Decision-making capacity for research participation among addicted people: a cross-sectional study.

BACKGROUND: Informed consent is a key element of ethical clinical research. Addicted population may be at risk for impaired consent capacity. However, very little research has focused on their comprehension of consent forms. The aim of this study is to assess the capacity of addicted individuals to provide consent to research. METHODS: 53 subjects with DSM-5 diagnoses of a Substance Use Disorder (SUD) and 50 non psychiatric comparison subjects (NPCs) participated in the survey from December 2014 to March 2015. This cross-sectional study was carried out at a community-based Outpatient Treatmen…

Adolescent medical providers' willingness to recommend genetic susceptibility testing for nicotine addiction and lung cancer risk to adolescents.

OBJECTIVE: To examine the influences of disease, lifestyle, and other factors on adolescent medical providers' willingness to recommend genetic susceptibility testing (GST). METHOD: Providers attending a national conference completed a self-report survey (n = 232) about their willingness to recommend hypothetical GSTs, differentiated by disease (nicotine addiction/lung cancer), patient lifestyle (nonsmoker/smoker), and other contextual factors. RESULTS: Compared to recommending GST unconditionally, providers were more willing to recommend GST with parental/patient consent/assent, and in the

What participants understand about a maternal serum alpha-fetoprotein screening program.

We investigated the knowledge of pregnant women participating in a maternal serum alphafetoprotein (MSAFP) screening program for the detection of neural tube defects (NTDs) in the fetus. Women participating in the screening program scored higher on two knowledge tests than a comparison group of pregnant women who were not offered screening. However, there were substantial gaps in the knowledge base of women in the program, as measured by one of the tests. Women did not misinterpret a negative test result to mean that the test had identified a potential problem with the fetus; instead, there is

Transient anosmia during acute phase electroconvulsive therapy: a report of 2 cases.

Treatment-resistant psychiatric illness poses a clinical challenge to psychiatrists in both inpatient and outpatient settings. Although electroconvulsive therapy is a relatively safe option for the treatment of these patients, initiation of this treatment requires a conscientious discussion of the risks and benefits of the procedure to obtain informed consent. The 2 cases presented here describe transient anosmia, a seldom reported but potentially significant adverse effect of electroconvulsive therapy.

[Right pneumonectomy for typical bronchial carcinoid in a 19-year-old man].

A 19-year-old man visited our hospital with a diagnosis of carcinoid arising from the right intermediate bronchus. Clinical examination revealed T3N1M0 lung cancer, so right pneumonectomy with mediastinal lymph node dissection was performed after obtaining informed consent from the patient and his parents. Final pathological diagnosis was T2N2M0 typical carcinoid. Typical bronchial carcinoid with lymph node metastasis is very rare in patients under 20 years old.

Participation of persons living with dementia in research: A means to address epistemic injustice.

Epistemic injustice refers to wronging or mistreating individuals in terms of their capacity as knowers, based on prejudices or negative att … eepers, their attitudes and perceptions are highlighted. In terms of ethical issues, the concept of informed consent needs to be elaborated. If the research is not expected to harm participants and may contribute to improving the lives of those with dementia, the perspective should be shifted from informed consent to ongoing consent assessment. While acknowledging …

Providing Trauma Informed Care During Anorectal Evaluation.

PURPOSE OF REVIEW: Patients with a history of gastrointestinal (GI) conditions report high rates of psychological trauma. This review discusses the impact of previous trauma, as well as interactions with the medical system, on a patient's physical and mental health. Trauma-informed strategies for improving patient care during gastroenterology procedures are provided. RECENT FINDINGS: History of trauma increases risk of developing GI conditions and re-traumatization during sensitive anorectal procedures (i.e., anorectal manometry, balloon expulsion testing). Trauma-informed strategies include

"I would much rather hear the truth so I can be prepared": Virtual peer support, health access, and information-seeking after hysteroscopy referral.

BackgroundDeficiencies in patient information undermine informed consent and patient safety in gynaecological care. When clinical communication fails, women turn to digital peer communities, directly affecting health access and decision-making. Hysteroscopy procedures are at heightened risk of this failure: pre-procedural information is routinely unstandardised, and anxiety affects up to 80% of patients, impairing attendance and completion.ObjectivesThis study investigated women's information needs following hysteroscopy referral, using naturally occurring online data to identify behavioural t…

An aid to the explanation of surgical risks and complications: the International Spinal Surgery Information Sheet.

STUDY DESIGN: Presentation and analysis of a patient information sheet. OBJECTIVE: To produce an evidence-based information sheet that will serve as an aide-memoire to the process of taking informed consent prior to spinal surgery. SUMMARY OF BACKGROUND DATA: Consent for a surgical intervention is the end of a process of discussion between the surgeon and the patient. It is essential that the patient has been provided with sufficient information to make an informed judgment as to whether the benefits of a proposed procedure will outweigh its risks. METHODS: We searched MEDLINE, the Cochrane…

Design an Intervention Study.

Randomized controlled trials are commonly designed to compare the effectiveness of treatments in rheumatoid arthritis (RA). In a clinical trial (intervention study), researchers apply interventions or preventive services to patients and examine outcomes. Clinical trial design consists of the following categories: choice of intervention, selection of patients, informed consent, baseline measurement, bank specimens, randomized allocation and blinding, and outcome measurements. Here, we discuss the design of clinical trials for RA.

[Risk management in brachial plexus block].

Peripheral nerve block has many advantages in surgical anesthesia with or without general anesthesia; postoperative analgesia, faster postoperative rehabilitation, and chronic pain management. However, serious adverse complications after peripheral nerve block can happen. Therefore, anesthetists should obtain full informed consent for possible complications, and require scrupulous attention to this procedure. This review focuses on complications of brachial plexus block because it is the most popular peripheral nerve block.

Sequelae of endoscopic sympathetic block.

Endoscopic sympathetic block as a treatment for primary hyperhidrosis is associated with certain sequelae. The reported occurrence of side effects still varies in the literature. As the majority of patients describe sequelae after sympathetic surgery, the frequency and importance of these persisting changes are still underestimated. Patient's informed consent should include and define side effects like gustatory sweating, olfactory sweating and bradycardia as likely, and compensatory sweating as obligatory.

Beyond Bleeding, Infection, Damage to Surrounding Structures: Challenges and Opportunities When Communicating about Surgical Risks.

Informed consent and shared decision-making are central tenets of surgical consultations and the decision to proceed, or not, with surgery. These standard practices require communication with patients about the risks and benefits of the procedures they are considering. Yet communicating about risk is not straightforward. Limited health literacy, cognitive pitfalls, and the emotional experience of illness or a new diagnosis can all impact how patients interpret the information they receive. Beyond the legal obligations to ensure patients are informed, communicating effectively about risk can al…

Ethical principles and parental choice: treatment options for neonates with hypoplastic left heart syndrome.

Nurses caring for children with congenital heart disease face unique challenges, especially when caring for neonates diagnosed with hypoplastic left heart syndrome (HLHS). The treatment options for these neonates present difficult choices for the child's decision makers and are not without significant life-altering consequences. In order to assist in the decision-making process, nurses as patient and family advocates should acknowledge the unique role they play in the informed consent process, while simultaneously identifying specific ethical principles that are components of this process. By …

Ethics as if Your Life Depended on It.

PurposeThis Commentary examines whether current ethics approval systems for medical research, grounded in the Belmont Report's principles of respect for persons, beneficence, and justice, paradoxically result in preventable deaths by delaying patient access to experimental therapies and proposes a framework for democratizing ethics oversight while maintaining safety.FindingsPeer-reviewed evidence demonstrates that ethics delays impose mortality costs that vastly exceed the harms they prevent, particularly for terminal illnesses. Bayesian decision analysis confirms current statistical threshold
