Complex Distal Radius Fractures: An Anatomic Algorithm for Surgical Management.

Most distal radius fractures result from low-energy mechanisms and can be successfully treated nonsurgically or with a variety of surgical techniques if indicated. High-energy distal radius fractures can involve extensive comminution or bone loss with concomitant ligament, soft-tissue, and neurovascular injuries. In patients with complex distal radius fractures, reconstruction of the distal radius, ulna, and triangular fibrocartilage complex can be challenging. Effective restoration of the bony architecture requires intimate knowledge of the anatomy of the distal radius, a thorough understandi

Titanium mini-plate for securing septal cartilage--a case of high-impact nasal trauma.

A technique is presented of securing the cartilaginous nasal septum using titanium mini-plates in the repair of a complex open comminuted type I nasoethmoid fracture, with internal impaction of the external cartilaginous nose. This technique allows primary skin closure and obviates the need for external metal-work and the attendant potential complications.

Problems of operative and non-operative treatment and healing in tibial fractures.

Based on the experience of the department of pediatric surgery at the Strasbourg University Hospital, the authors present indications and limitations of various treatment techniques for tibial shaft fractures in children. Two retrospective series are reported on: one group of patients treated with the Sarmiento technique and one group of patients treated with Elastic Stable Intramedullary Nailing (ESIN or Métaizeau technique). The main problem in treatment is restoration of the mechanical axis of the tibia, especially in isolated fractures treated conservatively. Residual varus deformity is fr

The role of three-dimensional computed tomography in the management of maxillofacial trauma.

Open reduction and internal fixation of facial fractures demand a detailed understanding of the three-dimensional pattern of injury. This is difficult if not impossible to obtain with present radiographic methods. The purpose of this study was to define the role of three-dimensional computerized tomography (3DCT) in surgical management of facial fractures. The two-part investigation: 1) compared the diagnostic accuracy of 3DCT with conventional CT and plain film studies; and 2) examined the clinical usefulness of 3DCT to surgeons. Twenty-four acute trauma patients suspected of having facial fr

Maxillofacial trauma patterns associated with external auditory canal fractures: Cone beam computed tomography analysis.

BACKGROUND/AIMS: There is a paucity of literature on external auditory canal (EAC) fractures secondary to maxillofacial trauma, with most of the literature on EAC fractures consisting of isolated case reports. To the authors' best knowledge, this is the first study to use cone beam computed tomography to evaluate the EAC region. The aim of this study was to assess the prevalence of external auditory canal (EAC) fracture following maxillofacial trauma and to evaluate the association between EAC fracture and other maxillofacial fractures and the region of trauma. MATERIALS AND METHODS: One hund

Comparison of Incremental Costs and Medicare Reimbursement for Intra- Versus Extra-Articular Distal Radius Fracture Surgery Using Time-Driven Activity-Based Costing.

PURPOSE: Balancing surgical costs of care with reimbursements is important for the solvency of surgical practices and cost containment in the health care system. This study aimed to compare the day of surgery costs and incremental Medicare reimbursement for open reduction and internal fixation (ORIF) of intra-articular and extra-articular distal radius fractures (DRFs). METHODS: Using time data and supply costs at a single, large academic health center, we used a time-driven activity-based costing analysis to calculate the total day of surgery costs for ORIF of DRFs for the three current proc

Supracondylar humeral fractures in children.

Operative fixation is indicated for most type-II and III supracondylar humeral fractures in order to prevent malunion. Medial comminution is a subtle finding that, if treated nonoperatively, is likely to lead to unacceptable varus malunion. Angiography is not indicated for a pulseless limb, as it delays fracture reduction, which usually corrects the vascular problem. A high index of suspicion is necessary to avoid missing an impending compartment syndrome, especially when there is a concomitant forearm fracture or when there is a median nerve injury, which may mask symptoms of compartment synd

[Collaboration between the geriatrician and the surgeon: a major contribution for the aged patient in rehabilitation].

BACKGROUND: Subtrochanteric fractures are complex fractures requiring prudent reeducation. In elderly patients who also have other illnesses, reeducation of walking may be quite difficult. CASE REPORT: An 89-year-old woman was referred for physical therapy after osteosynthesis of a comminutive subtrochanteric fracture. Weight bearing was contraindicated for 3 months due to her "incapacity to perform reeducation exercises". The patient also had several serious co-morbidities which had to be integrated into the reeducation program. DISCUSSION: In cases of complex fractures, reeducation must ta

Fractures of the proximal part of the femur.

The orthopaedic surgeon has a multitude of internal fixation devices and techniques available for use in the treatment of subtrochanteric fractures of the proximal femur. The successful use of second-generation locking nails is technically demanding. Close attention to positioning of the patient, reduction of the fracture, placement of the guide-wire, and insertion of the nail and of the proximal and distal locking screws is mandatory. The newer, high-strength hip-screws allow good fixation of a fracture that extends into the piriformis fossa. If medial comminution is present, this technique i

Hook Plate Fixation of an Isolated Pisiform Fracture with a Small Proximal Fragment: A Case Report.

INTRODUCTION: The report describes hook plate fixation for an isolated, displaced pisiform fracture characterized by a small proximal fragment. CASE REPORT: A 64-year-old woman presented with ulnar-sided wrist pain after a fall. Radiography and computed tomography revealed intra-articular displacement with mild comminution. Open reduction and internal fixation using a 1.2-mm hook plate resulted in a stable anatomical reduction without the need for fragment excision. Postoperatively, the wrist was immobilized in a volar splint in neutral position for 3 weeks, after which progressive mobilizati

TRUMATCHTM Graft Cage-Long Bone as a solution for tibial bone defect in traumatic aseptic non-union: a case report.

BACKGROUND: Tibial fractures, with an annual incidence of 51.7 per 100.000 adults, often result in complications like non-union and infection, particularly in open fractures. Non-union rates after intramedullary nailing are 9.7%. Hypertrophic non-union stems from unstable fracture sites, while atrophic non-union arises from inadequate biological environments. Treatment involves rigid bone fixation for hypertrophic non-union and combining biological tissue supply with mechanical stability for atrophic non-unions. This case report outlines a complex tibial non-union post-high-impact injury, deta

Essential concepts in the treatment of common joint fractures: A narrative review.

Proximal fractures of the humerus are at high risk of osteonecrosis if the medial calcar is disrupted and the metaphyseal extension of the head fragment is <8mm. Risk factors for failure of osteosynthesis include: poor bone stock, varus dislocation, head dislocation and medial comminution. During surgical treatment of distal humerus fractures, one should aim to restore the arc and stabilize the columns. Several technical rules have been outlined by O´Driscoll and are mentioned in this article in detail. In fractures of the distal radius, the length, the joint surface, and joint angles (palmar

[Treatment of redislocated fragments of long bones using plaster cast wedging].

The authors wish to remind of the advantages of an old conservative therapeutic method used in redislocated long bone fractures--cast wedging, Gipskeilung. This mode of treatment makes it possible to restore axial as well as planar dislocation and it is used in corrections of redislocated fractures of the leg and is applied during the last 3 years also in the treatment of fractures of the forearm in children. In 1997-2000 this procedure was used for treatment at the Clinic of Paediatric Surgery, Orthopaedics and Traumatology, 33 children aged 3-16 years, with a redislocated fracture of the leg

Ischial apophyseal fracture in an abused infant.

We report a previously healthy 4-month-old who presented to the hospital with leg pain and swelling and no history of trauma. Radiographs demonstrated a comminuted left femur fracture. Given the concern for child abuse, skeletal survey was performed and revealed four vertebral compression deformities. Although abuse was suspected, the possibility of a lytic lesion associated with the femur fracture and multiple spinal abnormalities raised the possibility of an underlying process such as Langerhans cell histiocytosis. Subsequently 18F-NaF positron emission tomographic (PET) scintigraphy reveale

Proximal Interphalangeal Joint Arthroplasty for Fracture.

Proximal interphalangeal joint arthroplasties can be performed in the setting of acute comminuted fracture, chronic fracture presentations, and posttraumatic arthritis. These surgeries provide excellent pain relief and patient satisfaction but patients should be cautioned not to expect an improvement in motion postoperatively. Despite high rates of minor complications and radiographic loosening, these implants have good rates of long-term survival with most revisions occurring in the early postoperative period. They provide viable alternatives to arthrodesis, osteotomy and amputation in the ap

Tibial tuberosity fractures in adolescents.

Tibial tuberosity fracture is uncommon. We reviewed five patients with the injury, presenting over a two-year period. All of them were adolescent boys who sustained the injuries during sport. They were treated with open reduction and internal fixation using cancellous screws with additional tension band wiring for comminuted fragments. Results were excellent, with complete union of fracture site, full range of movement by three to five months and no evidence of complication on follow-up for thirty months.

Supracondylar fracture of the femur after GUEPAR total knee arthroplasty. A new treatment method.

A 71-year-old woman incurred an unstable, comminuted supracondylar fracture of the femur above the tip of a GUEPAR prosthesis. The prosthesis was not loose. Immediate open reduction and internal fixation were performed using a segment cut from an intramedullary rod. This provided axial alignment and functioned as an internal stent with stable fixation of the fracture. Supplementary fixation with a plate and screws, cerclage wires, and an autogenous bone graft led to early mobilization, solid bony union, and an excellent functional result.
