INTRODUCTION: Low back pain (LBP) is a leading cause of disability and health care costs worldwide. Despite its prevalence, many patients fail to respond to surgical interventions and often face opioid dependency. Nonsurgical spinal decompression (NSSD) is a conservative treatment that may alleviate LBP by decompressing intervertebral discs, but it remains underutilized as a result of limited evidence and lack of insurance coverage.
METHODS: This retrospective chart review aimed to evaluate the effectiveness of NSSD and LBP outcomes, including pain intensity, neurological function, and activi

BACKGROUND AND OBJECTIVES: If nonoperative therapy fails, a unilateral cervical radiculopathy has traditionally been treated with anterior cervical diskectomy and fusion or posterior cervical foraminotomy/diskectomy. More recently, full-endoscopic posterior cervical foraminotomy/diskectomy (FEPCF) has gained popularity. In a cohort of patients with isolated, unilateral cervical radiculopathy, we sought to conduct a preliminary, pilot study to compare the total cost of surgical care between open and endoscopic posterior foraminotomy/diskectomy.
METHODS: A single-center, retrospective, cohort s

STUDY DESIGN: Retrospective cohort study.
OBJECTIVE: To investigate postoperative medical and surgical outcomes of 3-level cervical disk arthroplasty (CDA) in comparison to 3-level anterior cervical discectomy and fusion (ACDF).
SUMMARY OF BACKGROUND DATA: Cervical disk arthroplasty has emerged as a noninferior alternative to anterior cervical discectomy and fusion. However, few studies investigated the clinical outcomes of 3-level CDA. At present, CDAs beyond 2-level are performed as off-labeled procedures across the United States and can be deemed experimental by insurance companies.

OBJECTIVE: Shunt-dependent hydrocephalus is a major complication of aneurysmal subarachnoid hemorrhage (SAH). Despite this, the factors influencing shunt dependency and shunt failure remain unclear. Therefore, the aim of this study was to estimate shunt dependency and failure rates and determine the contributing factors in patients with aneurysmal SAH using the Korean National Health Insurance database over a 13-year period.
METHODS: Patients with aneurysmal SAH aged > 18 years who underwent surgical procedures were included. Using the shunt surgery prescription data, the shunt-dependent hydr

Sleep disturbance is prevalent in patients with degenerative spinal disease, and recent studies have reported that surgical treatment is more effective for improving sleep quality than conservative treatment. We aimed to investigate the perioperative changes of sleep problems in patients who underwent surgical treatment for degenerative spinal disease with a concurrent sleep disorder, and presented them according to various clinical profiles possibly associated with sleep disturbance. In addition, we identified factors associated with poor sleep improvement after surgery. This study used data

OBJECTIVES: Disparities in traumatic brain injury outcomes for ethnic minorities and the uninsured have previously been demonstrated; however, outcomes in undocumented immigrants have not been examined. We wanted to determine whether ethnicity, insurance, and documentation status served as risk factors for disparities in traumatic brain injury outcomes between undocumented immigrants and documented residents.
DESIGN: Retrospective study.
SETTING: Patients diagnosed with traumatic brain injury admitted to the surgical/trauma ICU at a level 1 trauma center serving a large immigrant population

PURPOSE: To determine whether the risk of adverse events and readmission after non-arthroplasty shoulder surgery is influenced by the outpatient setting of surgical care and to identify risk factors associated with these adverse events.
METHODS: The Humana Claims Database was queried for all patients undergoing arthroscopic shoulder surgery and related open procedures in the hospital-based outpatient department (HOPD) or ambulatory surgical center (ASC) setting, using the PearlDiver supercomputer. Arthroplasty procedures were excluded because they carry a risk profile different from that of o

BACKGROUND: Currently, there seems to be a paradigm change in the surgical treatment of proximal humeral fractures in patients aged 65 years and older, with a considerable increase in the use of reverse total shoulder arthroplasty (RTSA) compared with angular stable internal fixation (locking plate fixation). However, even among shoulder specialists there is controversy regarding the best treatment strategy.
QUESTIONS/PURPOSES: To evaluate for (1) a greater risk of in-hospital major adverse events, (2) a greater risk for in-hospital surgical complications, and (3) a greater risk of 30-day mor

BACKGROUND CONTEXT: Cauda equina syndrome (CES) occurs due to compression of the lumbar and sacral nerve roots and is considered a surgical emergency. Although the condition is relatively rare, the associated morbidity can be devastating to patients. While substantial research has been conducted on the timing of treatment, the literature regarding long-term rates of bladder dysfunction in CES patients is scarce.
PURPOSE: The aim of this study was to identify long-term rates of bladder dysfunction in CES patients and to compare those rates to non-CES patients who underwent similar spinal decom

BACKGROUND CONTEXT: Cervical spondylotic myelopathy (CSM) is a chronic spinal cord disease and can lead to progressive or stepwise neurologic decline. Several factors may influence this process, including extent of spinal cord compression, duration of symptoms, and medical comorbidities. Diabetes is a systemic disease that can impact multiple organ systems, including the central and peripheral nervous systems. There has been little information regarding the effect of diabetes on patients with coexistent CSM.
PURPOSE: To provide empirical data regarding the effect of diabetes on treatment outc

PURPOSE OF REVIEW: Structural racism is increasingly recognized as a fundamental driver of racial and ethnic health disparities, yet it remains insufficiently addressed in research. This review examines persistent disparities in urologic outcomes across malignant and benign conditions and highlights the role of upstream inequities and policies that shape care.
RECENT FINDINGS: The literature links structural racism to inequities across several domains, including screening policy and guideline implementation, referral patterns, subspecialty access, insurance design, and neighborhood-level fact

The COVID-19 pandemic disrupted health care utilization worldwide, followed by uneven recovery patterns. South Korea also experienced this recovery disruption due to the nationwide medical strike in 2024. Especially, this medical strike raised concerns about the vulnerability and instability of the national health care system amid supply-side shocks during the post-crisis recovery. Its impacts and aftermaths need to be analyzed and evaluated for the latter response. In this article, we investigated changes in health care utilization and costs using National Health Insurance claims data across

INTRODUCTION AND HYPOTHESIS: This study evaluated the reoperation rate and risk factors among women who underwent midurethral sling (MUS) insertion for stress urinary incontinence (SUI). Reoperation was defined as an additional incontinence procedure due to insufficient benefit from the initial MUS, excluding surgeries for mesh-related complications or device removal.
METHODS: We analyzed 221,082 women aged ≥ 20 years who received MUS between 2009 and 2015 in South Korea. Data from the National Health Insurance Service were used to determine reoperation rates, and Cox regression identified in

Posterior urethral valves (PUV) are congenital anomalies characterized by the persistence of mucosal folds in the urethra, leading to various degrees of obstruction. They are the most common cause of lower urinary tract obstruction in fetuses and children, with a severe prognosis, as one-third of affected children develop end-stage renal disease before adulthood. The French National Diagnostic and Care Protocol (NDCP) aim to provide healthcare professionals with guidelines for the optimal diagnostic and therapeutic management of PUV from the fetal stage to adolescence. The guidelines emphasize

Given the increasing trend of cholecystectomy, it is imperative to reassess surgical and surveillance strategies in consideration of the potential long-term risks for digestive tract cancers. The objective of this study was to assess the risk of gastrointestinal (GI) and hepato-biliary-pancreatic (HBP) cancer incidence after cholecystectomy. The data for this cohort study was obtained from the National Health Insurance Service database in Korea. 715,872 patients who underwent cholecystectomy between 2004 and 2020 were compared to 1,431,728 individuals who did not underwent cholecystectomy afte

Using Demographic and Health Survey data (2015-16) from the state of Andhra Pradesh, we estimate the differential probability of hysterectomy (removal of uterus) for women (aged 15-49 years) covered under publicly funded health insurance (PFHI) schemes relative to those not covered. To reduce the extent of selection bias into treatment assignment (PFHI coverage) we use matching methods, propensity score matching, and coarsened exact matching, achieving a comparable treatment and control group. We find that PFHI coverage increases the probability of undergoing a hysterectomy by 7-11 percentage
