Congenital dislocation of the hip usually results from capsular stretching caused by fetal malposition and crouching late in the third trimester. Early recognition of hip dislocation or instability soon after birth permits prompt treatment. Ortolani's and Barlow's maneuvers, respectively, reduce into and displace from the acetabulum a femoral head that is insecurely contained therein. The diagnosis of CDH in the first month of life usually depends on these clinical components of the physical examination of the newborn, because similar device, in this age group can usually maintain the displace…

Congenital dislocation of the hip in a child over the age of 4 may be discovered as a late untreated displacement, presenting with subluxation or dislocation, or as a late treated displacement presenting as a subluxating or subluxated hip. Conservative treatment is inadequate in untreated dislocations. Surgical reduction is the only possible procedure, associated with shortening femoral. The procedure may be used up to the age of 7 in bilateral cases, and up to the age of 12 in unilateral cases. Subluxating hips are recognised on the anteroposterior radiograph by a break in Shenton's line. A d…

The hip joint presents a coaptive relationship between the head of the femur and the acetabulum. Developmental changes in either component affect the responsive growth of its companion. Normal pressure of the head of the femur in the acetabulum maintains a balance in the proliferation of triradiate and acetabular growth cartilage. Secondary deformities occur as a result of failure of head-acetabular relationships as in congenital dislocation of the hip and primary epiphyseodesis of the triradiate cartilage (TRC). Anatomically the proximal femur adapts to the growth of the primary epiphyses and…

The revision of a course of documents allow the authors to state the course of the femoral antetorsion and to individualize retrotersion's cases. The study has ruled out sequelae of congenital hip dislocation. An approach of the X-ray's orientation of the acetabulum is tempted to appreciate its part in the rotation's anomalies of the hip. At last, the surgical directions are stated.

Long-term results of 52 fractures of the hip in 51 children and adolescents are discussed. A distinction was made between fractures in children (aged 1 to 11 years) and those in adolescents (aged 12 to 18 years). Irrespective of the type of fracture it was found that immediate operative treatment leads to the best results. In children as well as in adolescents the risk of avascular necrosis, premature closure of the epiphysis and non-union is less following immediate operative treatment than following delayed operative treatment or conservative treatment. The prognosis of these fractures depen…

Hip injuries in children present a wide spectrum of problems. Frequently because of the severity of the trauma involved, other injuries may take precedence and may require modification of the usual approaches to treatment. However, certain precepts are essential to the successful treatment of these injuries and should be kept in mind regardless of the milieu in which they are found. In the child with a hip dislocation, the potential presence of an acetabular, femoral head, femoral shaft, patellar or tibial plateau fracture must always be considered. Specialized x-ray views are necessary for th…

We analyzed the results of transfers of the iliopsoas or external oblique muscles performed to augment the abductor power of the hips in 149 patients with flaccid paralysis of the hips. More than 800 supplemental procedures were performed on the lower extremities of these patients. Twenty-three of the thirty-eight hips that had been subluxated or dislocated became stable in the reduced position after the transfer operation. Every hip that had been stable before the transfer remained stable, and thirty-seven patients who had been brace-dependent became brace-free. We concluded that transfers of…

Little attention has been paid to the various causes preventing or blocking a concentric reduction following traumatic dislocation of the hip without apparent fracture. In a review of fifty-four dislocations, nine required open reduction after either a failed attempt at closed reduction or a non-concentric reduction. The factors preventing closed reduction included the femoral head buttonholed through the capsule and the piriformis muscle displaced across the acetabulum. A concentric anatomical reduction was prevented because of an inverted lumbus or an osteocartilaginous loose body in the ace…
