Open Reduction Internal fixation of Incarcerated Greater Tuberosity Avulsion Fracture: A Case Report.

INTRODUCTION: Proximal humerus fractures are common injuries of the upper extremity. These fractures are frequent in the elderly population due to low-energy trauma and in the younger patient secondary to high-energy trauma that is associated with shoulder dislocations. Proximal humerus fractures are frequently classified according to the Neer classification which is defined by the number of segments and the amount displacement required to define the fracture as displaced. Segments include the greater tuberosity (GT), the lesser tuberosity, the humeral head, and the humeral shaft. Fractures ar…

[Glenohumeral osteometry-scapulometry in anterior shoulder instabilities. Study of one of the static stabilizers through computer tomography].

We have carried out a scapulometric study, using CT-scan, of 98 shoulders: 36 with recurrent anterior shoulder dislocation (RAD), 37 stable contralateral shoulders (CSS) and 25 normal shoulders (NS). Six parameters were evaluated: Horizontal and Vertical glenohumeral index, glenoid tilt, anteversion angle of the scapula, glenoid angle and humeral retroversion. We found statistically significant differences between the RAD and CSS groups in the horizontal glenohumeral index. Both the RAD and CSS groups showed significant differences in comparison with the NS group in the horizontal glenohumeral

[Hemiarthroplasty for management of proximal humeral fractures].

PURPOSE OF THE STUDY: With the advent of angle-stable implant systems in surgical treatment of proximal humeral fractures, the number of indications to shoulder hemiarthroplasty decreased dramatically, because these modern implants provide certain fixation of osteoporotic bone fragments in elderly patients. MATERIAL AND METHODS: The authors report on their experience with shoulder replacement surgery in 29 patients, of which 26 underwent urgent surgery. The indications for acute hemiarthroplasty included humeral head fractures in which destruction of the articular surface exceeded 40 %, and f…

Reverse Total Shoulder Arthroplasty After Proximal Humerus Fracture Using a Novel FiberTape Cerclage Tuberosity Fixation Technique.

An 82-year-old woman presenting with an acute, 4-part proximal humerus fracture-dislocation after a ground-level fall was treated with reverse total shoulder arthroplasty using a novel FiberTape cerclage tuberosity fixation technique. Surgery was done within 24 hours of injury using a deltopectoral approach. The reconstruction technique incorporated a standard metaphyseal press-fit stem, autologous bone graft, and a modified Six-Tec-based suture tuberosity repair combined with a tension-band construct derived from a prophylactic humeral shaft FiberTape cerclage (Arthrex) anchored distal to fra

Glenoid wear after shoulder hemiarthroplasty: quantitative radiographic analysis.

Symptomatic glenoid arthrosis may limit the long-term success of shoulder hemiarthroplasty in patients who are young and functionally demand … d prior surgery. This study suggests that glenoid cartilage erosion can be expected routinely after humeral head replacement in young, active individuals, and that such wear may adversely affect function or necessitate conversion to total shoulder arthroplasty.

Clinical and Functional Outcomes of Proximal Humerus Internal Locking System Plate Fixation in Proximal Humerus Fractures: A Short-term Follow-up.

INTRODUCTION: Proximal humeral fractures are increasingly common due to both high-energy trauma in younger individuals and low-energy falls in the elderly. Surgical management is indicated for displaced and unstable fractures, with locking plates widely used. However, conventional open reduction techniques may compromise soft tissues and vascular supply. Minimally invasive plate osteosynthesis (MIPO) offers an alternative that preserves soft-tissue integrity while enabling stable fixation. OBJECTIVE: This study aimed to evaluate the clinical and radiological outcomes of proximal humeral fract

Classic versus congruent coracoid positioning during the Latarjet procedure: an in vitro biomechanical comparison.

PURPOSE: The purpose of this biomechanical study was to compare the classic Latarjet technique and congruent-arc modification with respect t … een techniques (P = .613). In abduction external rotation, there was significantly greater anterior humeral head translation after the congruent technique than after the classic (9.9 and 6.5 mm, respectively, P = .013). Rotational range of motion was significantly reduced after classic (-25.8°) and congruent (-22.2°) transfers as compared with the 30% defect ( …

Gonococcal osteomyelitis in a pediatric patient with disseminated gonococcal infection: Implications for antimicrobial management.

We report a case of a female teenager with gonococcal septic arthritis of the right shoulder that also caused osteomyelitis of the humeral head. Infection with Neisseria gonorrhoeae is a frequently diagnosed sexually transmitted infection in the sexually active teenage population and disseminated gonococcal infection (DGI) is the most common systemic manifestation of acute gonorrhea. DGI commonly involves acute arthritis, tenosynovitis and dermatitis with less common complications of endocarditis, hepatitis and meningitis. In contrast, osteomyelitis has only rarely been reported as a result of…

A case of bone involvement in hairy cell leukemia successfully treated with radiation therapy.

We report the case of a 63-year-old man with a clearly established diagnosis of hairy cell leukemia, treated with multiple lines of chemotherapy, who complained of localized pain in the left humerus. Radiological findings showed a dystrophic-blastic area within the humeral head. Fine-needle biopsy confirmed the hypothesis of bone involvement of hairy cell leukemia. The patient underwent radiotherapy at a dose of 25 Gy, obtaining a complete clinical response with resolution of pain and a partial recovery of the normal radiological structure of the humerus after 2 months. In addition to the case…

Treatment of internal rotation contracture of the shoulder in obstetric brachial plexus lesions by subscapular tendon lengthening and open reduction: early results and complications.

In this prospective study of 19 consecutive children, the operative treatment of internal rotation contracture of the shoulder in obstetric brachial plexus lesions by subscapular tendon lengthening and open reduction of the humeral head is evaluated. The average age of the children was 3.7 years and average follow-up was 20 months. Active shoulder function, as measured by the Mallet score, improved significantly in the dimensions of external rotation, hand-mouth movement and hand-neck movement. However, eight of the 19 children developed a severe, functionally disturbing external rotation cont…

Arthroscopic stapling for detached superior glenoid labrum.

Superior labral tears of the shoulder involve the biceps tendon and labrum complex which may be detached, displaced inferiorly, and interposed between the glenoid and the humeral head. We have treated ten young athletes with painful shoulders due to this lesion by arthroscopic stapling. Arthroscopy at the time of staple removal, after three to six months, showed that all the lesions had been stabilised. Clinical review at over 24 months showed an excellent or good result in 80%. The two relative failures were due in one to residual subacromial bursitis, and the other to multidirectional should…

Salvage of shoulder joint through simplification of wound management.

Infection is a common sequela of open dislocation of the shoulder joint. This typically leads to joint surface and humeral head necrosis, resulting in severe limitation of motion. Few cases of open and infected glenohumeral joints have been reported, and none had a good functional outcome with pain-free, near-full range of motion. Here we report the case of a 24-year-old man, who sustained a severe degloving injury of his left shoulder with an open glenohumeral joint, in a road traffic accident. We describe a treatment plan that salvaged near-full range of shoulder motion and resulted in pain-…

Spiral fracture of the radius: an unusual case of shoulder dystocia-associated morbidity.

BACKGROUND: The most common neonatal complications associated with shoulder dystocia include transient brachial plexus palsy, clavicular fracture, and humeral fracture. Fracture of the fetal radius has not been previously reported. CASE: We encountered a shoulder dystocia with the fetal head in the right occiput anterior position that necessitated the McRoberts maneuver, suprapubic pressure, the Wood and Rubin maneuvers, and extraction of the posterior fetal arm to effect delivery. The 4610-g infant experienced a spiral fracture of the right (anterior) radius and a fracture of the left (poste

Solitary paralysis of the triceps muscle due to trauma.

We report a rare solitary paralysis of the triceps muscle. The patient, an 18-year-old man who had had a work-related accident, sustained a severe contusion of the proximal posterolateral arm after being struck by a piece of lumber. This injury produced an extraordinary amount of soft-tissue swelling in the region of the trauma. After the accident it was noted that active flexion of the elbow was preserved, but active extension was absent. Three months after injury, surgical exploration of the radial nerve and its rami musculares to the three heads of the triceps muscle was undertaken. Dense a

Reverse vascularized bone graft of the lateral distal humerus for non-union of the radial neck fracture: anatomical study and case report.

A few treatment options for radial neck non-union have been reported, including radial head excision, radial head replacement, and internal fixation with a bone graft. We describe a new treatment for radial neck non-union using a reverse vascularized bone graft of the lateral distal humerus. In the anatomical study, the posterior radial collateral artery (PRCA) was dissected in eight fresh-frozen cadaver arms. The number of branches from the PRCA to the humerus was determined, and the distances from these branches to the lateral epicondyle of the humerus were measured. We then used this inform

Variation in stress distribution patterns across the radial head fovea in osteochondritis dissecans: predictive factors in radiographic findings.

BACKGROUND: Predictive factors for the development of osteoarthritis in adolescent osteochondritis dissecans (OCD) of the humeral capitellum remain unclear. The objectives of this study were to assess subchondral bone density in the radial head fovea of patients with OCD and to evaluate stress distribution in the radiocapitellar joint. The relationship between radiologic classification and stress distribution, according to multivariate ordinal regression analysis, was also investigated. METHODS: Computed tomography (CT) imaging data from 54 male patients with OCD (mean age, 13.1 years) were c
