Vanek jejunal tumor, a rare cause of anemic syndrome. Review of the literature.

Inflammatory fibroid polyps (IPF), also called Vanek tumors, are rare, benign lesions of mesenchymal origin. They can affect both sexes with a majority incidence between 50-70 years. We present the case of a 53-year-old woman with an outpatient clinic due to severe iron deficiency anemia, gastroscopy and colonoscopy without relevant findings. A study with capsule endoscopy identify an ulcerated polypoid mass, which caused a slight stenosis of the lumen, allowing the passage of the capsule endoscopy compatible with Vanek's tumor.

Extraluminal inflammatory fibroid polyp mimicking as wandering fibroid/GIST.

Vanek's tumour, also called an inflammatory fibroid polyp (IFP), is a rare, non-cancerous growth from the submucosa of the gastrointestinal tract. We report the case of a woman in her mid-50s who came to the gynaecology clinic with breathlessness and anaemia. Initial imaging with an abdominal ultrasound and pelvic MRI showed a 4×4×4 cm pelvic mass, suspected to be a wandering fibroid, and surgical removal was planned. During the surgery, the mass was unexpectedly found to come from the jejunum. The general surgery team was consulted, and a 4×4×4 cm mass outside the jejunum was removed, initial

The c.386A>C p.(Asn129Thr) variant in SMAD4 is likely to be pathogenic, causing Juvenile Polyposis Syndrome. A case report of a mosaic variant.

BACKGROUND: Juvenile Polyposis Syndrome (JPS) is a rare autosomal dominant hereditary disorder characterized by the development of multiple hamartomatous gastrointestinal polyps. Here, we present a case of JPS with a mosaic variant in SMAD4. METHODS: Exome sequencing TRIO analysis, using germline DNA from the biological mother and father along with the index case (IC). RESULTS: A 46-year-old male with no family history of cancer presented with chronic iron deficiency anemia and was diagnosed with massive gastric polyposis (≥100 polyps). At the age of 59, he underwent a total gastrectomy, rev…

Single-incision laparoscopic surgery for jejuno-jejunal intussusception caused by an angiolipomatous polyp in an adult: A case report.

RATIONALE: Small bowel intussusception in adults is rare but is more likely to occur in the presence of a lead point. Surgical intervention is necessary in most cases, even if there is successful nonsurgical reduction of the intussusception. PATIENT CONCERNS: A 54-year-old woman who was transferred to our emergency room with complaints of intermittent cramping pain of 4 days' duration. DIAGNOSIS: Abdominal contrast-enhanced computed tomography revealed a jejuno-jejunal intussusception due to an angiolipomatous polyp. INTERVENTION: A single-incision laparoscopic surgery (SILS) was performed

[Clinical classification of Peutz-Jeghers syndrome].

OBJECTIVE: To propose the clinical classification of Peutz-Jeghers syndrome (PJS). METHODS AND RESULTS: Retrospective analysis of 52 patients with PJS admitted in Nanfang Hospital from 1980 to 2003 was conducted. Twenty-four patients were found to have family history of PJS, who had a mean age of 19 years. In the PJS patients, the incidence of gastric polyps was 64.4%, colorectal polyps 76%, and small bowel polyps 95%. The number of polyps was above 50 in 19 of the 31 patients with gastric polyps, in 18 of the 38 patients with colorectal polyps, and in 8 of the 19 patients with small bowel po

Solitary rectal ulcer syndrome presenting with rectal prolapse, severe mucorrhea and eroded polypoid hyperplasia: case report and review of the literature.

A case of solitary rectal ulcer syndrome in a 36-year-old woman presenting with severe, persistent mucorrhea and eroded polypoid hyperplasia as the predominant clinical features, who was ultimately noted to have symptoms of rectal prolapse, is presented. Endoscopically, she had multiple (50 to 60) small, whitish polypoid lesions in the rectum that were initially misinterpreted as being a carpeted villous adenoma, juvenile polyposis or atypical proctitis. The lesions were treated with argon plasma coagulation with resolution, but a solitary rectal ulcer developed. The patient then admitted to a

Cronkhite-Canada syndrome associated with advanced rectal cancer treated by a subtotal colectomy: report of a case.

A 41-year-old man with Cronkhite-Canada syndrome presented with multiple juvenile polyps with hyperplastic and adenomatous changes throughout his stomach and entire colorectum. Dysgeusia was recognized and the degree of hypoproteinemia was remarkable. A barium enema study and colonofiberscopy also revealed an advanced cancer in the rectum. Chronic hepatitis B and membranous glomerulonephritis were also present. It was difficult to design a conservative protocol using steroids for the treatment of protein-loosing enteropathy because the patient was a hepatitis B virus carrier. As a result, a su

[A cause of intestinal intussusception in an adult].

Inflammatory fibroid polyp is a very rare tumor involving the gastrointestinal tract and especially the stomach and small bowel. It presents either as a solitary pedunculated or sessile lesion arising from the submucosa. Despite its large size and sometimes very infiltrating growth, the polyp is benign and has a good prognosis. The aetiology remains unknown but it probably represents a reactive proliferation similar in many respects to the granuloma pyogenicum. We report an ileal case which presented itself as an acute intussusception. The very typical and complete sonographic and tomodensitom

[Evaluation of colon diseases by using new method of extracorporeal ultrasonic examination during usual colon endoscopy].

Generally two types of ultrasonic colon examination method are carried out. One is extracorporeal ultrasonographic examination (US). And another is endoscopic ultrasonographic examination (EUS). US is technically easy but is difficult to take good image of colon diseases. EUS can give us good and sufficient informations. But it is technically difficult and gives much trouble to patients. During usual endoscopy, we have tried extracorporeal ultrasonic examination by filling de-aired water around the diseased colon area. With this new method, we could easily identify the disease and got much bet

[Neurenteric cyst of the mediastinum--case report and review of the literature].

A case of an infant with a mediastinal neurenteric cyst is used to review this rare entity. By definition a neurenteric cyst is the combination of an entodermal cyst with a vertebral dysplasia. Neurenteric cysts are located in the posterior mediastinum, preferentially on the right side. In a third of the patients these cysts are associated with malformations of CNS a./o. GI tract. They occur predominantly in males. The symptoms of a mediastinal mass usually become obvious during the first months of life. The therapy of choice is complete resection.

[An autopsy case of Cronkhite-Canada syndrome developing after hemicolectomy for colonic cancer].

This is the patient previously reported as Cronkhite-Canada syndrome developing after left hemicolectomy for cancer of the descending colon. Ultimately, he died at the age of 62 of metastatic colonic cancer 8 years after the onset of the syndrome. In this paper, the findings of autopsy as well as the follow-up examination including the right hemicolectomy are presented, with particular emphasis on the histology of the polyps. The polyps of the stomach and colon were composed of cysticalydilated glands with inflamed and edematous stroma. In addition, most of the colonic polyps showed adenomatou

[Uses of endoscopic high frequency diathermy: polypectomy--sphincterotomy (papillotomy)].

Several complications may arise as a consequence of monopolar high frequency diathermy applied for control of hemorrhage, polypectomy and papillotomy because of the specific conditions of current flow. Such complications can be burns, thermic organ damage or dysfunction of implanted cardiac pacemakers. Results and experience obtained during the last decade however show, that this endoscopic method of surgery is efficient and of low risk, if appropriate precautions are taken.

Total colectomy, rectal mucosectomy and ileoanal anastomosis for familial polyposis coli--use of tube ileostomy.

Radical cancer-preventive management of familial polyposis coli is total removal of the colon and rectum. We have carried out this procedure with ileoanostomy for seven young patients with familial polyposis coli. There was no significant morbidity and all ileoanal anastomoses were completed without any problems of leakage or abscesses. Side-to-end anastomosis is very effective in preventing complications of ileoanostomy. Simple tube-ileostomy is also effective and is recommended rather than loop ileostomy. Restoration of anal function was excellent except in one patient whose internal sphinct
