Open plate fixation versus nailing for humeral shaft fractures: a meta-analysis and systematic review of randomised clinical trials and observational studies.

PURPOSE: This meta-analysis compares open reduction and internal fixation with a plate (ORIF) versus nailing for humeral shaft fractures with regard to union, complications, general quality of life and shoulder/elbow function. METHODS: PubMed/Medline/Embase/CENTRAL/CINAHL was searched for observational studies and randomised clinical trials (RCT). Effect estimates were pooled across studies using random effects models. Results were presented as weighted odds ratio (OR) or risk difference (RD) with corresponding 95% confidence interval (95% CI). Subgroup analysis was performed stratified for s

Inadvertent, intraoperative, non- to minimally displaced periprosthetic humeral shaft fractures in RTSA do not affect the clinical and radiographic short-term outcome.

INTRODUCTION: Little information is available on the clinical and radiographic outcome of intraoperative, non- to minimally displaced humeral fractures that occur during implantation of a stemmed, reverse shoulder prosthesis but are only recognized on routine postoperative radiographs. The goal of this study is to report the clinical and radiographic outcome for this rarely reported fracture type. MATERIALS AND METHODS: 39 conservatively treated non- to minimally displaced intraoperative periprosthetic humeral fractures after stemmed RTSA were detected from our radiographic database between 1…

Surgical management of delayed Gartland type III supracondylar humeral fractures in children: A retrospective comparison of radial external fixator and crossed pinning.

Radial external fixator has been proposed to treat delayed irreducible Gartland type III supracondylar humeral fracture, and this study aims to compare its effects with crossed pinning in a retrospective fashion. Delayed supracondylar humeral fracture is defined as more than 72 hours after injury, 2 or more than 2 times failed attempts of closed reduction can be deemed as irreducible fracture.Between January 2010 and January 2017, patients of Gartland type III supracondylar fractures of the humerus receiving surgery were all selected and reviewed. Overall, 39 patients fitting the inclusion cri

Surgical treatment of complex proximal humeral fractures in elderly patients: a matched-pair analysis of angular-stable plating vs. reverse shoulder arthroplasty.

BACKGROUND: To date, there is a lack of consensus regarding the type of surgical treatment for complex proximal humeral fractures (PHFs) in elderly patients, especially between joint preservation and joint replacement techniques. MATERIALS AND METHODS: We matched 60 patients (aged 73.2 ± 6.4 years) with complex PHFs in terms of sex, age, and Charlson Comorbidity Index, who underwent locking-plate open reduction-internal fixation (ORIF) or reverse shoulder arthroplasty (RSA) (2011-2017). Postoperative range of motion and functional outcome were assessed using the American Shoulder and Elbow Su…

Axillary nerve lesions after open reduction and internal fixation of proximal humeral fractures through an extended lateral deltoid-split approach: electrophysiological findings.

BACKGROUND: Axillary nerve injuries after shoulder surgery are rare. In most studies, the frequency of injury is usually determined using clinical examinations, but results from intraoperative neuromonitoring studies have revealed higher than expected rates. Few studies have investigated this topic. Our aim was to determine the frequency of axillary nerve lesions after open reduction and internal fixation of proximal humeral fractures by using electrophysiological assessments and to provide a review of the relevant literature. METHODS: This was a retrospective cohort study of 76 consecutive p…

Total elbow arthroplasty for acute distal humeral fractures in patients over 65 years old - results of a multicenter study in 87 patients.

INTRODUCTION: Fractures of the distal humerus represent 5% of osteoporosis fragility fractures in subjects over the age of 60. Osteoporosis, comorbidities and intra-articular comminution make management of this entity difficult. HYPOTHESIS: The hypothesis was that total elbow arthroplasty could be a reliable treatment option in subjects over the age of 65 presenting with a fracture of the distal humerus. MATERIALS AND METHODS: Eight-seven patients (80 women and 7 men) mean age 79 years old (65-93) underwent total elbow arthroplasty for the treatment of an AO type A fracture in 9 cases, type

Medial support by fibula bone graft in angular stable plate fixation of proximal humeral fractures: an in vitro study with synthetic bone.

BACKGROUND: Failure to achieve stable fixation with medial support in proximal humeral fractures can result in varus malalignment and cut-through of the proximal screws. The purpose of this study was to investigate the influence of an intramedullary fibula bone graft on the biomechanical properties of proximal humeral fractures stabilized by angular stable plate fixation in a bone model under cyclic loading. METHODS: Two fixation techniques were tested in 20 composite analog humeri models. In group F- (n = 10), fractures were fixed by an anatomically formed locking plate system. In group F+ (…

The medial collapse sign is associated with cubitus varus after pediatric supracondylar humeral fracture fixation: a retrospective cohort study.

BACKGROUND: Cubitus varus deformity is a common complication following pediatric supracondylar humeral fractures and rarely remodels spontaneously. However, surgeons lack a reliable intraoperative radiographic sign that directly reflects inadequate medial column reduction. This study aimed to evaluate whether the medial collapse sign (MCS) on immediate postoperative anteroposterior radiographs is associated with postoperative cubitus varus. METHODS: A retrospective cohort study included pediatric patients who underwent surgical treatment for supracondylar humeral fractures consecutively betwe…

Temporary Cement Prosthesis as a Bridge After Control of Axillary Artery Hemorrhage in Proximal Humeral Fracture-Dislocation: A Case Report.

CASE: A 64-year-old woman underwent open reduction and internal fixation for four-part proximal humeral fracture-dislocation with palpable distal pulses. Upon dislocated head removal, life-threatening hemorrhage occurred from posterior circumflex humeral artery tear. Bleeding was controlled through packing, resuscitation, and open vascular repair. Owing to unavailable shoulder prosthesis and prolonged operative time, a temporary cement prosthesis with Steinmann pins was used. At the 3-year follow-up, partial function was restored with no loosening. CONCLUSION: This case highlights axillary br

Late diagnosis of SLC34A3-related hereditary hypophosphatemic osteomalacia following a low-energy proximal humeral 4-part fracture.

Hereditary hypophosphatemic rickets with hypercalciuria (HHRH) is a rare FGF23-independent renal phosphate-wasting disorder caused by biallelic variants in SLC34A3, typically diagnosed in childhood or early adulthood. Delayed recognition in elderly individuals is uncommon, and the clinical features leading to diagnosis in this population remain poorly characterized. We report a case of a 77-yr-old Japanese man with HHRH who remained undiagnosed until advanced age and was identified following a proximal humeral fracture sustained after a low-energy fall. The severity of the fracture, together w

Radiographic identification method of safe medial pinning zone and its clinical efficacy in pediatric supracondylar humeral fracture fixation.

OBJECTIVES: Preventing iatrogenic ulnar nerve injury remains a critical challenge during medial pinning in pediatric supracondylar humeral fractures (SCHFs). This study seeks to define a safe zone for medial pin insertion and evaluate its effectiveness in minimizing complications. METHODS: Three-dimensional reconstructions of radiographs from a healthy elbow joint were generated using E3d software to delineate a safe medial pinning zone. Pediatric patients with modified Gartland type III or IV SCHFs who underwent closed reduction and percutaneous crossed pin fixation were categorized into two…
