The prevalence of intracranial complications in pediatric frontal sinusitis.

OBJECTIVE: Intracranial extension of infection represents a serious complication of sinusitis but with no clearly documented prevalence. The frontal sinus with its unique anatomical characteristics, has been singled out as a catalyst for intracranial spread, but without solid evidence. Our objective was to determine the prevalence of intracranial complications in pediatric acute frontal sinusitis and to test the claimed association. METHODS: A retrospective chart review of all children (</=16 years of age) admitted over an 18 year period with sinusitis was undertaken in a tertiary children's

Augmentation rhinoplasty.

Augmentation rhinoplasty using a silicone implant is the most popular operation in Japan, but is not without several complications such as exposure of the implant and its deviation. We obtained good results in preventing these complications by a fixation at the hollow which is made by chiseling the frontal bone and by fibrous tissue which grows through the small holes of the implant tail.

A newborn infant with craniofacial dysmorphism and polysyndactyly (Greig's syndrome).

A newborn infant with craniofacial dysmorphism and polysndactyly (Greig's Syndrome) is reported. This syndrome is manifested by postaxial polydactyly of the fingers and preaxial polydactyly of the toes, with syndactyly. The craniofacial dysmorphism is characterized by frontal bossing and other variable manifestations. X-ray examination revealed markedly advanced bone age. The mode of inheritance appears to be autosomal dominant.

Rare Complications of Chronic Rhinosinusitis Associated With Nasal Polyps and Osseous Metaplasia.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is typically a benign condition; however, osseous metaplasia (OM), skull base erosion, and intracranial extension are rare complications, especially in young patients. Reporting such cases is important to increase awareness of unusual presentations of CRSwNP and guide management. We report the case of an 18-year-old male with a history of asthma and prior polypectomy who presented with worsening frontal headaches, subtle visual changes, and right eye proptosis. Imaging revealed extensive pansinusitis with complete frontal sinus opacification, b

Traumatic tension pneumocephalus--intracerebral pneumatocele: a case report.

An 18-year-old Chinese man, suffering from head injury in a motorcycle accident, with right traumatic frontal intracerebral hemorrhage and frontal bone fractures, underwent emergency craniotomy with evacuation of hematoma at a local out-patient clinic and three months later, he suffered from CSF rhinorrhea. Headache, nausea and vomiting developed a week prior to admission. Tension pneumocephalus was diagnosed by computed tomography (CT) scan and plain skull X-ray. The fistula tract of the ethmoid sinus was investigated by radioisotope albumin (RISA) cisternography. The patient subsequently und…

Occult Fronto-Orbital Fracture Presenting as Traumatic Pseudomeningocele in a 4-Month-Old Infant: Successful Surgical Management in a Resource-Limited Setting.

Traumatic fronto-orbital pseudomeningocele is an exceptionally rare complication of paediatric head trauma and may be overlooked because of subtle or delayed radiological findings. It results from cerebrospinal fluid (CSF) leakage through a dural defect associated with skull fractures. We report the case of a four-month-old male infant who presented with progressive right frontal and supraorbital swelling ten days after a minor fall. Cranial computed tomography revealed an occult fracture of the right frontal bone extending into the superior orbital wall with a communicating CSF collection cau…

Multi-layer approach to complex traumatic anterior skull base fracture repair: A case report.

BACKGROUND: Anterior skull base fractures represent a unique challenge for neurosurgical repair due to the potential for orbital injury and … or successful repair - namely, with the use of a temporally-based pericranial flap, split-thickness frontal bone graft, and autogenous abdominal fat graft. The patient was followed for nine months postoperatively, over which time she experienced no significant complications. CONCLUSION: The goal of successful anterior skull base repair involves creating a dur …

Twisted story of eye migration in flatfish.

Early molecular markers for flatfish metamorphosis and eye migration must be linked to the ethmoid region, the earliest part of the flatfish cranium to change, as well as chondral and dermal ossification processes. Serial sections, morphological landmarks, and stereology were used to determine where and when the remodeling of tissues and asymmetry occurs in the head region of metamorphosing Atlantic halibut, Hippoglossus hippoglossus. Not all parts of the head remodel or migrate, and those that do may be asynchronous. Normal metamorphosis limits the torsion of the Atlantic halibut head to the

Cranial reconstruction with computer-generated hard-tissue replacement patient-matched implants: indications, surgical technique, and long-term follow-up.

The aim of this clinical study was to evaluate the effectiveness and safety of using computer-generated alloplastic (hard-tissue replacement) implants for the reconstruction of large defects of the upper craniofacial region. Fourteen patients who had large (> 150 cm2) preexisting defects of the cranium or cranio-orbital region underwent surgical reconstruction. Preoperatively, a three-dimensional computed tomographic scan was obtained from which an anatomic model was fabricated. The defect in the model was then used to create an alloplastic (hard tissue-replacement polymer) implant for reconst

Distinguishing rhinitis and nasal neoplasia by radiography.

To compare the incidence of radiographic signs in dogs with rhinitis and primary nasal neoplasia and to assess the performance of observers for distinguishing these conditions, the nasal radiographs of 72 dogs with either rhinitis (n = 42) or primary nasal neoplasia (n = 30) were examined by two independent observers using custom-designed forms to record their interpretations. Rhinitis was associated with a higher incidence of focal or multifocal lesions, localised soft tissue opacities, lucent foci, and a lack of frontal sinus involvement. Neoplasia was associated with soft tissue opacities a

Craniofacial growth disturbance after skull base and associated suture synostoses in the newborn chacma baboon: a preliminary report.

The purpose of this study was to create craniofacial growth disturbances via synostoses of sutures of the cranial base, as well as of other various selected sutures of the anterior cranial and posterior facial structures. Twelve sutures, single or in combination, were obliterated by means of autogenous bone transplantation in twenty-five baby baboons (Papio ursinus). The data obtained from the sacrificed animals were qualitatively analyzed. The results indicated that synostosis of any of the selected sutures does lead to craniofacial growth disturbances. Furthermore, no specific synostosed sut

Subcranial facial bipartition osteotomy with glabellar reverse V-shaped and temporal approaches instead of the bicoronal approach.

Patients with Apert syndrome show hypertelorism and midfacial hypoplasia, and their features are significantly improved through facial bipartition surgery. In addition, because patients with Apert syndrome demonstrate cranial deformity as well as other deformities, they require multiple surgical interventions throughout their development. We present herein a girl with Apert syndrome for whom subcranial facial bipartition was performed. We could not use the coronal approach because she had a terribly cicatricial scalp and wide calvarial defects caused by previous cranial surgeries carried out a

Giant epidermoid involving 3 cranial bones.

We describe a patient with an incidental finding of an expansile osteolytic lesion centered in the sphenoid involving 3 cranial bones-frontal, temporal, and sphenoid-and invading the brain parenchyma and orbit. Biopsy demonstrated an epidermoid. The lesion was excised through frontotemporal craniotomy. Despite the benign nature of the tumor, the epidermoid had a destructive pattern of growth. Complete resection was needed to adequately address the inflammatory mass effect of the large lesion.

Le Fort III osteotomy to correct dish-face deformity resulting from facial trauma.

Untreated craniofacial fractures may result in a deformity similar to that which results from underdevelopment of the maxilla. Such a deformity can be corrected by osteotomy simulating Le Fort type III fracture lines. This operation necessitates careful pre-operative planning which incorporates a feasibility study with the combined use of dental study casts, cephalometric radiography and life-size photographic reproductions. A case history which describes the management of late complications of a malunited class III fracture is given.
