Because neonates and very young children have little or no immunity to common streptococci and staphylococci, skin infections caused by these organisms must be regarded warily. Prompt and proper treatment may be critical in preventing progression of the lesions and averting systemic complications.

Onychomycoses represent the most frequently seen nail diseases and are the most difficult to treat of all skin mycoses. They are rare in chi … ls in chronic paronychia and onycholysis, and from nails in chronic mucocutaneous candidosis. Mixed infections by dermatophytes, molds, and/or yeasts are not uncommon. Probably, most fungi cannot infect a healthy nail organ, and only predisposing factors such as impaired blood circulation, peripheral neuropathy, diabetes mellitus, damage from repeated minor …

Human papillomavirus infection represents the most common mucocutaneous viral infection, and 3% to 5% of all patients have clinically evident warts. Human papillomavirus infections of the genital tract are one of the most common sexually transmitted viral infections in the United States. Data from STD clinics and private physicians' offices reveal that genital warts, one manifestation of genital HPV infection, have been diagnosed more frequently in recent years. With the use of a variety of diagnostic techniques, asymptomatic HPV infection has been identified in men and women and is probably m…

Because many Americans keep a wide variety of household pets, they are exposed to the many pet-associated diseases that are transmitted in various ways, including through bites, scratches, saliva, excreta, and respiration. This article briefly describes a number of the illnesses that may be acquired from household pets.

The first oral agents for treatment of mycoses included potassium iodide, griseofulvin and flucytosine. While each is still used in specific indications, the advent of ketoconazole in the late 1980s radically expanded the spectrum and efficacy of oral antifungals. Ketoconazole was the first drug sufficiently potent and benign to permit use for both superficial and deep fungal infections. Ketoconazole quickly proved highly effective in many systemic and cutaneous infections, but it was soon appreciated that high doses caused impairment of testosterone and ultimately cortisol synthesis. Dose-dep…

The mycoses are of increasing importance because of the AIDS epidemic. Oral and other forms of candidosis are nearly universal in AIDS patients. Cryptococcosis is the most common lethal mycosis, and may number tens of thousands of cases in AIDS worldwide annually. As the number of AIDS patients mounts, a rising tide of infections due to nearly all recognised fungal pathogens is occurring; examples include histoplasmosis, coccidioidomycosis and penicilliosis. These experiences have led to an investigation of new drugs which are compatible with the understanding that AIDS patients presently have…

Branhamella catarrhalis, formerly Neisseria catarrhalis, is a common bacteriological finding in the upper respiratory tract in infancy, and it ranks third on the list of bacterial causes of otitis media and sinusitis. In adults Branhamella catarrhalis may cause acute exacerbations of bronchitis and pneumonia, especially in patients with chronic lung disease. Branhamella catarrhalis is being recognized more frequently as a nosocomial pathogen and it may cause septicaemia in immunocompromised patients. Resistance to penicillin is prevalent and is due to production of beta-lactamases. The pathoge

Bacterial infections is one of the most important complications in the patients with pulmonary tuberculosis. We reported the causative microorganisms in these cases with special reference to various clinical features and presented the recommended treatment and prophylaxis against respiratory bacterial infections in the patients with pulmonary tuberculosis sequelae. In 1988 and 1989, 63 patients with tuberculosis sequela were demonstrated to have been infected with respiratory pathogenic bacteria by the quantitative sputum culture method (greater than or equal to 10(7)/ml) in Tokyo National Che…

It has been estimated that up to 25% of American women may experience acute dysuria yearly. Many physicians commonly equate dysuria with urinary tract infection and will treat empirically without adequate evaluation. This therapeutic trial of antibiotics represents undertreatment for many patients and inappropriate treatment for others. It is important that physicians understand that dysuria is only a symptom that can be produced by many different clinical entities.

Recurrent respiratory infections after the first years of life are not easily related to vascular rings as the cause of these infections. Therefore six cases of older children are presented in whom a vascular ring was the cause of their respiratory problems. None of them ever had stridor or swallowing problems in early infancy, and recurrent respiratory infections occurred later in life as a symptom of a vascular ring. Unfamiliarity with this association caused a delay in diagnosis and treatment in two patients and persistent lung damage in one child. Five of the 6 children recovered well afte…

Several important points regarding the treatment of urinary tract infections should be made. Single-dose and short-course antibiotic therapy is appropriate only for women with acute bacterial cystitis due to E. coli. Studies comparing single-dose to full-course therapy have not been sufficiently designed to draw valid statistical conclusions, and only TMP/SMX is recommended at this time. Recurrent UTI in women is almost always due to reinfection, which is best managed by prophylactic antibiotics. Acute bronchitis and acute exacerbations of chronic bronchitis are often due to viral infections, …

Streptococcus bovis infection was diagnosed in pigeons from 20 lofts submitted for post mortem investigation. Clinical signs were very variable and ranged from hyperacute death to chronic lameness with arthritis. Lesions were generally unspecific except for single cases of muscle necrosis with purulent myositis. Histological examinations demonstrated the septicaemic nature of the infections. Intravenous inoculations of S. bovis resulted in prostration, long lasting loss of weight and polyuria. Microscopically purulent hepatitis and nephritis were the most characteristic lesions. After oral ino…

Evaluation of 302 infertile married couples has identified Mycoplasma infection in 106 (35.5%) of women and 96 (31.8%) of men. The infection was associated primarily with cervical and endometrial abnormalities in the women and sperm abnormalities in the men. Tetracycline treatment resulted in clinical improvement both in the women and men. This study suggests Mycoplasma infection to be an etiological factor in infertile marriage.

Since 1986, 2,279 pregnant women were screened for anti Parvovirus B19 IgG and IgM antibodies. Of them 41% were seronegative, 54% had specific IgG but no IgM antibodies, indicating an infection in earlier life. Acute infection could be demonstrated by IgM antibody detection in 114 pregnant women (32% in the first, 54% in the second and 14% in the third trimester). Hydrops fetalis occurred in 10 of the 114 pregnancies (8.7%). In five of these cases intrauterine exchange transfusion was done and until now four healthy babies were born. Three fetuses died and one pregnancy was terminated, and two

For hepatitis B, universal screening of pregnant women for HBsAg along with the combined use of hepatitis B vaccine and HBIG in infants of infected mothers appears to be the best way to detect and prevent vertically transmitted infection. Cytomegalovirus is the most common congenital viral infection, but the lack of a good screening test precludes its accurate and rapid diagnosis. For HPV-B19, larger and more controlled studies are needed to confirm the safety and effectiveness of fetal blood transfusions in the management of hydrops fetalis caused by this infection. Finally, the fact that mos

Infections caused by small, free-living amebas are still unfamiliar to many clinicians, pathologists, and laboratorians. As of 31 July 1989, more than 140 cases of primary amebic meningoencephalitis caused by Naegleria fowleri and more than 40 cases of granulomatous amebic encephalitis caused by Acanthamoeba species (including two cases in patients with AIDS) and possibly by other free-living amebas had occurred worldwide. The recent increase in acanthamoeba keratitis (more than 200 cases), especially in contact lens wearers, has generated new interest in this group of amebas. Effective treatm…

The value of magnetic resonance (MR) imaging in evaluating granulomatous spinal infection was retrospectively assessed in 81 patients with proved disease; 27 were reexamined after administration of gadolinium diethylenetriaminepentaacetic acid, and 25 underwent follow-up studies. Blinded interpretations were correlated with clinical, microbiologic, and surgical findings. MR imaging enabled prediction of the presence of neurologic complications in 93% of patients and diagnosis of the type of infection in 94%, and correlated well with surgical findings in 24 of 27 patients.
