Explanatory models of epilepsy: key deterrents to seeking biomedical care and effective epilepsy management in Uganda.

BACKGROUND: The Sub-Saharan region has one of the largest populations with misconceptions about the etiology, manifestation, and management of epilepsy. Consequently, the majority of people living with epilepsy (PWE) receive unorthodox interventions that contribute to persistent uncontrolled seizures and poor quality of life. This study aimed to explore the prevailing explanatory models of epilepsy and their role in determining health-seeking behavior. METHODS: We utilized a qualitative, phenomenological design, with a sample of 128 participants, comprising of 19 Adolescents with Epilepsy (AW

Long-term care insurance and accessibility of home and community-based services for older adults: evidence from China.

With population aging, the establishment of universal long-term care insurance (LTCI) has emerged as a critical policy issue. This paper examines the effects of China's LTCI pilots on the physical accessibility of home and community-based services (HCBS) and specific services for older adults. Using three-wave panel data from the Chinese Longitudinal Healthy Longevity Survey (CLHLS), we analyze the rollout of LTCI pilots across different cities from 2014 to 2021, employing a time-varying difference-in-differences (DID) approach. Our findings indicate that LTCI significantly improves access to

Perspectives on Integrating Biological Assessments to Address the Health Effects of Childhood Adversities.

A majority of adults in the United States report a range of stressful and potentially traumatic childhood experiences (e.g., physical or sexual abuse, witnessing violence, neglect). Such adversities are associated with a range of mental (e.g., anxiety, mood, and behavioral difficulties) and physical (e.g., cardiovascular illnesses, diabetes, asthma) health problems. Increasingly, precision medicine approaches seek to prevent and treat such multifinal downstream health problems by identifying common etiological pathways (e.g., inflammation and immune pathways) and candidate biomarkers to target

Development of a brief telehealth-delivered counseling intervention to reduce suicide risk and improve HIV care engagement in Kilimanjaro, Tanzania.

Death by suicide is common among people living with HIV (PLWH) in Tanzania, and is strongly influenced by HIV stigma, lack of social support, and HIV-associated stress. Screening and intervention strategies for reducing suicidality, however, are not incorporated into standard HIV care. This study describes the development of a brief, nurse-led telehealth counseling intervention to address suicidal IDeation through HIV Education and counseling, advancing treatment Adherence, and reducing Stigma for renewed Hope (IDEAS for Hope) among PLWH in Tanzania. The three intervention sessions are based o

Mental Health Literacy about Personality Disorders: A Multicultural Study.

Mental health literacy (MHL) refers to lay people's knowledge and beliefs about the diagnosis and treatment of mental illness. The current study aimed at investigating MHL regarding personality disorders (PDs) multiculturally, comparing Turkish and Italian populations. In total, 262 participants responded to an online vignette identification task that required them to label the PDs of seven hypothetical subjects and rate various dimensions of their disorders. Narcissistic (25%), obsessive-compulsive (13%), and paranoid (12%) PDs were the most correctly labeled, while the average accuracy value

Iatrogenic triggers for anorexia nervosa and bulimia nervosa: A WHO safety database disproportionality analysis.

Eating disorders, characterized by abnormal eating, weight control behaviors or both include anorexia nervosa (AN) and bulimia nervosa (BN). We investigated their potential iatrogenic triggers, using real-world data from the WHO safety database (VigiBase®). VigiBase® was queried for all AN and BN reports. The reports were classified as `pediatric' or `adult' according to age. Disproportionality analyses relied on the Information Component (IC), in which a 95% confidence interval lower-end positivity was required to suspect a signal. Our queries yielded 309 AN and 499 BN reports. Isotretinoin w

Health professionals' attitudes toward medications for opioid use disorder.

Background: Research has shown that medications, especially opioid agonist treatments, are an effective way to treat opioid use disorder (OUD); however, negative attitudes held by health professionals contribute to their underutilization. Methods: A 23-year review of studies that examined health professionals' attitudes toward medications for OUD (MOUD) was conducted to describe the current state of knowledge and to inform future research and interventions. Results: Studies examined attitudes toward the use of methadone, buprenorphine, and naltrexone among various types of health professionals

The Projection From Ventral CA1, Not Prefrontal Cortex, to Nucleus Accumbens Core Mediates Recent Memory Retrieval of Cocaine-Conditioned Place Preference.

Drug-paired cues inducing memory retrieval by expressing drug-seeking behaviors present a major challenge to drug abstinence. How neural circuits coordinate for drug memory retrieval remains unclear. Here, we report that exposure of the training chamber where cocaine-conditioned place preference (CPP) was performed increased neuronal activity in the core of nucleus accumbens (AcbC), ventral CA1 (vCA1), and medial prefrontal cortex (mPFC), as shown by elevated pERK and c-Fos levels. Chemogenetic inhibition of neuronal activity in the vCA1 and AcbC, but not mPFC, reduced the time spent in the co

Focused antenatal care utilization and associated factors in Debre Tabor Town, northwest Ethiopia, 2017.

OBJECTIVE: Attending antenatal care helps to reduce the occurrence of maternal morbidity and mortality by providing chances for health promotion and information about danger signs, birth preparedness and where to seek care for pregnancy complications. Therefore identifying factors affecting the utilization of focused ANC service is of supreme importance. RESULTS: A total of 317 mothers who had a history of antenatal care for their last birth during the previous 6 months were included in the study from which 112 (35.3%, 95% CI 30.6, 40.4) of mothers attended focused antenatal care services. Towards Standardizing the Alcoholism Evaluation Of Potential Liver Transplant Recipients.

AIMS: For teams around the world, alcoholic liver disease patients comprise the largest, and clinically most controversial, group applying for liver transplant. And yet evaluation decisions for them remain highly variable by locale. METHODS: Targeting standardized assessment, we provide guidelines on what information the transplant team should seek, from what sources, and how best to make use of it. This report focuses on 'what to do and how to do it' in providing appropriate assessments for this complex patient group. RESULTS: Proper evaluation includes (a) taking the clinical history from

Likelihood of Attending Treatment for Anxiety Among Veteran Primary Care Patients: Patient Preferences for Treatment Attributes.

Anxiety is common, but under-treated, in primary care. Behavioral health providers embedded in primary care can help address this treatment gap. Guidance on anxiety treatment preferences would help inform tailoring of clinical practice and new interventions to be more patient-centered and increase treatment engagement. We surveyed 144 non-treatment seeking Veteran primary care patients (82.6 % male, 85.4 % White, age M = 59.8 years, SD = 13.9) reporting current anxiety symptoms (M = 13.87, SD = 3.66, on the Generalized Anxiety Disorder-7 Questionnaire) on their likelihood of attending anxiety

Pain management in heroin and cocaine users.

Drug addicts often seek medical help for pain. Numerous fears and beliefs may hinder the recognition, evaluation, and management of pain in addicts. Nevertheless, the same fundamental principles apply to these patients as to other patients in terms of pain evaluation, analgesic selection, and dosage adjustment. Clarity of the medical prescription is crucial. Specific points that require attention in addicts include the effects of the abused drugs on the nociceptive system, the nature and amount of abused drugs, concomitant nondrug addictions, co-morbidities, and the nature of the pain symptoms

Environment and obesity in the National Children's Study.

OBJECTIVE: In this review we describe the approach taken by the National Children's Study (NCS), a 21-year prospective study of 100,000 American children, to understanding the role of environmental factors in the development of obesity. DATA SOURCES AND EXTRACTION: We review the literature with regard to the two core hypotheses in the NCS that relate to environmental origins of obesity and describe strategies that will be used to test each hypothesis. DATA SYNTHESIS: Although it is clear that obesity in an individual results from an imbalance between energy intake and expenditure, control of

The use of the case-crossover design in studying illicit drug use.

The case-crossover design was developed to study time-varying exposures that cause transient excess risk of acute health events. It is a variant of case-control and subject-as-own-control research designs, involving use of information about exposure history of each case to estimate the transient effect. This kind of self-control design can help to reduce sampling bias otherwise introduced in the selection of controls, as well as confounding bias that might be derived from enduring individual characteristics, especially personality traits and other long-standing inherited or acquired vulnerabil

"Spiritlinking": a key to dynamic change. Fostering chaos enables leaders to form the networks and unity needed to fulfill their mission.

To contend with the anxiety and chaos inherent in rapid change, leaders can turn to ¿spiritlinking,¿ the deliberate and untiring act of building the high-energy networks where new ideas are born and new ways of delivering service take form. Spiritlinking leaders are able to envision and promote the paradox of disequilibrium. They seek truth, pushing the organization into a condition of imbalance where staid settledness gives way to new paradigms founded on constant transformation. Spiritlinking leaders are not bound by organizational charts. They have discovered that the greater the participat

Sleep, Health Care-Seeking Behaviors, and Perceptions Associated With the Use of Sleep Wearables in Canada: Results From a Nationally Representative Survey.

BACKGROUND: The popularity of sleep-tracking wearables has surged worldwide. Yet, there are significant gaps in understanding the real-life implications of this phenomenon. While wearables may offer insights about sleep and promote sleep health awareness, evidence remains mixed on whether they lead to improved sleep outcomes or fuel sleep anxiety. OBJECTIVE: This study aims to (1) determine the prevalence and sociodemographic predictors of using sleep wearables in Canada, (2) evaluate the perceived effects of wearable use on sleep and stress, (3) compare sleep and health care-seeking behavior

Behavior Change Intervention for Smokeless Tobacco Cessation Delivered Through Dentists in Dental Settings: A Pragmatic Pilot Trial.

INTRODUCTION: Evidence on smokeless tobacco (ST) cessation interventions is scarce. The South Asian (SA) region that shares more than 90% of the burden of ST use is grossly underrepresented in research on ST cessation. This study aimed to assess the feasibility of delivering and investigating a behavioral support intervention for ST cessation in dental settings in Pakistan. METHODS: A multicenter, pilot, two-armed parallel-group, individually randomized control trial, with a 1:1 allocation ratio, was conducted at two dental hospitals. Eligibility criteria included being an ST user seeking den

Contemporary epidemiology of metabolic dysfunction-associated steatotic liver disease.

INTRODUCTION: Metabolic dysfunction-associated steatotic liver disease (MASLD) is recognized as the most prevalent liver disease worldwide. Globally, it has impacted millions of individuals. With the recent change in terminology, MASLD is no longer a diagnosis of exclusion but rather a criterion-based diagnosis anchored around metabolic dysfunction. In addition to progressive liver fibrosis leading to cirrhosis, MASLD has been linked to extrahepatic complications, which include cardiovascular disease, non-liver-associated malignancies, and other adverse metabolic outcomes. Importantly, disease

Intersex Pretenders.

False claims of having an intersex condition have been observed in print, video, Internet media, and in live presentations. Claims of being intersexed in publicly accessible media were examined and evidence that they were false was considered sufficiently conclusive in 37 cases. Falsity was most often detected due to medical implausibility and/or inconsistency, but sometimes also using information from third-party or published sources. The majority, 26/37, of cases were natal males; 11/37 were natal females. Almost all (34/37) were transgendered, living, or aspiring to live, in their non-natal
