Firearm Injuries Causing High-Energy Tibial Fractures: A Prospective Cohort Study.

High-velocity gunshot tibial fractures present major reconstructive challenges owing to extensive contamination, soft-tissue devitalisation, and segmental bone loss that render early internal fixation hazardous. We aimed to document outcomes of a structured Ilizarov-based protocol for this injury type and describe a reproducible treatment framework for resource-constrained settings. Between January 2019 and December 2023, we prospectively enrolled 20 adult male patients with open tibial fractures from high-velocity firearm injuries at three Cairo trauma centres; all were treated definitively w

Tibial shaft fracture with knee and proximal tibiofibular dislocation: Case report.

Tibial shaft fractures are the most common type of long bone fracture, frequently associated with high-energy traumas such as traffic accidents, which carry a high mortality rate in Colombia. The combination of an open tibial shaft fracture with knee and proximal tibiofibular joint dislocation is extremely rare. The presented case involves these injuries, which were managed surgically in a single-stage with a follow-up of six months and good functional outcomes. Intramedullary nailing of the tibia was performed using an extra-articular, extra-synovial, semi-extended technique, which optimized

Is Using a Ring External Fixator in the Treatment of Congenital Pseudarthrosis of the Tibia Associated With Better Results or Using a Locking Plate?

BACKGROUND: Congenital pseudarthrosis of the tibia (CPT) is a rare disease. CPT is often unilateral and occurs between the middle and distal third of the tibia. Concurrent involvement of the fibula is present in more than half of cases. histologic studies indicate the presence of fibrous hamartoma tissue and a sick periosteum, which leads to recalcitrant bone fracture and, eventually, pseudoarthrosis. Although there are various surgical techniques, we intend to compare the 2 methods of external fixation versus internal plating. METHODS: Demographic data were collected from 26 patients with fr

Strength of Syndesmosis Fixation: Two TightRope versus One TightRope with Plate-and-Screw Construct.

Injuries involving the distal tibiofibular syndesmosis can lead to critical destabilization of the ankle mortise. Although specific indications for operative fixation remain unclear, accurate reduction of the syndesmosis has been correlated with the best functional outcomes. The purpose of this study was to evaluate the maximum torque and rotation to failure after fixation with a novel construct. Seven pairs (14 ankles) of embalmed cadaveric lower legs, disarticulated at the knee, were obtained. Each pair was randomly assigned to receive either two TightRopes (Arthrex) or a plate-and-screw con

The influence of distal locking on the need for fibular plating in intramedullary nailing of distal metaphyseal tibiofibular fractures.

Using human cadaver specimens, we investigated the role of supplementary fibular plating in the treatment of distal tibial fractures using an intramedullary nail. Fibular plating is thought to improve stability in these situations, but has been reported to have increased soft-tissue complications and to impair union of the fracture. We proposed that multidirectional locking screws provide adequate stability, making additional fibular plating unnecessary. A distal tibiofibular osteotomy model performed on matched fresh-frozen lower limb specimens was stabilised with reamed nails using conventio

Residual angulation of distal tibial diaphyseal fractures in children younger than ten years.

BACKGROUND: The purpose of this study was to evaluate the factors that influence residual angulation after treating pediatric distal tibial diaphyseal fractures. METHODS: We retrospectively reviewed the records of 75 children under the age of ten who were treated at two referral centers for distal tibial diaphyseal fractures. The mean patient age was 6.8 ± 2.3 years, and the mean follow-up duration was 4.1 ± 1.3 years (range, 3 to 6 years). Early postoperative and late follow-up radiographs were used to measure angulation. RESULTS: Twenty-four patients had valgus angulations >5° at the final

The extensile approach for the operative treatment of high-energy pilon fractures: surgical technique and soft-tissue healing.

Perhaps the most important advancement in the surgical treatment of high-energy pilon fractures has been the recognition of the need to delay primary surgery. However, at open reduction internal fixation an adequate incision must be made to clearly visualize the articular surface in an attempt to restore intraarticular anatomy. This article illustrates our extensile approach and its effect on soft-tissue healing. The approach allows complete access to the ankle joint to achieve reduction and fixation of the articular surface, as far medially or laterally as is necessary. In addition, it allows

The use of early flap coverage in severe open physeal fractures: a report of two cases.

Open physeal fractures associated with severe overlying soft tissue loss present significant challenges to the pediatric orthopaedic surgeon. In addition to providing adequate soft tissue coverage, preventing chronic infection, and achieving bony healing, surgical care should attempt to preserve the physis to maintain growth for the best long-term outcomes. Here we present two cases of open physeal disruptions with severe soft tissue loss. In each case, early use of vascularized flap coverage assisted in fracture healing and contributed to preservation of the physis and subsequent bony growth.

A cohort study to determine the minimal clinically important difference when using an anchor-based approach of the PROMIS Mobility score in children with lower limb injuries.

AIMS: The Patient-Reported Outcomes Measurement Information System (PROMIS) Mobility score is increasingly used as an outcome tool in lower limb fractures in children. We sought to determine the minimal clinically important difference (MCID) in these patients, using an anchor-based approach. METHODS: A prospective longitudinal cohort study involved the use of the PROMIS questionnaires alongside an anchor question at five timepoints over a period of six months. Children aged between five and 15 years with a newly diagnosed, isolated fracture of the femur, tibia, fibula, ankle, or metatarsals w

Optimal hinge level in opening wedge high tibial osteotomy: Biomechanical analysis using finite element method.

BACKGROUND: While the concept of a safe zone, which can minimize the hinge fracture when performing opening wedge high tibial osteotomy, has been introduced, there is a lack of understanding of the biomechanical environment at the lateral tibial cortex. This study aimed to evaluate the effect of the hinge level on the biomechanical environment at the lateral cortex of the tibia with heterogeneous finite element models. METHODS: Finite element models of biplanar opening wedge high tibial osteotomy based on computed tomography images of a control subject and three patients with medial compartme

Acute compartment syndrome of the foot following fixation of a pilon variant ankle fracture.

Acute traumatic compartment syndrome of the foot is a serious potential complication after fractures, crush injuries, or reperfusion injury after vascular repair. Foot compartment syndrome in association with injuries to the ankle is rare. This article presents a case of acute compartment syndrome of the foot following open reduction and internal fixation of an ankle fracture. A 16-year-old girl presented after sustaining a left ankle injury. Radiographs demonstrated a length-stable posterior and lateral malleolar ankle fracture. Initial treatment consisted of a bulky splint and crutches pendi

Double-crush syndrome after acetabular fractures. A sign of poor prognosis.

Injury to the sciatic nerve is one of the more serious complications of acetabular fracture and traumatic dislocation of the hip, both in the short and long term. We have reviewed prospectively patients, treated in our unit, for acetabular fractures who had concomitant injury to the sciatic nerve, with the aim of predicting the functional outcome after these injuries. Of 136 patients who underwent stabilisation of acetabular fractures, there were 27 (19.9%) with neurological injury. At initial presentation, 13 patients had a complete foot-drop, ten had weakness of the foot and four had burning

[Osteochondral lesions of the talus].

Osteochondral lesions occur either as osteochondral fractures (so called flake fractures) or osteochondritis dissecans. Both types of lesions are caused in the most of the cases by an adequate trauma. The injury is sustained during inversion of the ankle. If the foot is dorsiflexed, an anterolateral lesion will result from shearing forces by the fibula. If the inversed foot is plantar flexed and followed by rotation of the tibia on the talus, a postero-medial lesion will result from compression of the medial talar dome by the tibia, secondary to spiralling and shortening of the collateral liga

Retrograde intramedullary headless compression screw fixation for distal fibular fractures.

BACKGROUND: Although intramedullary headless compression screw fixation for distal fibular fractures has been described, its biomechanical properties remain underexplored in orthopedic literature. Distal fibular fractures are commonly treated with plate-and-screw constructs; however, complications related to hardware prominence, soft tissue irritation, and hardware removal are frequent. Retrograde intramedullary headless compression screw fixation may offer a biomechanically stable, minimally invasive alternative. OBJECTIVE: To biomechanically compare retrograde intramedullary headless compre

Non-union fracture neck femur in a toddler: Reconstructed by valgus osteotomy - A minimally invasive approach.

BACKGROUND: Non-union is one of the devastating complications of fracture neck of femur. Though a very rarely encountered entity in a toddler (1-3 years paediatric age group), non-united femoral neck fractures are reported in developing countries because of mismanagement by quacks and delay in referrals. For operative treatment, many different procedures have been described, including close/open reduction and internal fixation using K-wires, cannulated screws, fibula or nails. There is no evidence in the literature that one or other implant influences the rate of postoperative complications, s
